Treatment of unstable femoral shaft fractures with closed interlocking intramedullary nailing.

From 1979 to 1982, 64 femoral shaft fractures in 62 patients were treated by closed interlocking nailing at Harborview Medical Center, Seattle, WA, U.S.A., and Parkland Memorial Hospital, Dallas, TX, U.S.A. Twenty-nine patients sustained multiple system injuries and 29 of the involved extremities (45%) had at least one additional injury. There were 17 (26%) open fractures. Static mode nailing was used to treat 52 fractures; dynamic mode nailing was performed for 12 fractures. Patient follow-up averaged 17 months (range 7-41 months). The average time to union was 13.5 weeks. Normal femoral leng

Treatment of trochanteric fractures with Ender rods.

The technical aspects of Ender's method in the treatment of trochanteric fractures are described. Problems and complications encountered with this method and their prevention and treatment are discussed. Intraoperative and postoperative problems are subdivided into those in the hip area and around the knee joint, such as: varus deformity, protrusion of the rod medially or laterally or into the hip joint, cracking the distal femur, incorrectly placing the window on the anteromedial side of the femoral condyle, backing out of the rods, and supracondylar fractures.

A forward-striking technique for reducing fracture gaps during intramedullary nailing: A technical note with clinical results.

INTRODUCTION: A residual postoperative fracture gap between major bone fragments following intramedullary nailing of long-bone fractures is recognised as one of the major risk factors for delayed union and non-union. The most common method for reducing a fracture gap after nail insertion is through application of the backstroke technique. We introduce forward-striking as a new and simple technique that can be used to reduce fracture gaps during cephalomedullary or intramedullary nailing. PATIENTS AND METHODS: The forward-striking technique was used in 20 patients with subtrochanteric or femor

Quadriceps strength and anterior knee pain following tibia intramedullary nailing: Any clinical relationship?

INTRODUCTION: Anterior knee pain can be chronic sequelae of intramedullary nailing of the tibia. Several causes have been identified; no single reason can fully explain the occurrence. We, therefore, set out to find the rate of anterior knee pain in our practice and if any relationship exists between the anterior knee pain and extensor muscle strength. METHODOLOGY: A total of 72 knees in 36 patients with no prior history of knee pain, but had unilateral tibiofibular fracture, who had internal fixation with interlocking intramedullary nailing done and were followed up for at least 1 year were

Fibular strut graft for humeral aneurysmal bone cyst with varus deformity.

PURPOSE: Proximal humerus is a common site for ABC and frequently associated with varus deformity that limits shoulder abduction. A prospective study was conducted to evaluate the use of intramedullary non-vascularised autogenous fibular strut graft for reconstruction without internal fixation. METHODS: A total of 20 patients (12 girls, 8 boys) were managed for proximal humeral ABC with varus deformity by extended curettage, osteotomy, intramedullary fibular graft and composite bone substitute. Their ages ranged from 10 to 17 years (average, 13.3 years). The lesion was juxtaphyseal in 16 pati

Which implant is better for beginners to learn to treat geriatric intertrochanteric femur fractures: A randomised controlled trial of surgeons, metalwork, and patients.

BACKGROUND: We wondered whether the third-generation gamma nail-3 (GN-3) was better for junior surgeons to learn to treat geriatric intertrochanteric femur fractures than proximal femoral nail antirotation-II (PFNA-II). METHODS: This is a prospective randomised study of 350 patients who underwent GN-3 fixation and PFNA-II carried out by junior orthopaedic trauma surgeons from January 2011 February 2017. We compared nail positioning, complication rates, operative and fluoroscopy time, blood transfused, time to mobilisation, hospital stay, fracture union, mismatch, mortality and postoperative o

[The reversed locked internal plate fixator as an alternative internal fixation of problematic proximal femur fractures].

AIM: The aim of the study was to evaluate the application of a locked internal fixator in complex fractures of the proximal femur, in which the internal fixation with standard implants was not possible due to poor quality of bone or already failed internal fixation in the past. METHOD: Ten patients suffering from a pertrochanteric (n = 5), periprosthetic (n = 1) or subtrochanteric (n = 4) femural fracture between 2003 and 2008 were prospectively registered, underwent open reduction and internal fixation with an "upside-down" femur LISS (less invasive stabilisation system) and were followed up

Effect of varus and valgus alignment on implant loading after proximal femur fracture fixation.

INTRODUCTION: More than 10 % of proximal femur fractures repaired with either a sliding hip screw and side plate (SHS-P) or a sliding hip screw and intramedullary nail (SHS-IMN) demonstrate varus malreduction. The purpose of this study was to compare the effect of varus or valgus loading on comminuted intertrochanteric fractures repaired with SHS-P or SHS-IMN constructs. METHODS: Unstable intertrochanteric fractures with segmental comminution were generated in 12 cadaver proximal femurs, six of which were fixed with an SHS-P and six with an SHS-IMN. Both implants had a strain gauge at the lag

The outcomes of the management of complex distal tibia and ankle fractures in elderly with tibiotalocalcaneal nail in a minimum 12-month follow-up period.

PURPOSE: To evaluate the clinical outcomes of the use of tibiotalocalcaneal nail for the treatment of complex distal tibia and ankle fractures in elderly people, in a major trauma centre. METHODS: Elderly patients (age > 65) with distal tibia or ankle fractures that underwent stabilization with a tibiotalocalcaneal nail were eligible to participate. Exclusion criteria were patients that died or were lost to follow-up and cases in which the nail was used in a chronic setting, such as malunion and non-union. Main parameters evaluated were fracture union, complications and functional outcomes. T

Adolescent Distal Humerus Fractures: ORIF Versus CRPP.

BACKGROUND: Although supracondylar humerus fractures are common in young children, the incidence in adolescents is much lower. As a result, there is a paucity of literature to guide treatment. The purpose of this study was to review the treatment and outcomes for a consecutive series of distal humerus fractures in adolescents and to compare outcomes between patients treated with percutaneous skeletal fixation and those treated with open reduction and fixation. METHODS: A retrospective review of patients 10 to 17 years of age who underwent surgical treatment for a distal humerus fracture from

Complications of ESIN osteosynthesis--Experience in 270 patients.

BACKGROUND: Elastic stable intramedullary nailing (ESIN) osteosynthesis has been used in our department for the treatment of long-bone fractures in children and adolescents for more than 17 years. During this period we have shown that ESIN has several advantages compared with other methods of treatment. However, as with every other method, ESIN has its drawbacks and complications. These occur primarily if indication criteria are not respected or ESIN technique is inadequate. This paper presents the rate of complications that occurred with this method in our patients, and the means of preventio

Closed intramedullary tibial nailing using the Marchetti-Vicenzi nail.

We present the results of using the Marchetti-Vicenzi nail in the management of 41 closed and 16 type I-III open tibial fractures in 56 patients. The treatment goals were to achieve stability and fracture alignment allowing early functional treatment of the knee and ankle by immediate partial or full weight-bearing. Early callus formation and rapid mobilisation were obtained in 84% of the patients. The mean time to clinical union was 9.8 weeks (radiological union 28.4 weeks) for closed fractures and also 9.8 weeks for open fractures (radiological union 28.7 weeks). No late rotational deformiti

Complications of Marchetti locked nailing for humeral shaft fractures.

In this retrospective study 50 humeral fractures (36 acute, 6 pathological fractures and 8 non-unions) were treated by retrograde locked bundled Marchetti nailing. No intraoperative complications occurred. Postoperative complications included 7 non-unions (4/36 acute fractures and 3/8 delayed union), and 2 intraarticular penetrations of the secondary nails. However, at the subsequent removal of the implant 5 supracondylar fractures occurred.

Reamed against unreamed nailing of the femoral diaphysis: a retrospective study of healing time.

In order to assess the results of the AO unreamed femoral nail (URFN), and specifically its effects on healing, 147 consecutive patients treated were reviewed. These included 50 reamed femoral nails (RFN) and 97 unreamed femoral nails. Exclusion of pathological fractures, revisions and fractures outside the femoral diaphysis left 51 procedures in which the healing process could be studied. Twenty-four unreamed and 27 reamed femoral nails in patients with diaphyseal fractures AO (32) were followed up by clinical review and radiographically until union or death. There were two deaths from multip

[Corrective osteotomies of the thigh after faulty healing of infected burn wounds].

The occurrence of burn injuries and fractures of the extremities often necessitates that vital precautions be taken first and the possibility of osseous defective healing be initially accepted. After the vital functions have been stabilized, late corrective procedures become necessary. Two cases of the treatment strategy for femur fractures which healed defectively are reported.

Evaluation of cerclage wiring in the treatment of subtrochanteric fractures.

Treatment of subtrochanteric fractures is challenging because of their typical displacement pattern. Use of circumferential cerclage wires can be added to intramedullary nailing to facilitate better anatomical reduction. Concerns exist regarding additional soft tissue damage and ischemia of the periosteum. The aim of this study was to assess the effect of cerclage on union and infection rates. The postoperative results of 115 patients over 11 years were retrospectively viewed. Twenty-three patients were treated with cerclage. The primary outcome measure was 'return to theatre for fixation fail

Urinary catheterisation and deep wound infection after hip fracture surgery.

We have studied the incidence of deep wound infection after hip fracture surgery and its relationship to urinary catheterisation. Infection after hemiarthroplasty occurred in 16/949 cases and 10/1,143 sliding hip screw fixations. No cases of deep sepsis were seen for the 203 patients treated with an intramedullary nail or the 816 patients treated with parallel screws. The use of urinary catheterisation was compared with a control group of patients. In the sepsis group, 12/18 patients had been catheterised peri-operatively or within 24 h, as opposed to 18/36 patients in the control group; the d
