Sleep-related breathing and sleep-wake disturbances in ischemic stroke.

BACKGROUND: Sleep-related breathing disturbances (SDB) and sleep-wake disturbances (SWD) are often neglected in stroke patients. Recent stud … and adaptive servoventilation may be used for central SDB. SWD including insomnia, disturbances of wakefulness (hypersomnia, excessive daytime sleepiness, fatigue), sleep-related movement disorders (restless legs syndrome, periodic limb movements during sleep), and parasomnias (REM sleep behavior disorder) are found in 10%-50% of patients. SWD are associated …

In the wake of progress--ethical problems of renal failure treated by dialysis.

The ability of dialysis in the 1960s to prolong the lives of patients with terminal renal failure created a running and changing debate in bioethics. The first issue was the selection of suitable individuals to be treated in the limited facilities available. Progress in technology was ahead of any medical or ethical consensus. The costs of treatment soon exceeded the will or means of health funders to pay, creating a system of covert rationing and fiscal fudges with which physicians reluctantly colluded. Evidence, epidemiology and public opinion eventually prevailed and the UK now provides a s

Esophageal emergencies: things that will wake you from a sound sleep.

Esophageal emergencies are a common problem facing practicing gastroenterologists and it is important to know what therapies are indicated for different situations. Patients ingesting caustic agents should be monitored intensively for signs of perforation and ultimately for signs of stricture development. Foreign bodies impacted in the esophagus should be removed promptly to prevent perforation. Although esophageal perforations are generally managed surgically, conservative management of localized perforations has become more common especially with improved antibiotics and the use of nonsurgic

Public reliance on risk communication channels in the wake of a cryptosporidium outbreak.

In the spring of 1993, about 39% of Milwaukee-area residents suffered through a nationally publicized illness brought about by cryptosporidium, a parasite that had infested the metropolitan drinking water supply. Our study, based on a telephone survey of 610 local adult residents, indicates that worry about becoming ill in the future with cryptosporidiosis relates more strongly and consistently to public reliance on, and use of, media for cryptosporidium information than do a range of risk perception and experience variables. We propose that more studies should take an audience-centered approa

A contribution to the genetics of febrile seizures: waking and sleep EEG in siblings.

Waking and sleep EEGs were recorded in 67 siblings of 52 patients with febrile seizures (FS). Epileptic activity was noted in at least 1 sibling for 28 of the 52 patients (53.8%). Epileptic discharges were noted in 33 (49.2%) of the 67 siblings. Thirty-two siblings had 3-4 Hz spike wave complexes, and 1 sibling had independent centrotemporal spike foci (rolandic foci). Epileptic activity was noted exclusively in the waking state in only 3%, in waking and sleep in 31.3%, and only in sleep in 14.9%. The greatest number of epileptic discharges occurred in waking during hyperventatilation (32.8%)

[Hemodynamics in waking rats undergoing testicular exposure to elevated temperature].

Changes of systemic and regional hemodynamic parameters of the rat during heating of the lower part of the body in 41 degrees C water and after removing of the testis from abdominal cavity, were measured with labeled microsphere technique. Experimental cryptorchism reduced the blood pressure due to a decrease in total peripheral resistance. Hyperthermia of the testis increased the heart rate. Heating of the scrotal region led to an increase in the blood flow that did not, however, reach the value of the blood flow in testis of the control unrestrained rats, thus suggesting an inadequate blood

Sleep-wake complaints in patients with sleep-related respiratory disturbances.

The relation of sleep complaint to sleep continuity and respiratory disturbance was studied by comparing 2 series of patients with sleep apnea, one group complaining of insomnia and the other of excessive daytime sleepiness. On polysomnographic evaluation, patients with insomnia complaints had fewer and shorter, primarily central, apneas that had little hypoxemic effects. Patients with excessive sleepiness complaints had more and longer, primarily obstructive, apneas that produced significant hypoxemia. Sleep of the excessively sleepy patients was lighter and longer, whereas that of the patien

Effects of locus coeruleus lesions upon sleeping and waking in the rabbit.

The effects of radiofrequency lesions in the region of the locus coeruleus (LC) upon sleep--waking states and behaviors were investigated in chronically implanted New Zealand White rabbits. Polygraphic recordings were taken prior to and at 5- and 14-day intervals following lesioning. In animals exhibiting absence of paradoxical sleep and the presence of bizarre motor behavior, additional recordings were taken 30 days postlesion. Prelesion sleep-wakefulness pattern data were comparable to those previously observed in intact rabbits, including the recently reported absence of sustained PS-relate…

[Research progress of sleep-wake neural circuit mediated by hypothalamus].

The hypothalamus is a critical central hub for regulating sleep-wake patterns in organisms, maintaining the balance between daily activity a … optic area (POA) and its subregions, and dorsomedial hypothalamic nucleus (DMH) can induce sleep or wakefulness through distinct circuit mechanisms. At the same time, the limitations of the traditional sleep-wake flip-flop model are discussed, along with the supplements and hypotheses. Multiple hypothalamic nuclei form extensive connections with other brain r …

Balancing Ischemia and Hemorrhage After Tenecteplase in Wake-Up Stroke.

BACKGROUND: Contemporary stroke management assumes discrete clinical scenarios with clear separation between ischemic and hemorrhagic risks. When these processes coexist, guideline recommendations may be insufficient or conflicting. CASE SUMMARY: A 74-year-old woman with a wake-up stroke received computed tomography perfusion-guided thrombolysis. She developed hemorrhagic transformation, followed by intermediate-risk pulmonary embolism (PE) requiring anticoagulation. Despite treatment, recurrent intracranial large-vessel occlusion occurred; mechanical thrombectomy achieved complete reperfusio

Sleep-wake dysregulation and altered melatonin in neurofibromatosis type 1.

STUDY OBJECTIVES: Neurofibromatosis type 1 (NF1) is a common autosomal dominant disorder resulting from pathogenic variants in the NF1 gene. Preclinical studies suggest the NF1 gene regulates sleep and circadian processes, yet objective human data are limited. Characterizing sleep disturbances, sleep-related biomarkers, and their relationships to neurodevelopmental outcomes, may identify potential novel therapeutic targets in children with NF1. This study aimed to compare sleep-wake profiles, circadian rhythmicity, and melatonin production in children with NF1 compared to typically developing

EEG microstates during wake and NREM sleep in insomnia disorder.

Insomnia disorder (ID) is prevalent and debilitating, yet its neurophysiological basis remains unclear. Abnormalities in temporal parameters of electroencephalography (EEG) microstates have been linked to diverse neuropsychiatric disorders, but their dynamics across wakefulness and non-rapid eye movement (NREM) sleep in chronic insomnia remain unexplored. In this study, EEG microstate dynamics across wakefulness and NREM sleep were examined in adults with ID (n = 33) and healthy controls (HC; n = 29). Simultaneous EEG and functional magnetic resonance imaging (fMRI) were acquired during noctur…

Body-Brain Integration: The Lower Brainstem in Sleep-Wake Regulation.

The lower brainstem is a key hub linking visceral physiology with the regulation of brain states. This review synthesizes recent findings demonstrating how several nuclei within this region-including the nucleus of the solitary tract (NST), parabrachial nucleus (PBN), and other medullary circuits-function as an integrated network that couples sleep-wake regulation to the body's homeostatic demands. The NST serves as a central gateway, translating cardiovascular, immune, and digestive signals into a sleep drive, whereas the PBN plays a pivotal role in processing threat-related inputs to promote

[Chinese expert consensus on the clinical application of wake-promoting agents].

Wake-promoting agents mainly excite the cerebral cortex and promote wakefulness. They are clinically used to treat excessive daytime sleepiness (EDS) caused by narcolepsy and other diseases, or to maintain alertness during special tasks. In recent years, a variety of wake-promoting agents have been approved for the treatment of EDS related to narcolepsy, obstructive sleep apnea and other diseases at home and abroad. However, a dedicated expert consensus on the clinical application of wake-promoting agents remains lacking. To facilitate the rational and standardized clinical application of wake…

The frequency and predictors of hematoma expansion in wake-up intracerebral hemorrhage.

BACKGROUND: Hematoma expansion (HE) is a key determinant of poor outcomes in intracerebral hemorrhage (ICH), but its frequency and predictors in wake-up ICH remain insufficiently characterized. This study aimed to compare HE between wake-up and clear-onset ICH and to identify predictors of HE in wake-up ICH. METHODS: We analyzed patients with spontaneous ICH from a prospective cohort between August 2022 and September 2025. The primary outcome was HE defined as an absolute increase in volume > 6 mL or > 33% from baseline on follow-up computed tomography (CT). Poor outcome was defined as a modi

Leukemia's Wake-Up Call - A Case Report of a Patient's Journey.

A 26-year-old male complained of fever, right neck swelling, anorexia, and weight loss for 4 months. The patient noticed a right swelling in the root of the neck that progressed from 1 × 1 cm to 5 × 6 cm. We reported a Kikuchi-like acute lymphoblastic leukemia (ALL) case. Three 3 × 3 cm lymph nodes were found in the right cervical area at IIB and III levels, firm, mobile, and nontender. Petechiae and purpura on the left upper limb, wet purpura on the palate, a positive Hess test, and temperature spikes were present. The patient is responding well to ALL treatment.
