Alkaline reflux gastritis is a clinical syndrome that results from a mucosal injury by duodenal contents. The operative management and clinical course of 17 patients treated for severe pancreatico-duodenal injuries from 1983 to 1990 was reviewed. The etiology of these injuries was gunshot wound in 15 patients; stab wound in 1 patient; and a motor vehicle accident in 1 patient. Selective proximal vagotomy (SPV) was performed in 435 patients whose age ranged from 16 to 80. The isolated variant of SPV was applied in 130 and SPV with a stomach draining operation was conducted in 305 patients; 320 patients were examined after the operation in follow-up periods of 7 years and more. Postoperative lethality was 0.6%. A recurrent ulcer was revealed in 4.3% of patients. The causes of the recurrence are indicated. The results were found to be excellent and good in 76%, satisfactory in 16.9%, and poor in 7.1% of the examined patients.

The present study investigated the integrity of the rat gastric mucosa after 6 hours of vagotomy without drainage. Transection vagotomy was employed to ensure complete gastric vagal denervation. Vagotomy without drainage produced gastric distension and mucosal injury confined to the glandular part. Plasma levels of gastrin, adrenocorticotropin (ACTH) and somatotropin (STH) were determined 24 hours after pylorus ligation and after pylorus ligation and vagotomy by radioimmunoassay. Pylorus ligation increases the serum levels of these hormones, while vagotomy inhibits the production of these ones. We conclude that the antiulcerogenetic effect of surgical vagotomy is closely related with the changes in the level of these hormones.

The effect of vagotomy on parietal microflora was studied in intact rats and rats with chronic experimental gastric ulcer using stereometry, as well as light, transmission and scanning electron microscopy. Vagotomy was found to increase the relative amount of parietal microflora on day 30 following denervation, especially in the duodenum and pylorus. Chronic gastric ulcers are also associated with a rise in the relative amount of parietal microflora.

The effect of selective proximal vagotomy (SPV) was examined in six dogs on the gastric emptying of solid food and the motility of the antrum and duodenum (basic rhythm, motility index, and contractile activity percentage). The ulcer disease of the duodenum with the symptoms of compensated and subcompensated stenosis is not considered to be a contraindication for instrumental pylorodilatation and proximal selective vagotomy. Pylorodilatation for correct indications gives a persistent elimination of stenosis under most physiological conditions as compared with those after proximal selective vagotomy with draining operation destructing the pylorus.
