Doppler ultrasonography was used to examine 198 neck-vein systems (67 right, 131 left) in 148 patients. The vessel lumen is usually echo-free: the veins are supple, and the internal jugular and sometimes the subclavian veins can be compressed. The venous confluence is Y-shaped. Blood flow was symmetrical and biphasic in 57% of cases, continuous in 29% of cases, and monophasic in 13% of cases. The vasculitis of Henoch-Schönlein purpura may involve the scrotum and clinically mimic diseases requiring surgical intervention, such as testicular torsion or an incarcerated inguinal hernia. The ultrasonographic features include marked edema of the scrotal skin and contents with intact vascular flow in the testicles. These findings may suggest the correct diagnosis and prevent unnecessary surgical exploration.

Duplex ultrasonography (US) was performed in 23 patients with portal vein thrombosis. In 7 children and 16 adults, duplex US demonstrated intraluminal thrombi (4 cases), thrombus and periportal collaterals (2 cases), and cavernomatous transformation of the portal vein (17 cases). Ultrasonography (US) and serology (double diffusion for antigen 5 (dd5] were used for the screening of hydatid disease in 1,418 asymptomatic individuals from two provinces in Argentina. A total of 122 positive subjects were identified and in 120 of them lesions consistent with hydatid disease were observed at US (98.4%) while only 19 had positive dd5 tests (15.8%). Renal sonography is quite sensitive, although experience has demonstrated its lack of specificity. The abnormal renal sonogram should therefore suggest several differential possibilities. This article addresses the strengths and limitations of renal ultrasound. Although renal ultrasound was once thought to be tissue-specific in many instances, this has not proved to be true. It is thus incumbent on the sonographer to interpret the findings in light of the overall clinical setting and to recommend other imaging modalities as appropriate for confirmation of the final diagnosis.

While alcoholic cirrhosis is the most common cause of portal hypertension and associated gastrointestinal bleeding, portal vein occlusion can produce similar clinical manifestations. Ultrasonography is an accurate, noninvasive procedure for detecting portal vein thrombosis and distinguishing infrahepatic venous obstruction from intrahepatic obstruction due to cirrhosis.

High resolution real-time B-mode ultrasound scanning using a sterile 7.5 MHz transducer was performed during operations on the biliary tract. Ultrasound is useful for locating the common bile duct in the presence of acute inflammation and other abnormal anatomy, detecting small calculi in the gallbladder with a thickened wall, measuring the caliber of the common bile duct, and identifying calculi in the common bile duct. Intraoperative ultrasonography can permit earlier and more precise appraisal of operative findings and, thereby, decrease operative time.

Ultrasonic characteristics of 21 adrenal neoplasms (19 patients) examined by gray scale ultrasonography are reviewed. The significance of the adrenal-renal cleavage plane is discussed. In our experience the cleavage plane only has definite localization value when it separates the adrenal gland from the kidney on all ultrasonographic sections. Forty patients with pathologically-proved adrenal masses, and two patients with extra-adrenal pheochromocytomas, were examined with gray scale B-scan. The authors describe unusual echo patterns of adrenal masses due to necrosis or hemorrhage, variations in the effects of large adrenal masses on surrounding organs and vessels, and the differential diagnosis.
