Varicocele is a common finding in adult and adolescent males and may lead to potential infertility. Many methods are available to the practitioner for evaluation of the problem. The least invasive is color duplex imaging of the spermatic vein, to visualize the presence of reflux (backflow) in the vein, as well as to define flow characteristics. It is recommended that adolescent males be screened using this modality beginning at age 10-15 years if they appear to have physical findings indicating a varicocele or if there is a family history of male fertility problems or varicocele. DESIGN: A prospective controlled study in 88 randomly selected adolescents.

Testicular biopsies of 38 infertile males with varicocele were examined for vascular changes. Endothelial prominence, intimal fibrosis and increased amount of collagen in the media were present in veins, venules and capillaries. The small vessels were thickened with the deposition of homogeneous bluish pink material. The material stained green on Van Geison stain. The vascular changes were bilateral even in patients of unilateral varicocele. The severity of these changes was not related to tubular morphology.

Experimental varicoceles were created in adult male Wistar rats by partially occluding the left renal vein. Experimental varicocele was created by partial ligation of the left renal vein in rats 4, 7 and 12 weeks old to compare the effects of varicocele on the testis at different times of onset: prepuberty, adolescence and adulthood. A total of 40 infertile men with a normal physical examination underwent gonadal venography to determine the presence of a subclinical varicocele. We analyzed bilateral testicular biopsies of 188 infertile patients with idiopathic left varicocele. The left side had significant pathological tubular testicular changes compared to the right side, whereas in only 7 per cent of the patients tubular damage was more pronounced on the right side. The Leydig cell pathological score was identical in both testes in 95 per cent of the patients. X-ray findings on the internal testicular artery following abdominal aortography are described. A group of 32 patients with left-sided varicocele were examined. The course of internal testicular artery was visualized in 27 patients. In 5 sterile patients prolonged filling of the left internal testicular artery up to the venous phase of the abdominal aortography has been observed. The non-physiological courses of the artery are described.

Vascular lesions of the scrotum though most often varicoceles, can occasionally represent either benign or malignant tumors containing vascular elements. With the use of a case-control design, we have studied seminal cytology in 53 infertile men with large left varicoceles to determine whether they had a specific morphologic pattern. An equal number of infertile controls without varicoceles were matched for characteristics such as left testicular size, serum follicle-stimulating hormone levels, and sperm concentration to ensure similar degrees of testicular dysfunction. Ten normal men and 45 patients with varicocele (with or without sexual complaints) had evaluation of plasma androgen levels. Except for those with reduced libido, no differences were observed among the groups. These findings suggest a nonhormonal mechanism responsible for the erectile disturbances observed in some patients with varicocele.

The theory that insufficiency of the spermatic cord layers could be the etiological factor in the development of varicocele is reviewed critically. A prospective study of 40 patients operated upon for a left indirect inguinal hernia was done. The spermatic cord layers were excised as a routine part of the operation but combined manual and Doppler examinations revealed no postoperative reflux into the pampiniform plexus. We conclude that the spermatic cord layers probably do not have a major role in the pathogenesis of varicocele.
