Minimally invasive video-assisted mitral valve surgery: our lessons after a 4-year experience.

BACKGROUND: Right thoracotomy is a well known alternative to median sternotomy to gain access to the left atrium. To avoid the potential drawbacks associated with sternotomy coupled to the desire for a smaller scar and a more rapid rehabilitation in young and active patients, we investigated the purported advantages in patients undergoing video-assisted Port-Access mitral valve surgery. METHODS: Between February 1997 and November 2000, 175 patients (94 men, 81 women) with a mean age of 60 years (range 25 to 84) underwent either Port-Access mitral valve repair (n = 117) or replacement (n = 57)

Primary spontaneous pneumothorax treated by video assisted thoracoscopic surgery--results of intermediate follow up.

BACKGROUND: Primary spontaneous pneumothorax (PSP) is a common clinical condition associated with high recurrence if no definitive treatment is given and could be life threatening if left untreated. The best treatment for this condition, however, remains controversial. AIMS: A prospective study of patients with PSP treated by video assisted thoracoscopic surgery (VATS) after a mean follow up of 16 months. METHODS: Between September 1992 and April 1994, 114 VATS procedures were performed on 110 patients with PSP (including four patients with bilateral presentation) by one surgical team from a

[Introduction of Minimally Invasive Thoracoscopic Surgery for the Anterior Mediastinum;Subxiphoid Video-assisted Thoracoscopic Thymectomy and Robot-assisted Thymectomy].

In Japan, robot-assisted surgery for malignant lung tumors, benign mediastinal tumors, and malignant mediastinal tumors has been covered by the national health insurance since 2018. Hence, the number of domestic robotic surgical procedures is increasing. Recently, we introduced endoscopic surgery such as video-assisted thoracoscopic surgery (VATS) via a subxiphoidal approach for thymectomy. Here, we compared VATS and robotic surgery via a subxiphoidal approach in terms of clinical factors. During the study period, 5 consecutive patients who underwent robotic thymectomy and 24 patients who unde

Quality of Lymph Node Dissection in Lung Cancer Surgery: A Comparative Analysis of Robotic-Assisted Versus Video-Assisted Thoracic Surgery Using Novel Pathological Criteria.

BACKGROUND: Lymph node dissection is essential for lung cancer staging and treatment planning. This study compares the extent and quality of lymph node dissection between robot-assisted thoracic surgery (RATS) and video-assisted thoracic surgery (VATS). METHODS: In this prospective cohort study, 40 patients undergoing oncologic lobectomy via RATS (n = 20) or VATS (n = 20) were included. We assessed the number of explored lymph node stations, dissected nodes, and microscopic integrity criteria. RESULTS: RATS resulted in a higher median number of explored stations (5 vs. 4; p = 0.0375) and res

Endoscopic breast surgery: where are we now and what might the future hold for video-assisted breast surgery?

Endoscopic surgery has been extensively used for many surgical conditions and has gained acceptance as an alternative and less invasive approach to open surgery. However, minimal access endoscopic techniques have yet to be translated into mainstream clinical practice in breast surgery. More recently, technical innovations have made it feasible to conduct endoscopic breast cancer resection, with or without breast reconstruction, through wounds inconspicuously hidden in the axilla and periareolar region. Several clinical trials have now been conducted to demonstrate technical feasibility, assess

Endobronchial Fluoroscopy-Guided Management of Persistent Air Leak After Video-Assisted Thoracoscopic Surgery (VATS)-Assisted Deroofing of Pleural Hydatid Cysts.

Persistent air leak (PAL) is a challenging postoperative complication after thoracic surgery. We present the case of a woman in her 30s with hydatid disease involving the abdomen and lungs, who developed PAL following video-assisted thoracoscopic surgery (VATS)-assisted deroofing of left pleural hydatid cysts. Conservative management with chest tube drainage and negative suction failed to resolve the air leak. On postoperative day seven, bronchoscopy with balloon occlusion localized the leak to the upper lobe of the left lung. A combined bronchoscopy and fluoroscopy-guided approach was used fo

Comparative effectiveness of robotic and video assisted thoracic surgery for lung cancer across key outcomes.

Robotic-assisted thoracic surgery (RATS) and video-assisted thoracoscopic surgery (VATS) are widely used for minimally invasive resection of pulmonary tumors, but they differ in technical capabilities and resource demands. We conducted a PRISMA-based systematic review and meta-analysis of randomized and cohort studies identified by searching PubMed, Embase, Web of Science, Scopus, and the Cochrane Library through June 2025. Across 25 studies including 41,417 patients, RATS was associated with lower conversion to thoracotomy rates and less blood loss, more extensive lymphadenectomy, higher R0 r

New Trends in Uniportal Video-Assisted Thoracoscopic Surgery for Primary Spontaneous Pneumothorax: A Narrative Review.

Background: Minimally invasive thoracic surgery has advanced since the introduction of multiportal video-assisted thoracoscopic surgery (mVATS) in 1991. Primary spontaneous pneumothorax (PSP) is an ideal condition for refining minimally invasive techniques owing to its straightforward procedures and predictable bullae distributions. Methods: Uniportal VATS (uVATS), which involves a single incision, is an alternative to mVATS, offering reduced postoperative pain, lower paresthesia rates, and comparable recurrence outcomes. This review explores two main uVATS approaches: intercostal and subxipho
