Total Clinical Course and Autopsy Findings of Left Ventricular Outflow Tract Obstruction Due to Sigmoid Septum: Histologically Proven Isolated Basal Septal Hypertrophy.

We herein report the total course and autopsy findings of a woman who complained of chest discomfort and had plasma B-type natriuretic peptide 43 pg/mL and left ventricular outflow tract obstruction (with a resting pressure gradient of 181 mmHg) due to sigmoid septum at 73 years of age. Betaxolol and verapamil decreased her pressure gradient to 14 mmHg, but the pressure gradient (101 mmHg) again worsened. The betaxolol dose was increased and cibenzoline was added, resulting in a pressure gradient ≤21 mmHg. An autopsy was performed after death from a urinary tract infection at 80 years of age.

The rastelli procedure for transposition of the great arteries: resection of the infundibular septum diminishes recurrent left ventricular outflow tract obstruction risk.

BACKGROUND: The Rastelli procedure is the standard surgical treatment of d-transposition of great arteries (d-TGA), ventricular septal defect (VSD), and pulmonary stenosis. Late morbidity is significant due to recurrent left ventricular outflow obstruction (LVOTO), early conduit obstruction, and arrhythmias, with troublesome late mortality. To avoid recurrent LVOTO, we routinely enlarge the VSD and resect the infundibular septum before LV baffling to the aorta. We examined the efficacy of this approach in mitigating recurrent LVOTO risk. METHODS: Late echocardiographic and time-related clinic

[Modified Konno procedure for a child with left ventricular outflow tract obstruction after repair of atrioventricular septal defect: a transaortic transpulmonary approach].

A one-year-old infant underwent repair of atrioventricular septal defect with common orifice. About 2 years later, echocardiography revealed a left ventricular outflow tract obstruction for the first time. Because of progression of the obstructive lesion, a modified Konno procedure through a transaortic transpulmonary approach was later performed at 8 years old, and the postoperative course was uneventful. This is a useful procedure for left ventricular outflow tract obstruction with normal aortic valve and aortic annulus, because it can both preserve a native aortic valve and dose not necessi

Takotsubo syndrome complicated by cardiogenic shock due to left ventricular outflow tract obstruction, acute mitral regurgitation, and atrial fibrillation: a case report.

BACKGROUND: Although Takotsubo syndrome (TTS) is generally considered a benign disease, recent reports showed the incidence of cardiogenic shock due to left ventricular outflow tract obstruction (LVOTO), mitral regurgitation (MR), and primary pump failure was estimated to be 6-20%. CASE SUMMARY: A 78-year-old woman presented with chest pain and cold sweats 2 days after surgery for lung cancer. Acute coronary syndrome was suspected based on her symptoms, electrocardiography, transthoracic echocardiography (TTE), and laboratory data; thus, emergency catheterization was performed. Normal coronar

Myocardial Mechanics Parameters That Predict Left Ventricular Outflow Tract Obstruction in Patients With Hypertrophic Cardiomyopathy: A Cardiovascular Magnetic Resonance Feature Tracking Analysis.

OBJECTIVE: To identify left ventricular (LV) myocardial mechanics predictors of LV outflow tract obstruction (LVOTO) in patients with hypertrophic cardiomyopathy (HCM). METHODS: Thirty-nine adults with HCM and 21 controls underwent cardiovascular magnetic resonance. The feature tracking (FT) analysis results of HCM patients with and without LVOTO and controls were compared. RESULTS: Global radial strain measured on the short-axis slice (GRS-SAX) (odds ratio [OR], 1.09; 95% confidence interval [CI], 1.02-1.15; P < 0.01), global longitudinal strain measured on the long-axis slice (GLS-LAX) (OR

Associations Between Multiple Circulating Biomarkers and the Presence of Atrial Fibrillation in Hypertrophic Cardiomyopathy with or Without Left Ventricular Outflow Tract Obstruction.

Atrial fibrillation (AF) is the most common arrhythmia in patients with hypertrophic cardiomyopathy (HCM). Data regarding the correlations of biomarkers and AF in HCM patients are rather limited. We sought to explore the associations between the presence of AF and circulating biomarkers reflecting cardiovascular function (N-terminal pro-brain natriuretic peptide, NT-pro BNP), endothelial function (big endothelin-1, big ET-1), inflammation (high-sensitivity C-reactive protein), and myocardial damage (cardiac troponin I, cTnI) in HCM patients with and without left ventricular outflow tract obstr

Role of noninvasive imaging modalities to better understand the mechanism of left ventricular outflow tract obstruction and tailored lesion-specific treatment options.

Left ventricular outflow tract obstruction (LVOTO) has important prognostic implications in patients with hypertrophic cardiomyopathy (HCM). Echocardiography provides critical information to establish LVOTO as a unique feature of HCM by demonstrating heterogeneity of hypertrophy patterns and the systolic anterior motion of mitral leaflets, resulting in mitral-septal contact. Currently, 2 treatment strategies are available for reduction of muscle mass to relieve LVOTO: surgical myectomy and percutaneous alcohol septal ablation. Both focus on mechanical removal of the hypertrophied septum. Howev

Risk factors in the origin of congenital left-ventricular outflow-tract obstruction defects of the heart: a population-based case-control study.

The aim of our project was to study possible etiological factors in the origin of congenital heart defects (CHDs) because in the majority of patients the underlying causes are unclear. Cases with different CHD entities as homogeneously as possible were planned for evaluation in the population-based large data set of the Hungarian Case Control Surveillance of Congenital Abnormalities. Dead or surgically corrected 302 live-born cases with different types of left-ventricular outflow tract obstructive defects (LVOT, i.e., valvular aortic stenosis 76, hypoplastic left heart syndrome 76, coarctation

Accessory mitral valve associated with aortic and mitral regurgitation and left ventricular outflow tract obstruction in an elderly patient: a case report.

A 65-year-old man was admitted to our hospital because of exertional dyspnea. Transthoracic and transesophageal echocardiography showed a parachute-like structure measuring 20 x 16 mm, which projected into the left ventricular outflow tract (LVOT) and passed through the aortic valve in systole, and prolapsed back into the left ventricular cavity in diastole. Moderate aortic and mitral regurgitation were also observed, as well as LVOT obstruction with a peak gradient of 30 mmHg. There were no other congenital cardiac abnormalities. In addition, real-time three-dimensional transthoracic echocard

Fabry disease: percutaneous transluminal septal myocardial ablation markedly improved symptomatic left ventricular hypertrophy and outflow tract obstruction in a classically affected male.

Fabry disease (alpha-galactosidase A deficiency) is an X-linked recessive lysosomal storage disease in which left ventricular hypertrophy (LVH) is common, and if severe, may mimic hypertrophic obstructive cardiomyopathy. Alcohol-induced percutaneous transluminal septal myocardial ablation (PTSMA) has been used as a safe and effective method to alleviate LVH obstruction in patients with hypertrophic obstructive cardiomyopathy (HCM). We describe a case of a classically affected Fabry 53-year-old male with symptomatic HCM (NYHA class III with exertional angina) who was treated with PTSMA. The pro

D-Transposition of the Great Arteries with Ventricular Septal Defect and Left Ventricular Outflow Tract Obstruction (D-TGA/VSD/LVOTO): A Survey of Perceptions, Preferences, and Experience.

Several procedures have been introduced to manage this complex congenital heart malformation. It is not clear that any one approach is best for all patients. Decision-making for a given patient may be influenced by multiple patient factors. There is little doubt that surgeon and institutional experience and preference also play a role. We have focused our survey on three procedures, i.e., the Rastelli, Nikaidoh (aortic root translocation), and réparation à l'étageventriculaire (REV) procedures. The questionnaire was directed to all Congenital Heart Surgeon Society member surgeons. A total of 6

Half-turned truncal switch operation for dextro-transposition of the great arteries with ventricular septal defect and left ventricular outflow tract obstruction and an abnormal coronary pattern.

We describe a surgical technique for a half-turned truncal switch operation in a 5-year-old child with dextro-transposition of the great arteries (D-TGA), a ventricular septal defect, a left ventricular outflow tract obstruction and a complex coronary pattern. The benefit of the half-turned truncal switch is the creation of haemodynamically superior biventricular outflow tracts and the maximal use of an autologous pulmonary valve in the right ventricular outflow tract, thereby avoiding the right ventricular-pulmonary artery conduit.

Arterial switch and modified Konno procedure with detachment of the aortic root for transposition of the great arteries and left ventricular outflow tract obstruction.

The arterial switch operation for the transposition of the great arteries accompanied by a ventricular septal defect and posterior malalignment of the outlet septum is a surgical challenge. We describe a new surgical technique in 2 patients having this pathology. The surgical technique consists of the arterial switch operation and a modified Konno procedure through partial detachment of the semilunar valve of the right ventricle as in the Ross procedure.

[Obstruction of the left ventricular outflow tract due to an anomalous muscle bundle and an accessory tissue on the mitral valve associated with mitral stenosis].

This paper details a rare cause of subaortic obstruction, a discrete membrane combined with an anomalous muscle bundle and an accessory tissue on the mitral valve. A 13-year-old boy was referred to our hospital for surgical relief of subaortic obstruction and mitral stenosis. He had received two additional operations, the first was for coarctation of aorta when 5 years old and the second was for discrete subaortic stenosis when 6 years old. Recatheterization at this time revealed an peak systolic gradient of 44 mmHg across the left ventricular outflow tract, and an peak diastolic gradient of 7
