This case describes a middle-aged woman found to have tachycardia-induced cardiomyopathy secondary to chronic liothyronine overuse. Despite a non-specific presenting complaint, she was found clinically to have heart failure and evidence of fluid overload. ECG showed that she was in atrial flutter. A chest X-ray showed marked cardiomegaly and bilateral pleural effusions. Imaging of the abdomen showed a dilated inferior vena cava and evidence of systemic congestion. Trans-thoracic echocardiography showed a left ventricular ejection fraction of under 30%. Thyroid function tests showed a severely

Functional mitral regurgitation (FMR) can be broadly categorized into 2 main groups: ventricular and atrial, which often coexist. The former is secondary to left ventricular remodeling usually in the setting of heart failure with reduced ejection fraction or less frequently due to ischemic papillary muscle remodeling. Atrial FMR develops due to atrial and annular dilatation related to atrial fibrillation/flutter or from increased atrial pressures in the setting of heart failure with preserved ejection fraction. Guideline-directed medical therapy is the first step and prevails as the mainstay i

Computerized arrhythmia monitors recognize only a few of the significant arrhythmias and generally fail to detect arrhythmias of supraventricular origin. This is because conventional surface leads, which are sufficient for QRS recognition, are highly inadequate for automated P-wave detection. A new two-lead system, which includes a swallowable capsule-electrode for esophageal monitoring of atrial activity, is used in an on-line arrhythmia monitor. Three interval measurements (AA, AR and RR) and a QRS shape measurement provide the foundation for a detailed interpretation of each beat. Building

Having briefly touched upon the problem of terminology and classification of arrhythmias the authors consider the diagnosis and clinical evaluation of some disorders, including the extrasystole (the significance of the extrasystolic interval), ectopic arrhythmias from the region of the atrio-ventricular junction (with simultaneous consistent or transent disruption of the intraventricular conduction), isolated atrial tachycardia, some variants of auricular fibrillation, flutter paroxysms, paroxysms of ventricular tachycardia, with continued auricular fibrillation in particular, and also the ear

The case of a patient with narrow QRS-complex supraventricular tachycardia and atrial flutter is described. The 12-lead surface electrocardiogram (ECG) revealed sinus rhythm with ventricular pre-excitation and negative δ waves in leads II, III and aVF, indicating Wolff-Parkinson-White syndrome with a posteroseptal accessory pathway (AP). Coronary sinus angiography revealed the presence of a diverticulum near the coronary sinus ostium. The AP was successfully ablated using radiofrequency energy applied in the neck of the diverticulum, following several failed attempts at catheter ablation from

Narrow QRS tachycardia are supraventricular tachycardia, atrial tachycardia or junctional tachycardia. The diagnosis is done with the ECG, after identification of atrial activation, eventually after vagal manoeuvres, and transoesophageal recordings. DC Choc, atrial pacing, or antiarrhythmic drugs can do interruption. All atrial tachycardia must be anticoagulated prior regularisation. There is seldom an emergency for regularisation, as drugs can always slow the ventricular rate down. Prevention of relapses uses oral antiarrhythmic drugs, sometimes pacing. Most of these tachycardia can be cured

During the neonatal period the majority of cardiac tumors seen are rhabdomyomas, and the clinical manifestations are notably variable. Symptoms of rhabdomyoma depend on the presence of intracardiac obstructions, myocardial involvement, or rhythm disturbances. Most cardiac rhabdomyomas regress in follow-up, but some need medical/surgical intervention during the neonatal period as a result of resistant atrial or ventricular arrhythmia. The present case study is a description of an 8-day-old neonate with atrial bigeminy that caused nonconducted premature pulse-related bradycardia, and cardiac rha

Cardiac arrhythmias are an important aspect of fetal and neonatal medicine. Premature complexes of atrial or ventricular origin are the main cause of an irregular heart rhythm. The finding is typically unrelated to an identifiable cause and no treatment is required. Tachyarrhythmia most commonly relates to supraventricular reentrant tachycardia, atrial flutter, and sinus tachycardia. Several antiarrhythmic agents are available for the perinatal treatment of tachyarrhythmias. Enduring bradycardia may result from sinus node dysfunction, complete heart block and nonconducted atrial bigeminy as th

We describe the case of a 50-year-old woman with the clinical diagnosis of cardiomyopathy associated with supraventricular tachycardia refractory to pharmacological treatment. The totally irregular tachyarrhythmia was the result of different episodes of atrial tachycardia, atrial flutter and atrial fibrillation that could be identified in the surface ECG. These findings and the patient's symptoms were all caused by a single focal tachycardia originating from the left upper pulmonary vein. Ablation of this focus represented a curative antiarrhythmic therapy also restoring a normalized ventricul

Aortic insufficiency was suspected in a thin 1-year-old colt with a grade IV/V decrescendo holodiastolic murmur and a bounding arterial pulse. Echocardiographic findings (diastolic fluttering of the septal leaflet of the mitral valve, left ventricular volume overload, and incomplete aortic valve closure) were diagnostic for aortic valve insufficiency. Moderately thick fibrotic aortic valve leaflets were found at necropsy. Fenestrations were found in the aortic and pulmonic valve cusps. Congenital valvular disease may have led to aortic valvular insufficiency in this horse.

Electrophysiologic studies can be used in evaluating a large number of arrhythmias and conduction disturbances. These tests are extremely useful in the diagnosis and treatment of sick sinus syndrome, paroxysmal supraventricular tachycardias, atrial flutter, and AV conduction abnormalities. The decision to implant a permanent pacemaker in patients with a history of syncope also may be aided by the results of electrophysiologic studies. Finally, electrophysiologic studies have been used in the diagnosis and treatment of recurrent sustained ventricular tachycardia and in the evaluation of survivo

In a patient with patent ductus arteriosus, a pulmonary regurgitant murmur developed after the treatment of endocarditis. M-mode echocardiograms showed abnormal echo mass, characterized by fine diastolic fluttering, in the right ventricular outflow tract during diastole which were continuous with anterior pulmonary leaflet and thick pedunculated lesions attached to the anterior pulmonary leaflet were seen to be moving freely by cross-sectional echocardiography. At autopsy the destruction of the pulmonary valve was confirmed. This study suggests that diagnosis of flail pulmonary valve is feasib

Background: Chronic lymphocytic leukaemia (CLL) is the most common form of leukaemia among adults, particularly in Western nations. The introduction of Bruton's tyrosine kinase (BTK) inhibitors as a treatment of CLL, namely, ibrutinib, which is a first-generation BTK inhibitor, has significantly improved the treatment landscape for CLL. However, ibrutinib has been associated with an increased risk of atrial fibrillation (AF) and hypertension. Real-world studies that compare the cardiovascular safety of ibrutinib with bendamustine plus anti-CD20 monoclonal antibody are not widely available. Met

We describe our experience with 1000 electric cardioversions performed at the emergency ward in the Hospital of Cardiology y Neumology, National Medical Center I.M.S.S. The objectives are: 1. Report our experience. 2. Investigate if digitalis treatment should be discontinued before the procedure. 3. Determine if all patients should be on anticoagulant therapy for elective cardioversion. 4. Indicate the optimal anesthetic drug with minimal side effects. A therapeutic procedure was performed in 73% of our cases and an elective one in the remaining 27%. Patients were grouped as ischemic heart dis

The diagnosis and treatment of cardiac dysrhythmias answers the following four questions: Is the patient stable? Is the rate fast or slow? Are the ventricular complexes wide or narrow? Is the rhythm regular or irregular? The most common narrow complex regular tachycardias are sinus tachycardia, atrial flutter, atrial tachycardia that blocks, and paroxysmal supraventricular tachycardia. Carotid sinus massage is useful in differentiation. Irregular narrow-complex tachycardias are usually atrial fibrillation. An ultra-rapid wide-complex or polymorphous irregular tachycardia is likely to be atrial

Endocardial electrocardiograms permit the analysis of auricular depolarization and contribute to the study of ventricular activation. Direct recording of auriculograms from the right atrium and indirect recording of the left auriculograms through the coronary sinus and pulmonary artery, show the different auricular activation pathways in common atrial flutter, atrial tachycardia and in case of retrograde atrial depolarization (via the AV node, septal or parietal accessory pathway). His bundle electrograms obtained via the femoral vein or, less easy, via the brachial vein, has introduced an ess

The authors examined the electrocardiographic features of ventricular fibrillation and other ventricular paroxysms induced by electric shock in the guinea pig. The analysis of the electrocardiographic patterns, as well as the vectorial study of the cardiac potentials during repeated episodes of ventricular "anarchy" induced in more than forty animals, showed the following data: 1. following electric shock, ventricular paroxysms appear, featuring paroxysmal tachycardia, typical or impure flutter, tachycardia--either "torsade de pointe" or "spindle" type, and typical ventricular fibrillation. BACKGROUND: Realistic modeling of cardiac inter-beat (RR) intervals is highly desirable for basic research in cardiac electrophysiology, clinical management of heart diseases, and developing signal processing tools for ECG analysis.
METHODS: We present an open source computer model that is capable to generate realistic time series of RR intervals in both physiologic and pathologic conditions. Detailed model structure and the software implementation are described.
RESULTS: Examples are provided on how to use this model to generate RR intervals in atrial fibrillation with ventricular pacing, n

We report the case of a 35-yr-old patient who presented with high fever and chills. He had undergone a patch closure of the ventricular septal defect 18 yr before. One year later, a VVI pacemaker was implanted via the right subclavian vein because of complete heart block. Nine years after that, a new VVI pacemaker with another right ventricular electrode was inserted controlaterally and the old pacing lead was abandoned. Trans-thoracic and trans-esophageal echocardiogram identified the pacemaker lead in the right ventricle (RV) attaching hyperechoic materials and also a fluttering round hypere

Adenosine has become the preferred acute treatment for common types of supraventricular tachycardia because of its efficacy and safety. There have been a few reports of serious proarrhythmic events associated with its use, including the induction of atrial fibrillation in adult patients. Three instances of adenosine-induced atrial proarrhythmia (two atrial fibrillation and one atrial flutter) have been observed in children with manifest or concealed Wolff-Parkinson-White syndrome at the Hospital for Sick Children, Toronto, Ontario since 1990, which indicates a previously unreported risk of atr
