Analysis of tardy ulnar nerve palsy associated with cubitus varus deformity after a supracondylar fracture of the humerus: a report of four cases.

Four cases of tardy ulnar nerve palsy associated with a cubitus varus deformity of the elbow secondary to a supracondylar fracture of the humerus are presented. All patients had surgical management of their ulnar nerve palsy. In two patients, the ulnar nerve was entrapped by scar tissue at the abnormal position and the nerve developed a sharp V-shaped kink when the elbow was flexed. In one patient, the ulnar nerve displaced anteriorly with elbow flexion and spontaneously reduced into the ulnar nerve groove with elbow extension. In one patient, the ulnar nerve remained in the ulnar nerve groove

Intramedullary osteosynthesis 3. Küntscher nailing in the humerus.

With regard for the principles that govern the correct application of intramedullary osteosynthesis in the long bones of the lower limb, the authors also support its use in the humerus. After an exhaustive survey of the specific problems at this site, a critical review of 130 cases observed over the last 15 years is reported. Extremely favourable results emerged in the treatment of the humerus with this means of synthesis, thanks to its surgical, mechanical and biological advantages.

Comparison of intraarticular distal humerus fracture outcomes treated with or without olecranon osteotomy - A case series.

A case series was extracted from the trauma registry at Aga Khan University Hospital from the period June 2015 to June 2019. Included were 16 adult patients who presented with intra-articular distal humerus fracture type C2. The functional, clinical and radiological outcomes of fractures treated with or without olecranon osteotomy up to 12 months follow-up were compared. Outcomes were assessed at 6 weeks, 3, 6 and 12 months re-visits. Among the 16 studied patients, 9 (56%) were males and 7 (44%) were females. In the group without osteotomy, there was a good functional and clinical outcome with

Grading of functional results of elbow joint arthrolysis after fracture treatment.

In the treatment of posttraumatic contracture of the elbow joint, arthrolysis is a proven procedure. We used a stepwise operative approach starting laterally and including an additional medial and dorsal incision if needed. A total of 91 patients with arthrolysis of the elbow could be followed-up on average 44 months (range 9-102 months) joint after operative (58, 63.7%) and non-operative (33, 36.3%) fracture treatment. The mean preoperative range of motion (ROM) in flexion/extension was 49 degrees (SD +/- 38 degrees), while in pronation/supination it was 89 degrees (SD +/- 66 degrees). Postop

[Epidemiology of gunshot wounds at Ciudad Juárez, Chihuahua General Hospital].

BACKGROUND: Since 2007, Ciudad Juárez, Chihuahua has been considered as one of the most violent cities in the world. The General Hospital in this city is the main facility where patients with gunshot wounds are taken. The increased number of admissions of patients with these injuries to many hospitals in the country deserves special attention, as it has an impact on hospital resources and management protocols. OBJECTIVES: To disseminate the epidemiology of fractures caused by gunshot wounds and the hospital care of these patients. METHODS: A retrospective, observational cohort study was cond

What is the preferred timing for surgery after proximal humerus fractures: an e-modified Delphi Survey Study.

BACKGROUND: Certain clinically selected proximal humerus fractures (PHFs) require operative treatment to preserve shoulder function. There is no consensus on the timing of surgery after PHFs, and guidelines are lacking. This study utilized the Delphi method to (1) find consensus on the preferred timing for multiple PHF patterns potentially requiring operative treatment and (2) assess statistical stability of consensus. METHODS: A three-round electronic modified Delphi study was conducted between May and October 2025. Sixty-one shoulder experts from Australia, Europe, Asia, and North America p

Arthroscopic-assisted plate fixation for displaced large-sized comminuted greater tuberosity fractures of proximal humerus: a novel surgical technique.

PURPOSE: The purpose of the present study was to describe the use of a novel hybrid surgical technique-arthroscopic-assisted plate fixation-and evaluate its clinical and anatomical outcomes in the management of large, displaced greater tuberosity (GT) fractures with comminution. METHODS: From 2009 to 2011, this novel technique was performed in 11 patients [2 men and 9 women; median age, 64 years (range 41-83 years)] with large, comminuted GT fractures, with fragment displacements of >5 mm. The preoperative mean posterior and superior migration of the fractured fragment, as measured on compute

Results of stemless shoulder arthroplasty: a systematic review and meta-analysis.

Stemless shoulder arthroplasty relies solely on cementless metaphyseal fixation and is designed to avoid stem-related problem such as intraoperative fractures, loosening, stress shielding or stress-risers for periprosthetic fractures. Many designs are currently on the market, although only six anatomic and two reverse arthroplasty designs have results published with a minimum of two-year follow-up.Compared to stemmed designs, clinical outcome is equally good using stemless designs in the short and medium-term follow-up, which is also the case for overall complication and revision rates.Intraope

Total Elbow Arthroplasty for Distal Humerus Fractures in Japanese Elderly Patients: Clinical Outcomes of 23 Cases with Minimum 2-year Follow-up.

INTRODUCTION: Open reduction and internal fixation (ORIF) with articular reconstruction is the standard treatment for distal humerus fractures; however, ORIF is challenging in elderly patients with severe comminution and poor bone quality. Although favorable outcomes of total elbow arthroplasty (TEA) have been reported for complex distal humerus fractures deemed unsuitable for ORIF, clinical data in Japanese patients remain limited. This study reports the clinical outcomes of TEA in Japanese patients aged 65 years or older when ORIF was deemed unfeasible. METHODS: We retrospectively reviewed

Radiological Union and Functional Recovery Following JUST UNIC™ Fixation for Comminuted Anatomical Neck Proximal Humerus Fractures: A Single-Centre Retrospective Case Series.

Background Comminuted proximal humeral fractures involving the anatomical neck pose challenges in fixation stability and vascular preservation. The JUST UNIC™ (Evolutis, Briennon, France) bilboquet-inspired fixation system has been developed to address these limitations by providing metaphyseal stability with minimal periosteal disruption. This study assesses the clinical, functional, and radiological outcomes following JUST UNIC fixation in this fracture subset. Methods This retrospective single-centre case series included 25 patients with comminuted proximal humerus fractures involving the a…

Protocol for the ProFHER (PROximal Fracture of the Humerus: Evaluation by Randomisation) trial: a pragmatic multi-centre randomised controlled trial of surgical versus non-surgical treatment for proximal fracture of the humerus in adults.

BACKGROUND: Proximal humeral fractures, which occur mainly in older adults, account for approximately 4 to 5% of all fractures. Approximately 40% of these fractures are displaced fractures involving the surgical neck. Management of this group of fractures is often challenging and the outcome is frequently unsatisfactory. In particular it is not clear whether surgery gives better outcomes than non-surgical management. Currently there is much variation in the use of surgery and a lack of good quality evidence to inform this decision. METHODS/DESIGN: We aim to undertake a pragmatic UK-based mult…

Circular external fixation for treatment of distal humerus fractures: case report.

High-energy blasts can lead to complex intra-articular distal humerus fractures with extensive soft tissue loss, and treatment is fraught with complications. We describe 2 patients with such injuries treated successfully with the use of aggressive wound management followed by distal humerus fracture stabilization with a circular external fixator. We chose this circular external fixator over the Ilizarov frame because of our experience and success in the treatment of open tibia fractures with negligible malalignment and high union rate. This circular external fixator allows for indirect reducti

Management of the floating elbow injury in children. Simultaneous ipsilateral fractures of the elbow and forearm.

Simultaneous ipsilateral fracture of the elbow and forearm--floating elbow--is an uncommon injury. During a 7-year period we prospectively followed 12 children who presented with completely displaced supracondylar fractures of the humerus associated with a forearm fracture of the same limb. All patients underwent emergency operative reduction and percutaneous K-wire stabilisation. At a minimum of 18 months, all patients were assessed clinically and radiologically and the results evaluated according to a conventional scoring system. Ten patients had good or excellent outcomes, and there were tw
