An unusual case of transvaginal impalement is presented. At initial exploration the broad ligament and vagina were repaired. When bile drained from suction catheters reexploration revealed an injury to the duodenum and diaphragm. Wounds that penetrate the vagina may injure nonpelvic viscera. Vaginal penetrating wounds should be managed like other abdominal penetrating injuries.

The authors reports a case of cervical and abdominal subcutaneous emphysema, associated to pneumoretroperitoneum and pneumomediastinum, with no free air in the abdominal cavity, secondary to perforated diverticulitis of the sigmoid colon. We comment the rarity of this clinical presentation, the physiopathologic mechanisms of subcutaneous air presence in cases of non instrumental perforation of abdominal viscera and the ominous significance of the subcutaneous emphysema in such perforations.

Asplenia syndrome is a rare congenital complex of splenic agenesis, cardiac malformation, and malposition of the abdominal viscera. Prolonged functional survival is very uncommon, with death usually caused by severe infection or congestive heart failure. A 21-year-old man with typical asplenia syndrome who is currently asymptomatic and in his third year of college is described.

An 11 year old girl died unexpectedly as a result of spontaneous subarachnoid hemorrhage originating from a ruptured giant fusiform aneurysm of the basilar artery. The aneurysm had developed as a long standing complication of arterial fibromuscular dysplasia. This discussion describes the light and electron microscopic findings in the aneurysm and the histologic examination of abnormalities in the muscular arteries of the viscera.

Each of two Filipino agricultural workers experienced coccidioidal infection, characterized by a solitary skin lesion, by regional lymphadenopathy, and by a benign clinical course, without evidence of involvement of the lungs or other viscera. It is believed that these are cases of primary cutaneous coccidioidomycosis incurred under "natural" conditions. A similar interpretation seems acceptable for at least six other reported cases.

Sternal cleft is a rare congenital condition associated with unprotected viscera and respiratory failure. We present a case of a sternal cleft, absent right clavicle, and congenital diaphragmatic paresis in a neonate. The sternum was closed primarily, and the patient was taken back for plication of the diaphragm. At 3 years follow-up, the patient is free from complications.

A 38-year-old man developed an infection that led to necrosis of the abdominal wall after perforation of a gastric ulcer. A split-thickness skin graft over the abdominal viscera was used to achieve temporary primary closure. After full systemic and local stabilisation, it was reconstructed with a free innervated latissimus dorsi myocutaneous flap.

A human embryo (Carnegie stage 21) with tracheoesophageal malformations (esophageal atresia and tracheoesophageal fistula) and anomalies at the caudal end of the embryo (anorectal atresia, rectovesical fistula, vertebral and notochordal defects, and agenesis of the metanephros) was studied. Other anomalies observed were: absence of right umbilical artery, fusion of spinal ganglia, and absence of cloacal outlet of mesonephric ducts. The possible pathogenesis of these associated malformations is discussed.
