Midterm outcomes of brachial arterio-arterial prosthetic loop as permanent hemodialysis access.

OBJECTIVE: The objective of this study was to report the midterm outcomes of the brachial arterio-arterial prosthetic loop (AAPL) as an alternative permanent vascular access in strictly selected patients with end-stage renal disease. METHODS: This single-center prospective observational study was conducted between January 2014 and June 2017 and included 89 brachial AAPL procedures. Primary, assisted primary, and secondary patency rates were calculated using Kaplan-Meier analysis. RESULTS: Exhausted peripheral veins were the most common indication for brachial AAPL. Patients were followed up

Brachio-basilic upper arm transposition fistulas vs. prosthetic brachio-axillary vascular access grafts-Which one is preferred for hemodialysis?

INTRODUCTION: There is still controversy on the use of brachio-basilic upper arm transposition fistula (BBAVF) and prosthetic brachio-axillary vascular access grafts (BAPTFE) in patients with no suitable cephalic veins for creating an autogenous brachio-cephalic fistula. METHODS: In a randomized controlled clinical trial, 60 hemodialysis patients who were not a suitable candidate for BCAVF were randomly assigned into two groups: BBAVF and BAPTFE. The patients were clinically followed up to 1 year and the patency rate and access-related complications were compared between the two groups. FIND

Complex recanalization techniques for complex femoro-popliteal lesions: how to optimize outcomes.

Complex lesions within the femoro-popliteal vascular territory, amongst others, include more than 15 cm long or heavily calcified occlusions of the superficial femoral artery (SFA) or total occlusions of the popliteal artery (PA). For those type-C/-D lesions TASC-II recommendations originating from 2007 advocate bypass surgery as the therapy of choice if the patient is a suitable candidate for this. Against the background of evolving endovascular techniques which often allow recanalization of even long and calcified lesions as well as improved patency rates after endovascular treatment of such

Exploratory Transcriptomic Profiling of Vein Tissue from Diabetic Patients Undergoing Arteriovenous Fistula Creation for Hemodialysis.

AIM: Arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis; however, its patency is limited, particularly in patients with diabetes mellitus (DM). This study aimed to obtain exploratory data on whether changes in the venous transcriptome are present at the time of AVF creation in patients with DM and whether these changes are related to the graft prognosis. METHODS: We conducted prospective transcriptome analyses using venous samples obtained during AVF creation from 22 patients (12 with DM and 10 without DM). RNA sequencing was performed, and clinical outcomes, includ

Basilic vein transposition fistulas versus prosthetic bridge grafts in patients with end-stage renal failure.

BACKGROUND: Basilic vein transposition fistulas (BVTFs) and prosthetic bridge grafts (PBGs) provide good vascular access for hemodialysis. To evaluate the patency and complication rates after arteriovenous fistula formation, a concurrent series of patients was reviewed. METHODS: Between September 2003 and September 2009, 147 hemodialysis access procedures were performed in 147 consecutive patients at Van Research and Training Hospital and Yuzuncu Yil University Hospital, Van, Turkey. All access procedures were planned on the basis of preoperative duplex scans of arm and forearm veins. Functio

Impact of evolving strategies for arteriovenous graft creation and management on patency outcomes.

BACKGROUND: Arteriovenous grafts (AVGs) are essential alternatives for hemodialysis patients who are unsuitable for arteriovenous fistulas (AVFs). As surgical expertise and monitoring strategies evolve, understanding the long-term performance and influencing factors of AVGs is crucial. METHODS: This retrospective study analyzed 980 patients who underwent AVG creation at a single center between 2014 and 2022. Patients were grouped into three periods based on practice evolution. Primary and secondary patency rates at 6, 12, and 24 months were assessed via Kaplan-Meier's analysis, and Cox regres

Does 111indium-platelet deposition predict patency in prosthetic arterial grafts?

The relationship between the rate of 111In-platelet deposition on vascular grafts and subsequent thrombosis has been examined in patients undergoing femoropopliteal by-pass. Sixty-seven patients undergoing femoropopliteal by-pass using vein, Dacron or PTFE were randomized to aspirin plus dipyridamole (ASA/DPM) or placebo. Autologous 111In-platelets were injected in the second postoperative week and Thrombogenicity Index (TI) calculated as the mean daily rise in the ratio of radioactivity graft/contralateral thigh. Graft patency was assessed to 1 year. Mean (+s.e.m.) TI at 1 week in 21 grafts t

The brachial artery-brachial vein fistula: expanding the possibilities for autogenous fistulae.

OBJECTIVE: The National Kidney Foundation Dialysis Outcomes and Quality Initiative recommends autogenous access for new dialysis procedures. The patient requiring hemodialysis with inadequate superficial arm veins represents a formidable challenge to the surgeon. Our objective is to describe results with an alternative access procedure, the autogenous brachial-brachial artery (ABBA) access in patients with inadequate superficial arm veins. METHODS: One surgeon created 163 new dialysis accesses in 122 patients during 40 consecutive months at a university hospital. There was 97% patient follow-

Is Distal Revascularization with Interval Ligation Still the Gold Standard?: A Comparative Meta-Analysis and Meta-Regression of Surgical Treatments of Dialysis Access-Associated Steal Syndrome.

KEY POINTS: Surgical treatment of dialysis access-associated steal syndrome achieves high symptom resolution rates, approaching 90% across observational studies. No significant differences were detected between distal revascularization with interval ligation and proximalization of the arterial inflow in symptom relief, complications, or primary patency. Current evidence supports an individualized, anatomy-driven approach rather than a single gold standard. BACKGROUND: Distal revascularization with interval ligation (DRIL) is widely regarded as the gold standard for dialysis access-associated

Prevailing insights into anastomotic angles of surgically created arteriovenous fistulas: a literature review.

OBJECTIVE: An arteriovenous fistula (AVF) is the most common type of vascular access, given its low infection rate, few complications, good patency potential, and long service life. Although preferred for most patients with chronic kidney disease (CKD), those undergoing dialysis continue to experience AVF surgical failures and complications, with 60% of AVFs failing to mature. The anastomotic angles chosen for AVF creation are usually ones that surgeons find easiest to manually control. At present, many sources have confirmed that variations in anastomotic angle culminate in differing geometri

Ultrasound evaluation of dialysis access-related distal ischaemia.

Dialysis access-related distal ischaemia is a rare yet potentially rather risky complication of haemodialysis angioaccess. Timely diagnosis is crucial to target both the goals of the access team: first of all to preserve the function of the hand ideally along with angioaccess patency. Unfortunately for some patients, urgent access ligation and central vein catheter insertion would be needed to save the hand. After a first clinical examination to determine the diagnostic suspicion, the ultrasound evaluation would provide nearly all the needed information to confirm the diagnosis and to determin
