[Biological osteosynthesis in children and adolescents].

Minimally invasive osteosynthesis techniques are very suitable for treatment of unstable fractures in children. Because of little additional soft tissue trauma, these techniques are also called biological osteosynthesis. They allow fracture treatment in children and adolescents without any harm to the growth plate. Angular deformities can be corrected due to further growth. Kirschner wires, threaded or unthreaded, elastic intramedullary rods or external fixators are most commonly used for osteosynthesis. After successful closed reduction of the fragments under x-ray amplifier control, the impl

Fractures of the base of the middle phalanx treated by a dynamic external fixation device.

Intraarticular fractures of the base of the middle phalanx are difficult to treat, especially if the fragments are comminuted. A spring dynamic traction device is described and the results in the first 14 patients presented. Patient acceptance has been high and the results comparable to other methods of treatment.

Transcutaneous Osseointegrated Prosthesis Systems (TOPS) for Rehabilitation After Lower Limb Loss: Surgical Pearls.

BACKGROUND: The biology of osseointegration of any intramedullary implant depends on the design, the press-fit anchoring, and the loading history of the endoprosthesis. In particular, the material and surface of the endoprosthetic stem are designed to stimulate on- and in-growth of bone as the prerequisite for stable and long-lasting integration1-8. Relative movement between a metal stem and the bone wall may stimulate the formation of a connective-tissue interface, thereby increasing the risk of peri-implant infections and implant loss9-12. The maximum achievable press-fit (i.e., the force cl

CSA-90 Promotes Bone Formation and Mitigates Methicillin-resistant Staphylococcus aureus Infection in a Rat Open Fracture Model.

BACKGROUND: Infection of open fractures remains a significant cause of morbidity and mortality to patients worldwide. Early administration of prophylactic antibiotics is known to improve outcomes; however, increasing concern regarding antimicrobial resistance makes finding new compounds for use in such cases a pressing area for further research. CSA-90, a synthetic peptidomimetic compound, has previously demonstrated promising antimicrobial action against Staphylococcus aureus in rat open fractures. However, its efficacy against antibiotic-resistant microorganisms, its potential as a therapeut

Complications of the Luque-Galveston scoliosis correction technique in paediatric cerebral palsy.

PURPOSE OF THE STUDY: Severe scoliosis is a very frequent condition in cerebral palsy children (CP). It is surgically managed, with unit rod instrumentation being the gold standard in English-speaking countries. The purpose of this work was to report on a small, homogeneous series of non-ambulatory, quadriplegic, spastic patients treated by the Luque-Galveston technique in Strasbourg, France. We present the radiographic outcome of the technique along with a full description of any post-operative complications encountered. MATERIALS AND METHODS: Twenty-eight children were operated on according

Factors associated with recurrent fifth metatarsal stress fracture.

BACKGROUND: Many surgeons agree that fifth metatarsal stress fractures have a tendency toward delayed union, nonunion, and possibly refracture. Difficulty healing seems to be correlated with fracture classification. However, refracture sometimes occurs after low-grade fracture, even long after apparent resolution. METHODS: The records of 168 consecutive cases of fifth metatarsal stress fracture (163 patients) treated by modified tension band wiring from March 2002 to June 2011 were evaluated retrospectively. Mean length of follow-up was 23.6 months (range, 10-112 months). Forty-nine cases cla

Transosseous Multiple Finger Flexor Tendon Pulley Reconstruction.

BACKGROUND: Closed finger flexor tendon pulley injuries occur most frequently in rock climbing but also during other heavy finger strength work or exercises. While single pulley injuries receive a conservative therapy, multiple pulley injuries often require a surgical repair, as otherwise they will lead to contracture and loss of range of motion and functionality. While established surgical techniques, which use bone encircling tendon graft techniques, were found to produce osteonecrosis of the phalanx, our new transosseous technique avoids this problem. INDICATIONS: All triple pulley injurie

Screw fixation of the upper cervical spine in the pediatric population. Clinical article.

OBJECT: Rigid fixation of the upper cervical spine has become an established method of durable stabilization for a variety of craniocervical pathological entities in children. In children, specifically, the use of C1-2 transarticular screws has been proposed in recent literature to be the gold standard configuration for pathology involving these levels. The authors reviewed the use of rigid fixation techniques alternative to C1-2 transarticular screws in children. Factors evaluated included ease of placement, complications, and postoperative stability. METHODS: Seventeen patients, ranging in

Is steel wire closure of sternotomy better than polyester suture closure?

INTRODUCTION: Median sternotomy is the preferred approach for open heart surgeries. The sternotomy incision is predominantly closed with either steel wire or polyester suture. The type of material used is primarily based on the surgeon's choice, and both materials achieve a good result. No prospective clinical study has been undertaken to evaluate differences in the incidence of wound infection and the degree of pain associated with both techniques. PATIENTS AND METHODS: Our randomized controlled double-blind study included 200 adults undergoing single-valve replacement. The technique of surg

Comparison of complications in transtrochanteric and anterolateral approaches in primary total hip arthroplasty.

Three surgical approaches to primary total hip arthroplasty (THA) have been in use since Charnley popularized the transtrochanteric approach. This study was designed to examine the difference in morbidity between the transtrochanteric approach and the anterolateral approach in primary THA. Information on 891 patients who underwent primary THA performed by a single surgeon was collected prospectively between 1998 and 2003 using a modified SF-36 form, preoperatively, intraoperatively, and at 3 months postoperatively. The transtrochanteric group had higher morbidity and more patients who were dis

Threaded versus porous-surfaced implants as anchorage units for orthodontic treatment: three-dimensional finite element analysis of peri-implant bone tissue stresses.

PURPOSE: A 3-dimensional finite element model was developed to investigate the cause of different crestal bone loss patterns observed around sintered porous-surfaced and machined (turned) threaded dental implants used for orthodontic anchorage in a previously reported animal study. MATERIALS AND METHODS: Twenty-noded structural solid elements with parabolic interpolation between nodes were used for modeling the bone-implant interface zone. A 3-N traction force acting between either 2 porous-surfaced or 2 machined threaded implants placed in canine premolar mandibular sites and bone profiles o

Treatment of cubitus varus using the Ilizarov technique of distraction osteogenesis.

Seven children with a post-traumatic cubitus varus deformity were treated using the Ilizarov technique of distraction osteogenesis. The outcome was rated as excellent in each case and all were satisfied with the cosmetic appearance. No complications had been encountered by the latest follow-up at a mean of 66.7 months. This technique seems reliable for the treatment of such deformities, provided that it achieves full correction by gradual distraction. Nerve palsy and unsightly scars are avoided, and the range of movement of adjacent joints is preserved.

Chronic Lead Toxicity Presenting With Neurologic Decline in an Adult Male.

Lead toxicity in adults is uncommon and can present with progressive neurological manifestations that mimic neurodegenerative diseases. We describe the case of a 61-year-old man with a several-year history of worsening gait instability, cognitive decline, dysarthria, and involuntary movements in the setting of persistently elevated blood lead levels. The patient's symptoms began in late 2023 with balance difficulties and frequent falls, followed by progressive cognitive impairment, personality changes, and a 100 lb unintentional weight loss. His clinical course was further complicated by chron

Robot-assisted targeting for osteoid osteoma: a systematic review.

Accurate nidus access is essential for treating osteoid osteoma, but the incremental contribution of physical robotic targeting should be distinguished from imaging, definitive treatment, and the wider care pathway. We systematically reviewed peer-reviewed human reports in which a robot physically positioned or constrained an instrument used for biopsy or treatment. PubMed/MEDLINE, Embase, Web of Science Core Collection, the Cochrane Library, Wanfang Data and the China National Knowledge Infrastructure were searched from inception through August 2026, without a final language restriction.
