Costoclavicular venous decompression in patients with threatened arteriovenous hemodialysis access.

BACKGROUND: Autologous arteriovenous fistulas are frequently threatened by central venous obstruction. Although this is frequently ascribed to indwelling catheters and neointimal venous remodeling, we believe that extrinsic compression of the subclavian vein as it passes through the costoclavicular junction (CCJ) may play a significant role in a subset of dialysis patients. METHODS: We reviewed our experience with CCJ decompression for arteriovenous fistula dysfunction at our institution. Decompression followed principles for venous thoracic outlet syndrome: bony decompression with thorough v

Concomitant paravertebral FDG uptake helps differentiate supraclavicular and suprarenal brown fat uptake from malignant uptake when CT coregistration is not available.

OBJECTIVES: Fluorine-18 fluorodeoxyglucose (F-18 FDG) uptake in brown adipose tissue (BAT) in the supraclavicular, superior mediastinal, paravertebral, and suprarenal/perinephric regions has been recognized. Of these 4 areas, uptake in the supraclavicular, mediastinal, and suprarenal areas may be difficult to differentiate from malignancy for those who interpret PET images only without CT coregistration or fusion. We assessed the prevalence and concomitance of F-18 FDG uptake in these 4 BAT regions. METHODS: A total of 1495 F-18-FDG PET studies were reviewed. Distinct patterns compatible with

Upper extremity stress fractures.

Although less common than lower-extremity stress fractures, upper-extremity stress fractures are becoming recognized much more frequently. A majority of these fractures are caused by overuse and fatigue of the surrounding musculature and, as a result, may be prevented by appropriate training and conditioning. Diagnosis is made by history and physical examination with the aid of plain radiographs, bone scans, and MRI. Most of these fractures heal with a period of relative rest followed by a structured rehabilitation program. A small percentage of these fractures, however, may require surgical f

Shoulder contracture and osseous deformity in obstetrical brachial plexus injuries.

BACKGROUND: The purposes of this study were to determine the prevalence of and the association between shoulder contracture and osseous deformity in a cohort of children with an obstetrical brachial plexus injury and to identify the risk factors for these complications. METHODS: In a retrospective cohort study, all children with an obstetrical brachial plexus injury who had been born between January 1991 and January 1998 at one academic medical center and all those with the same diagnosis who had been born elsewhere during the same period and were referred to the medical center within six wee

Imaging the joint above and below: investigating the need for "completion imaging" in geriatric fragility distal radius fractures.

BACKGROUND: Distal radius fragility fractures (DRFFs) are common injuries in older adults following low-energy falls. While wrist radiograph … ccurred in 51 patients (27%), most commonly hip or pelvic ring fractures (6%), followed by shoulder/clavicle (4%) and intracranial injuries (4%). Completion imaging was obtained in 179 patients (95%), with a low diagnostic yield of 3.8% for hand radiographs, 3.9% for elbow radiographs, and 0% for forearm radiographs. All additional injuries were clinically …

Comparison of truly anatomic coracoclavicular ligament reconstruction with vs. without acromioclavicular ligament reconstruction for acute Rockwood type V acromioclavicular joint dislocations: a prospective cohort study.

No implant failure or clavicle/coracoid fracture was observed in each group during the follow-up period. CONCLUSION: As compared with the TACCR technique, the TACCR-AC technique was a safe, reliable, and novel surgical technique that yielded better radiologic results and compara …

Ultrasound-Guided Biceps-Tracking Access to the Superior Labral-Biceps Anchor Complex: A Technical Report With Cadaveric Demonstration of Gross Injectate Localization.

The superior labral-biceps anchor complex is a technically difficult ultrasound target because of its deep location, close relationship to t … nt technical limitation was the narrow acoustic and spatial window between the coracoid process and clavicle. Interpretation of these findings should be cautious because localization was assessed by gross anatomical evaluation only, and the injectate was a high-viscosity surrogate material whose dispersion characteristics may differ from those of clinicall …

Ustekinumab as a novel treatment of chronic nonbacterial osteomyelitis: a case report.

BACKGROUND: Chronic nonbacterial osteomyelitis (CNO) is a rare inflammatory disorder that commonly causes bone destruction in the metaphysis of long bones, pelvic and vertebral bones, the clavicle, and the mandible. CNO is often misdiagnosed and underreported. This presentation of debilitating back pain caused by CNO showcases diagnostic challenges and potential for novel therapeutic treatment. CASE DESCRIPTION: A 14-year-old male with Crohn's disease presented with acute, worsening lower back pain and two months of hematochezia after recently stopping infliximab. Physical examination showed …

Histologic Assessment of Medial and Lateral Menisci in Degenerative Knees Using Special Stains: A Histomorphometric Study.

Forequarter amputation (interscapulothoracic amputation) includes surgical removal of an upper limb and the shoulder girdle, including the scapula and a portion of the clavicle. We aim to report about our recent experience of having to resort to this mutilating surgery and the clinicopathological variables in that context. The study was done at a cancer centre in Northeast India. It was an ambispective study design, where the patient cohort who underwent FQA was identified retrospectively from the operative register of major surgeries for the time period 1st June 2020 to 31st May 2022 (24 mont…

All patients had both a plane radiography of the frontal plane including the clavicle and a plane radiography of the entire spine in the sagittal plane from the cervical spine to the pelvis taken before surgery. Reliability of Upright Chest Radiography as a Diagnostic Screening Tool for Acromioclavicular Joint Dislocations.

BACKGROUND: The standard radiographic view for diagnosing acromioclavicular joint (ACJ) dislocations is the Zanca view of both shoulders to assess superior displacement of the distal clavicle by measuring the coracoclavicular distance (CCD) and comparing it with the uninjured shoulder; however, there is no consensus on a gold standard for diagnostic measures to classify acute ACJ injuries. PURPOSE: To compare the CCD from an upright chest radiograph with a standard Zanca view of both shoulders in patients with an ACJ dislocation. STUDY DESIGN: Cohort study (diagnosis); Level of evidence, 3. …
