Early experience with the snorkel technique for juxtarenal aneurysms.

OBJECTIVE: The lack of readily available branched and fenestrated endovascular aneurysm repair (EVAR) options has created an opportunity for creative deployment of endograft components to treat juxtarenal aneurysms. We present our early experience with "snorkel" or "chimney" techniques in the endovascular management of complex aortic aneurysms. METHODS: We retrospectively reviewed planned snorkel procedures for juxtarenal aneurysms performed from September 2009 to August 2011. Our standardized technique included axillary or brachial cutdown for delivery of covered snorkel stents and mostly pe

Endovascular repair of complicated chronic distal aortic dissections: intermediate outcomes and complications.

INTRODUCTION: Patients with chronic distal aortic dissection (CDAD) remain at high risk for late aorta-related events and reinterventions, and the ideal management strategy remains undefined. Open surgical procedures carry morbidity, but scant data for thoracic endovascular aortic repair (TEVAR) of CDAD exist. This study reports our intermediate-term results with TEVAR for complicated CDAD. METHODS: All cases of TEVAR for complicated (aortic growth, malperfusion, intractable pain) CDAD at our institution between 2000 and 2007 were retrospectively reviewed. Demographic information, indications

Incidence and outcomes after infolding or collapse of thoracic stent grafts.

OBJECTIVE: Device-related complications in the thoracic aorta are partly due to the unavoidable proximal angulation and increased flow-related forces. The present study evaluated the incidence, predictors, and outcome of the complication of infolding with the GORE TAG thoracic endoprosthesis (TAG device) to better understand the factors that might help predict these events. METHODS: We reviewed all complaints reported to W. L. Gore and Associates (Flagstaff, Ariz) related to device infolding after the use of the GORE TAG device on or before December 2008. Events related to device infolding we

Hybrid repair of thoracic aortic lesions for zone 0 and 1 in high-risk patients.

PURPOSE: Some patients with aortic arch or descending thoracic aorta pathologies are not suited for open repair because of comorbidities that may increase their risk of procedural complications or death. Endovascular approaches may also be difficult when there are inadequate proximal landing zones in the aortic arch. We report our experience using rerouting techniques with bypass, stenting of the branches, or a combination of both to create a landing area in zones 0 and 1 of the aortic arch. METHODS: Since November 2002, thoracic aortic endoluminal grafts were placed in 38 patients in whom th

[Triple rule-out computed tomography in emergency departments].

In the emergency department patients with chest pain play an important role because the underlying and concealed diseases can be life-threatening. The complaints are often nonspecific and patients also have different risk profiles. In patients in the emergency department with nonspecific chest pain a"1-stop strategy" for elucidation of the three main organs lung, hear and aorta with the 4 main differential diagnoses (aortic dissection, ruptured aortic aneurysm, pulmonary embolism and coronary heart disease) is desirable. Technical advances in computed tomography (CT) in recent years make this

Thoracic endovascular repair as a safe management strategy for aortobronchial fistulas.

OBJECTIVES: This study assessed the safety and efficacy of thoracic endovascular aortic repair (TEVAR) in the management of aortobronchial fistulas. METHODS: A retrospective review was performed at Emory University Hospital to identify all patients who presented with an aortobronchial fistula. The diagnosis was based on clinical, radiologic, and bronchoscopic findings. Patients who underwent TEVAR as definitive management of these fistulas were identified. Demographics, history of thoracic aorta pathology or intervention, type and number of endografts used, need for reoperation, and clinical

Apico-aortic conduit for aortic stenosis in a patient with chronic thoracic wall infection.

We report a case of an apico-aortic bioprothesis-valved conduit for a 70-year-old-woman with symptomatic, severe aortic stenosis and severe calcification of the ascending aorta. She had a history of mastectomy and radiation therapy for breast cancer and was suffering from radiodermatitis and chronic thoracic wall infection. Transthoracic echocardiography showed severe aortic valve stenosis with heavy calcification and high aortic valve pressure gradients. In patients with a chronically infected thoracic wall, median sternotomy is considered to be a high risk procedure, resulting in postoperati

Familial aortic aneurysm and dissection due to transforming growth factor-beta receptor 2 mutation.

This report describes the clinical course and management of a patient with Loeys-Dietz syndrome (LDS) type 2. In 2003, a 31-year-old male was diagnosed with acute aortic dissection type B; in the following six years he underwent multiple surgical and endovascular aortic procedures at different thoracic and abdominal levels secondary to progressive enlargement of both the non-dissected thoracic aorta and the false lumen distal to the left subclavian artery. LDS is characterized by arterial tortuosity and aneurysms as a result of heterozygous mutations in genes encoding transforming growth facto

Potential risk for intra-aortic balloon-induced obstruction to the celiac axis or the renal artery in the Asian population.

To evaluate the potential risk for intra-aortic balloon (IAB)-induced obstruction to the celiac axis (CA) or the renal artery (RA) when IAB size is chosen according to patient height and the tip is placed 2 cm distal to the origin of the left subclavian artery (LSCA), the computed tomography images of 150 Asian adults were reviewed to identify the distance from the LSCA to the CA (LSCA-CA) and to the RA (LSCA-RA). The diameter of the aorta at the level of the T9 vertebra, CA, and RA was also measured. The length and caliber of the IAB was selected according to two manufacturer's recommendation

Dynamics of homemade aortic endografts: in vivo study in humans with computed tomography scanner modeling.

BACKGROUND: Several cases of aortic endograft rupture have been described. In most cases, they stem from component wear and perforation of the graft, leading to leakage. Friction of the stents on the graft can cause abrasion and perforate the textile. This friction results from movements inside the endograft implanted in the aorta exposed to blood flow, arterial pressure, and the movements of the aorta itself. METHODS: To study in vivo the movements of homemade stent grafts (HMSGs) designed and constructed by the surgeons at La Pitié Salpêtrière Hospital (Paris), the displacements of the meta

Feasibility and limitations of an automated 2D-3D rigid image registration system for complex endovascular aortic procedures.

PURPOSE: To examine the feasibility of an automated 2-dimensional (2D) to 3- dimensional (3D) image registration system to simplify the navigational challenges faced in complex endovascular aortic procedures. METHODS: An automated 2D-3D image registration system was used to overlay pre-acquired 3D computed tomography images onto fluoroscopy images taken during endovascular aneurysm repair. Errors between the 3D overlay and digital subtraction angiograms were measured and correlated with aortic neck angulation. A mean discrepancy < or =3 mm was considered clinically acceptable. RESULTS: There

[Role of endovascular therapy for redo surgery in patients after aortoiliac aneurysm exclusion].

INTRODUCTION: Redo surgery or reintervention following conventional or endovascular aortoiliac reconstruction often requires exclusion of new aneurysms. In the present study the potentials of endovascular management of such lesions are investigated. METHODS: All patients with endovascular reoperation for of newly developed aortoiliac aneurysms were identified from a prospectively run data-base. The indications and results of endovascular therapy were analysed retrospectively. In detail, data were analysed for the type of original operation, interval until and kind of reoperation, and results
