[Current trends in the treatment of rhegmatogenous retinal detachment and perioperative positioning strategies in Germany: results of a retina.net survey].

BACKGROUND: Rhegmatogenous retinal detachment (RRD) is an ophthalmological emergency with an increasing incidence in Germany. Despite established methods, perioperative positioning is important to improve outcomes and avoid complications but may be challenging, particularly in immobile patients. An analysis of the current trends in surgical treatment and perioperative positioning practices for RRD in Germany compared to historical data from 2018 is essential. MATERIAL AND METHODS: An online survey with five hypothetical cases of acute RRD was distributed to vitreoretinal surgeons via the reti

Isolated retinal telangiectatic masses.

The clinical findings of 20 patients with an isolated peripheral retinal mass secondary to retinal telangiectasis were reviewed. The important clinical features were subretinal exudation, vitreous haemorrhage and retinal detachment. No associated systemic abnormalities were identified. Early results suggest that cryotherapy is effective for the reduction of subretinal exudation but not for macular oedema. Patients with extensive retinal detachment complicated by vitreous haemorrhage and epiretinal membrane formation were treated with vitreoretinal surgery.

Elevated intraocular pressure in a child with Stickler syndrome after scleral buckle surgery for retinal detachment.

Stickler syndrome, a rare connective tissue disorder, presents with a broad spectrum of ocular manifestations, including myopia, vitreoretinal degeneration, glaucoma, and retinal detachment. While extensive data exists on the treatment outcomes of retinal detachment repair in Stickler syndrome, the potential risks associated with acute postoperative intraocular pressure elevation remain underexplored. We report the case of a 10-year-old boy with Stickler syndrome who underwent scleral buckling surgery and subsequently experienced ocular hypertension.

A modified technique of pupillary stretching.

In eyes undergoing vitreoretinal surgery up to now dilatation of a miotic pupil by the pupillary stretching technique could be performed only in aphakic eyes. Now a new needle with a tiny eye at its tip has been developed. With the help of this needle, 4 iris loop sutures can be led around the iris only passing the anterior chamber. This permits pupillary stretching also in phakic eyes. The modified technique was used on 3 phakic and 2 aphakic eyes.

A Rare Complication Causing Another Rare Complication: Corneal Dellen with Infiltration Secondary to Subconjunctival Silicon Oil.

The usage of silicon oil (SO) in vitreoretinal surgery is associated with potential complications. One such complication is the extravasation of SO into the subconjunctival space through open sclerotomies. Subconjunctival SO (SCSO) can cause irregularities in the ocular surface which predisposes to complications like corneal dellen. This corneal dellen can get infected in rare situations. SCSO needs to be removed to stabilize the ocular surface. We present a case of corneal dellen with infiltration which developed as a consequence of SCSO and its management.

Neurodegeneration and neuroprotection in retinal detachment.

Retinal detachment (RD) occurs when the neurosensory retina separates from the retinal pigment epithelium (RPE). The most frequent type, rhegmatogenous retinal detachment (RRD), is caused by full-thickness retinal breaks that typically arise from vitreoretinal traction during posterior vitreous detachment. These breaks permit fluid to enter the subretinal space, leading to acute and often severe visual loss. Key risk factors include aging, myopia, pseudophakia, and the occurrence of posterior vitreous detachment. RRD constitutes a surgical emergency. Modern vitreoretinal procedures achieve hig

Volumetric assessment of the space between the posterior hyaloid and internal limiting membrane using SD-OCT.

The purpose of this study was to measure, for clinical and surgical utility, the distance between the posterior hyaloid and the internal limiting membrane using a spectral domain optical-coherence tomography (SD-OCT) device. Eight eyes of eight patients with subclinical partial posterior vitreous detachment (PPVD) with vitreomacular traction syndrome were examined with SD-OCT. To quantify the vitreoretinal distance, the basic applications of SD-OCT (Spectralis HRA+OCT Heidelberg Engineering) were modified. We conducted a topographic study of the entire posterior pole by defining new maps, each
