Cementless total hip arthroplasty in advanced tuberculosis of the hip.

PURPOSE: The use of total hip arthroplasty (THA) to treat advanced tuberculous arthritis, particularly during the active phase, is challenging. The aim of this study was to evaluate the efficacy of cementless THA for advanced hip joint tuberculosis. METHODS: This study reviewed 32 patients (mean age at surgery, 49.4 years [range, 24-79 years]) who underwent cementless THA between 2007 and 2012. All patients were diagnosed with advanced tuberculosis of the hip on the basis of clinical manifestations, radiographic findings, and histological examination. All procedures were performed by a single

Reversed shoulder arthroplasty with inversed bearing materials: 2-year clinical and radiographic results in 101 patients.

INTRODUCTION: This study documents 2-year clinical and radiographic results following reversed total shoulder arthroplasty using a novel prosthesis with inverted bearing materials (polyethylene glenoid; metal humeral component). This design was intended to avoid massive PE abrasion on the humeral side. Therefore, we predicted a lack of subsequent osteolysis-induced exacerbation of scapular notching, and because of other design features and modified operating technique a reduced notching rate. MATERIALS AND METHODS: An ongoing, prospective, international, multicenter study of patients implante

Magnetic Oculomotor Prosthetics for Acquired Nystagmus.

PURPOSE: Acquired nystagmus, a highly symptomatic consequence of damage to the substrates of oculomotor control, often is resistant to pharmacotherapy. Although heterogeneous in its neural cause, its expression is unified at the effector-the eye muscles themselves-where physical damping of the oscillation offers an alternative approach. Because direct surgical fixation would immobilize the globe, action at a distance is required to damp the oscillation at the point of fixation, allowing unhindered gaze shifts at other times. Implementing this idea magnetically, herein we describe the successfu

Rotationplasty as a salvage of failed primary limb reconstruction: up to date review and case report.

We report a case of a 15-year-old girl who is a known case for proximal focal femoral deficiency. She underwent several surgical lengthening procedures. Her course was complicated by femur nonunion, knee joint dislocation, stiff knee, and final lower limb discrepancy of over 15 cm. Rotationplasty was performed to salvage her previous surgical failures and restore her function. Surgery was uneventful and the patient was able to walk with the prosthesis in a few months after index surgery. Although the outcome of rotationplasty is visually unusual, for some children restored function and physica

A "clean" technique for managing bleeding duodenal ulcer with a new aortic prosthesis: endoscopic-assisted repair of bleeding duodenal ulcer.

Aortic graft infection is one of the most dreaded surgical complications. In the perioperative patient with fresh aortic prosthesis, this is a particularly complex problem. Opening the bowel changes an operation to a "clean-contaminated" or "contaminated" case. This increases the risk of all infectious complications in the patient. Theoretically, our method of repair reduces the risk of infection by eliminating the duodenotomy. The direct visualization with the endoscope replaces the need to open the potentially contaminated bowel and reduces the risk of bacterial translocation and bacteremia.

Clinical guidelines for the management of keratoconus patients with gas permeable contact lenses based on expert consensus and available evidence.

PURPOSE OF REVIEW: Fitting rigid gas permeable contact lenses (RGP CLs) in keratoconic patients is the most common visual rehabilitation option to improve patients' quality of life, but require long patient and practitioner chair time. The purpose of this review is to provide evidence-based clinical practice guidelines (CPG) in the management of patients with keratoconus with RGP CLs. RECENT FINDINGS: An extensive literature review from 1990 to 2017 identified 354 potentially relevant publications. Fifty-two articles were reviewed and included in the CPG. An international expert panel of eigh

Automated and subjective refraction with monofocal, multifocal, and EDOF intraocular lenses: review.

Automated refraction (Scheiner principle) is universally used to start a visual examination. Although the results are reliable in eyes implanted with monofocal intraocular lenses (IOLs), they may be less precise with multifocal (mIOL) or extended depth-of-focus (EDOF) IOLs and can even indicate a refractive error that does not clinically exist. Autorefractor results with monofocal, multifocal, and EDOF IOLs were investigated through literature search analyzing the papers reporting the difference between automated and clinical refraction. The average difference ranged between -0.50 diopter (D)

Latest Development in Extended Depth-of-Focus Intraocular Lenses: An Update.

In recent years, there has been an overwhelming influx of different types of intraocular lenses (IOLs) as treatment for presbyopia. The extended depth-of-focus (EDOF) technology creates a single elongated focal point to enhance depth of focus, in contrast to the multiple foci of multifocal (MF) lenses. In this way, the EDOF lenses aim to reduce photic phenomena, glare, and halos, which have been reported in MF IOLs. A potential disadvantage of this is a blur due to decreased retinal image quality when the amount of the aberrations is increased excessively. Multifocality and EDOF characteristic

The direct influence of retinal degeneration on electrical stimulation efficacy: Significant implications for retinal prostheses.

Photoreceptor loss and inner retinal network remodeling severely impacts the ability of retinal prosthetic devices to create artificial vision. We developed a computational model of a degenerating retina based on rodent data and tested its response to retinal electrical stimulation. This model includes detailed network connectivity and diverse neural intrinsic properties, capable of exploring how the degenerated retina influences the performance of electrical stimulation during the degeneration process. Our model suggests the possibility of quantitatively modulating retinal ON and OFF pathways

In Vitro Cytotoxicity of D18 and Y6 as Potential Organic Photovoltaic Materials for Retinal Prostheses.

Millions of people worldwide are diagnosed with retinal dystrophies such as retinitis pigmentosa and age-related macular degeneration. A retinal prosthesis using organic photovoltaic (OPV) semiconductors is a promising therapeutic device to restore vision to patients at the late onset of the disease. However, an appropriate cytotoxicity approach has to be employed on the OPV materials before using them as retinal implants. In this study, we followed ISO standards to assess the cytotoxicity of D18, Y6, PFN-Br and PDIN individually, and as mixtures of D18/Y6, D18/Y6/PFN-Br and D18/Y6/PDIN. These

Artificial iris implantation in congenital aniridia: A systematic review.

Congenital aniridia is a rare, panocular disorder with a main phenotypic characteristic of a partial or complete absence of the iris existing alongside other ocular morbidities such as cataract, keratopathy, optic nerve and foveal hypoplasia, and nystagmus. The iris abnormality, however, often leads to symptoms such as photophobia, glare, and decreased visual acuity, as well as cosmetic dissatisfaction. Current management options for the iris deficit include colored iris contact lenses, corneal tattooing, and tinted contact lenses. Symptoms arising from small iris defects can be resolved with

[Basics of Practical Contact Lens Fitting (Part 1: Materials, Indications and Differences in Optics Compared to Glasses)].

The great potential of contact lenses for improving vision in all forms of corneal irregularities, e. g. keratoconus, after keratoplasty, after keratitis, is in danger of being forgotten. Especially by the younger generation of ophthalmologists and opticians there is a risk that the skill of fitting contact lenses professionally will be lost. There are many situations in which the practicing ophthalmologist should recognize that the contact lens can be a useful option for the visual rehabilitation of his patient. In this article, we provide a summary of "The basics of practical contact lens fi

[Presbyopia Correction with Intraocular Lenses].

Surgical correction of presbyopia can be performed by refractive lens exchange, replacing the lens with a monofocal, a multifocal or an extended-depth-of-focus (EDOF) intraocular lens (IOL). Option 1, a monofocal IOL, must be combined with monovision, otherwise presbyopia cannot be corrected. Monovision in this context means one eye for distance and one eye targeted for - 1.5 D. With this concept glasses are still required for near (30 - 40 cm) and for driving, but intermediate distances and everyday life is usually possible without glasses. Option 2, a multifocal IOL, results in spectacle ind

Diffractive corneal inlay for presbyopia.

A conceptually new type of corneal inlays for a customized treatment of presbyopia is presented. The diffractive inlay consists on a small aperture disc having an array of micro-holes distributed inside the open zones of a Fresnel zone plate. In this way, the central hole of the disc lets pass the zero order diffraction and produces an extension of the depth of far focus of the eye, while the diffracted light through the holes in the periphery produce the near focus. Additionally, the micro-holes in the inlay surface fulfill the essential requirement of allowing the flow of nutrients through i
