We performed vasectomy and microsurgical repair of the vas deferens in 20 rats utilizing absorbable and non-absorbable 10-0 sutures. At 40 days the results of the repair procedure were evaluated by seminal fluid and fertility analyses, deferentography, and histologic examination of the anastomoses. The procedure failed in one case that presented occlusion of the spermatic cord from a granuloma.

Between 1973 and 1988, AVSC supported 1,516,478 female sterilizations and 401,856 vasectomies in 50 countries. Overall, 73 deaths were attributable to voluntary sterilization procedures (yielding mortality rates of 4.7 deaths per 100,000 female sterilizations and 0.5 per 100,000 vasectomies). Sperm antigenicity is demonstrated by the formation of sperm antibodies following vasectomy and by the possible occurrence of hypersensitivity reactions in women to seminal fluid. It may be that immunosuppressor factors in seminal plasma, as well as certain placental hormones, could explain the fact that sperm are not rejected immunologically in the female genital tract. The role of these different immune factors in male infertility and in the pathogenesis of AIDS is discussed.

The occurrence of an urethrovasocutaneous fistula is an extremely rare event. We report the first case of such a fistula in a patient with anterior urethral stricture. The patient had epididymo-orchitis preceding the occurrence of the fistula. Increased intravesical and intraurethral pressure during voiding and the patulous ejaculatory ducts were the predisposing factors in this case. The patient was managed successfully by visual internal urethrotomy, bilateral vasectomy and excision of the fistula.

Serum estrone, estradiol, estriol, progesterone, 17 alpha-hydroxyprogesterone, and testosterone were determined in the sera of 25 normal males and 25 patients with obstructive azoospermia. An increase in testosterone and estriol levels, and a decrease in other hormones were demonstrated in patients with obstructive azoospermia. These long-term effects of obstruction of the vas apparently also occur in comparable situations, for example, after vasectomy.

Absence of the vas deferens, especially if unilateral, should alert the clinician to an underlying renal anomaly, and further urological investigation is mandatory. If the vas is shown to be bilaterally absent in an infant, cystic fibrosis should be excluded. In the context of vasectomy, exploration of the palpably absent side should be unnecessary if intravenous urography detects a combined anomaly, although post-operative seminal analysis will still need to be performed.
