A V0 core neuronal circuit for inspiration.

Breathing in mammals relies on permanent rhythmic and bilaterally synchronized contractions of inspiratory pump muscles. These motor drives emerge from interactions between critical sets of brainstem neurons whose origins and synaptic ordered organization remain obscure. Here, we show, using a virus-based transsynaptic tracing strategy from the diaphragm muscle in the mouse, that the principal inspiratory premotor neurons share V0 identity with, and are connected by, neurons of the preBötzinger complex that paces inspiration. Deleting the commissural projections of V0s results in left-right de

Characterization of periodic and non-periodic breathing pattern in chronic heart failure patients.

Periodic breathing (PB) has a high prevalence in chronic heart failure (CHF) patients with mild to moderate symptoms and poor ventricular function. This work proposes the analysis and characterization of the respiratory pattern to identify periodic breathing pattern (PB) and non-periodic breathing pattern (nPB) through the respiratory flow signal. The respiratory pattern analysis is based on the extraction and the study of the flow envelope signal. The flow envelope signal is modelled by an autoregressive model (AR) whose coefficients would characterize the respiratory pattern of each group. T

Oronasal breathing during exercise.

The shift from nasal to oronasal breathing (ONBS) has been observed on 73 subjects with two independent methods. A first group of 63 subjects exercising on a bicycle ergometer at increasing work load (98--196 W) has been observed. On 35 subjects the highest value of ventilation attained with nasal breathing was 40.2 +/- 9.41 . min-1 S.D. Ten subjects breathed through the mouth at all loads, while 5 never opened the mouth. On 13 subjects it was not possible to make reliable measurements. On a second group of 10 subjects utilizing a different techniques which did not need a face mask, the ventil

Alteration by hyperoxia of ventilatory dynamics during sinusoidal work.

The effects of hyperoxia on ventilatory and gas exchange dynamics were studied utilizing sinusoidal work rate forcings. Five subjects exercised on 14 occasions on a cycle ergometer for 30 min with a sinusoidally varying work load. Tests were performed at seven frequencies of work load during air or 100% O2 inspiration. From the breath-by-breath responses to these tests, dynamic characteristics were analyzed by extracting the mean level, amplitude of oscillation, and phase lag for each six variables with digital computer techniques. Calculation of the time constant (tau) of the ventilatory resp

Sensory-mechanical effects of a dual bronchodilator and its anticholinergic component in COPD.

This randomized, double-blind, crossover study examined the physiological rationale for using a dual long-acting bronchodilator (umeclidinium/vilanterol (UME/VIL)) versus its muscarinic-antagonist component (UME) as treatment for dyspnea and exercise intolerance in moderate COPD. After each 4-week treatment period, subjects performed pulmonary function and symptom-limited constant-work rate cycling tests with diaphragm electromyogram (EMGdi), esophageal (Pes), gastric (Pga) and transdiaphragmatic (Pdi) pressure measurements. Fourteen subjects completed the study. Both treatments improved spiro

Stability of the human respiratory control system. II. Analysis of a three-dimensional delay state-space model.

A number of mathematical models of the human respiratory control system have been developed since 1940 to study a wide range of features of this complex system. Among them, periodic breathing (including Cheyne-Stokes respiration and apneustic breathing) is a collection of regular but involuntary breathing patterns that have important medical implications. The hypothesis that periodic breathing is the result of delay in the feedback signals to the respiratory control system has been studied since the work of Grodins et al. in the early 1950's [1]. The purpose of this paper is to study the stabi

Assessment of Cardiorespiratory Interactions during Apneic Events in Sleep via Fuzzy Kernel Measures of Information Dynamics.

Apnea and other breathing-related disorders have been linked to the development of hypertension or impairments of the cardiovascular, cognitive or metabolic systems. The combined assessment of multiple physiological signals acquired during sleep is of fundamental importance for providing additional insights about breathing disorder events and the associated impairments. In this work, we apply information-theoretic measures to describe the joint dynamics of cardiorespiratory physiological processes in a large group of patients reporting repeated episodes of hypopneas, apneas (central, obstructi

A data-driven model to estimate breathing-induced intra-trunk blood shifts during exercise.

The pressure swings generated by the respiratory muscles induce blood shifts (Vbs) between the trunk and the extremities. Vbs varies with swing amplitude and breathing pattern and can reach sizable volumes. Although Vbs was successfully explored using double-body plethysmography, the extent of intra-trunk blood shifting (between abdomen and thorax, VbsIT) remains to be quantified. We here present an electrical model of the cardiovascular system that allows to derive quantitative estimates of breath-by-breath VbsIT. We first validated the model with experimental data collected from healthy part

[Heart failure and central respiratory dysregulation. Cheyne-Stokes respiration during sleep in advanced left heart failure].

Central sleep apnoea, especially Cheyne-Stokes respiration, is found in 45 to 66% of patients with congestive heart failure (CHF) in functional classes NYHA II to IV. Cheyne-Stokes breathing cycles are characterised by central apnoeas, followed by a crescendo--like increase of tidal volume into hyperventilation and a subsequent decline of tidal volume, ending in another central apnoea. Cheyne-Stokes respiration has been shown to be a poor prognostic factor for patients with CHF. Apnoeas and hypopnoeas cause marked oxygen desaturation and rises of carbon dioxide concentrations in the blood. The

Optimal ventilatory patterns in periodic breathing.

The goal of this study was to determine whether periodic breathing (PB), which is highly prevalent during sleep at high altitudes, imposes physiological penalties on the respiratory system in the absence of any accompanying disease. Using a computer model of respiratory gas exchange, we compared the effects of a variety of PB patterns on the chemical and mechanical costs of breathing to those resulting from regular tidal breathing. Although PB produced considerable fluctuation in arterial blood gas tensions, for the same cycle-averaged ventilation, higher arterial oxygen saturation and lower a

Elevation of spectral components of electrodermal activity precedes central nervous system oxygen toxicity symptoms in divers.

BACKGROUND: Oxygen-rich breathing mixtures up to 100% are used in some underwater diving operations for several reasons. Breathing elevated oxygen partial pressures (PO2) increases the risk of developing central nervous system oxygen toxicity (CNS-OT) which could impair performance or result in a seizure and subsequent drowning. We aimed to study the dynamics of the electrodermal activity (EDA) and heart rate (HR) while breathing elevated PO2 in the hyperbaric environment (HBO2) as a possible means to predict impending CNS-OT. METHODS: EDA is recorded during 50 subject exposures (26 subjects)

Patient-ventilator interaction during acute lung injury, and the role of spontaneous breathing: part 2: airway pressure release ventilation.

Airway pressure release ventilation (APRV) and bi-level positive airway pressure (BIPAP) are proposed to reduce patient work of breathing (WOB) sufficiently and to obviate issues related to patient-ventilator synchrony, so that spontaneous breathing can be maintained throughout the course of acute lung injury (ALI). Thus, APRV/BIPAP should reduce requirements for sedation and muscle paralysis, and thereby reduce the duration of mechanical ventilation. Only 17 human, animal, or lung-model studies have examined these claims, either directly or indirectly. Most did not target patients with ALI. S…

The effect of long term ventilatory support on hemodynamics in children with spinal muscle atrophy (SMA) type II.

BACKGROUND: Bi-level Positive Airway Pressure (Bi-PAP) treatment improves breathing efficiency and ventilation in children with SMA type II, but the effects of positive airway pressure swings on heart rate (HR) and blood pressure are not known. Here we studied children with SMA to determine whether Bi-PAP administered during sleep is associated with changes in hemodynamics. METHODS: Ten children aged 8-12 years on long term Bi-PAP therapy were evaluated during a routine overnight sleep study. We recorded HR, ECG, thoraco-abdominal movements and blood gases. Blood pressure was estimated indire

[Electrical activity of the brain in different ventilation mode and its relationship with pattern of quite breathing and forced expiratory indices].

The influence of hyperventilation on electric activity of the brain has traditionally been the subject of study of physiologists. The role of the brain in the processes of adaptation to cold air inhalation is still studied not enough. The aim of the work is to define the peculiarities of brain reaction to isocapnic hyperventilation with cold air and identify correlation between its electric activity against different modes of ventilation and the pattern of breathing and parameters of the forced expiration. 21 healthy volunteers went through complex functional examination. Qualitative differenc

Sleep-Disordered Breathing and Central Respiratory Control in Children: A Comprehensive Review.

Background/Objectives: Sleep-disordered breathing (SDB) is a primary concern in children's health. Research suggests that repeated oxygen drops during sleep-common in SDB-may harm the brainstem's breathing control centres. This damage likely occurs through oxidative stress, inflammation, and cell death, which weaken the brain's ability to regulate breathing. Over time, these effects could lead to functional changes (e.g., disrupted chemical signalling) and physical damage in critical brain regions, creating a cycle of unstable breathing. However, much of this evidence comes from animal or lab

Breathing with neuromuscular disease: Does compensatory plasticity in the motor drive to breathe offer a potential therapeutic target in muscular dystrophy?

Duchenne muscular dystrophy is a fatal neuromuscular disease associated with respiratory-related morbidity and mortality. Herein, we review recent work by our group exploring deficits and compensation in the respiratory control network governing respiratory homeostasis in a pre-clinical model of DMD, the mdx mouse. Deficits at multiple sites of the network provide considerable challenges to respiratory control. However, our work has also revealed evidence of compensatory neuroplasticity in the motor drive to breathe enhancing diaphragm muscle activity during increased chemical drive. The findi
