BACKGROUND/AIMS: A relationship between fatty liver and lung function impairment has been identified, and both are independently associated … decline in lung function. RESULTS: Among 6,149 individuals, the annual rates of decline in forced vital capacity (FVC) and forced expiratory volume measured in the first second of exhalation (FEV1) were higher in the worsened/persistent steatosis group than in the normal/improved steatosis group. In multivariable analysis, persistent or worsened status of fatt …

Spirometry for the diagnosis of airway obstruction in patients with risk factors for COPD: the GOLD and lower limit of normal criteria.

OBJECTIVE: The identification of persistent airway obstruction is key to making a diagnosis of COPD. The GOLD guidelines suggest a fixed criterion-a post-bronchodilator FEV1/FVC ratio < 70%-to define obstruction, although other guidelines suggest that a post-bronchodilator FEV1/FVC ratio < the lower limit of normal (LLN) is the most accurate criterion. METHODS: This was an observational study of individuals ≥ 40 years of age with risk factors for COPD who were referred to our pulmonary function laboratory for spirometry. Respiratory symptoms were also recorded. We calculated the prevalence of

Daytime predictors of nocturnal hypercapnic hypoventilation in children with neuromuscular disorders.

OBJECTIVES: To examine objective daytime predictors of nocturnal hypercapnic hypoventilation (NHH) and identify a forced vital capacity (FVC) z-score cut off that predicts NHH using the 2012 Global Lung Function Initiative (GLI) reference equations in pediatric neuromuscular patients. DESIGN: Single-centre retrospective medical record review. SETTING: Tertiary pediatric hospital in Australia. PATIENTS: Children (<18 years old) with a neuromuscular disorder (NMD) who had a diagnostic sleep study over a 5-year period. RESULTS: Fifty children were included, median age 11.9 years (interquartil…

Does the nitrogen single-breath washout test contribute to detecting pulmonary involvement in rheumatoid arthritis? A pilot study.

OBJECTIVE: There has been growing interest in studying small airway disease through measures of ventilation distribution, thanks to the resurgence of the nitrogen single-breath washout (N2SBW) test. Therefore, this study evaluated the contribution of the N2SBW test to the detection of pulmonary involvement in patients with rheumatoid arthritis (RA). RESULTS: Twenty-one patients with RA underwent clinical evaluation, pulmonary function tests (PFTs), including the N2SBW test, and computed tomography (CT). The main tomographic findings were air trapping and bronchiectasis (57.1% and 23.8% of cas

Association of anxiety sensitivity-physical concerns and FVC with dyspnea severity in sarcoidosis.

OBJECTIVE: The purpose of the study was to evaluate the relationship of an objective functional lung parameter (FVC) and a subjective psychological factor (physical symptom concerns) with dyspnea in sarcoidosis. Dyspnea constitutes one of the most common and burdensome symptoms in sarcoidosis, yet little is known about its mechanisms and, in particular, psychological. METHOD: A total of 107 hospitalized sarcoidosis patients (Female=50, Mage=45.3years) volunteered to take part in the correlational research study. Participants underwent spirometry and completed the MRC Dyspnea Scale and the Anx

Therapeutic implications of the pathophysiology of COPD.

This review examines 18 studies published > or =30 yrs ago. They show that the earliest manifestation of chronic obstructive pulmonary disease (COPD) is an increase in residual volume suggesting that the natural history of COPD is a progressive increase in gas trapping with a decreasing vital capacity (VC). The reduction in VC forces the forced expiratory volume in 1 s to decline with it. This is aggravated by rapid shallow breathing leading to dynamic hyperinflation. The earlier studies show that this is energetically opposite to a minimal work or force pattern and is responsible for dyspnoea…

[Lymphangioleiomyomatosis in a post-menopausal women].

Lymphangioleiomyomatosis is a rare disease characterised by pulmonary cysts with abnormal proliferation in the lymphatic system. It occurs almost exclusively in women of fertile age due to a hormonal influence, for this reason it is extremely rare in post-menopausal patients. In these cases it is usually associated to hormone replacement therapies. It is known that this diseases is strongly associated with other conditions, such as tuberous sclerosis and renal angiomyolipomas. We present a case of a post-menopausal patient suffering from lymphangioleiomyomatosis, with no history of hormone the

[Celiac disease associated with systemic sclerosis].

Systemic scleroderma (SS) affects the connective tissue, with involvement of multiple organs. Digestive system involvement occurs in 50% of patients. SS is frequently associated with other autoimmune diseases. Celiac disease (CD) is an autoimmune disorder that affects the digestive system and is trigged by gluten intake. These two diseases share some HLA antigens. We describe the case of a woman with a diagnosis of SS who presented with weight loss. CD was diagnosed. Because the literature on this topic is scarce, this case is compared with a prior review and some similarities were found. Diag

PARTICIPANTS: Fifteen patients (mean age 63 +/- 4 years) with idiopathic PD (Group A) and 15 healthy matched controls (Group B) were studied. All patients were under treatment with L-Dopa/Carbidopa and classified according to the UPDRS motor scale: 8 had mild disease (UPDRS < 12), 6 moderate (UPDRS: 12-22) and 1 severe (UPDRS > 22).
