Unnecessary ordering of magnetic resonance imaging of the knee: A retrospective chart review of referrals to orthopedic surgeons.

There is a noticeable increase in the unnecessary ordering of Magnetic Resonance Imaging (MRI) of the knee in older patients. This quality improvement study assessed the frequency of unnecessary pre-consultation knee MRIs and investigated the effect on the outcome of the patients' consultation with the orthopedic surgeon. 650 medical charts of patients aged 55 years or older referred to an orthopedic clinic with knee complaints were reviewed. Patients arriving with a pre-consultation MRI were identified, and the usefulness of the MRI was evaluated using the appropriateness criteria developed t

Alterations in Synovial Fluid Biomarker Levels in Knees With Meniscal Injury as Compared With Asymptomatic Contralateral Knees.

BACKGROUND: Changes in the joint microenvironment after an injury to the articular surface of the knee have been implicated in the pathogenesis of osteoarthritis. While prior studies focused on changes in this microenvironment after anterior cruciate ligament ruptures, few have explored the biomarker changes that occur in the setting of meniscal injuries. PURPOSE: To determine whether meniscal injury results in significant alterations to synovial fluid biomarker concentrations as compared with noninjured contralateral knees. Additionally, to explore the relationship between synovial fluid bio

Long-term Subjective, Clinical, and Radiographic Outcome Evaluation of Meniscal Allograft Transplantation in the Knee.

BACKGROUND: Although meniscal allograft transplantation (MAT) has been performed for nearly 30 years, there are few long-term published studies of the technique. HYPOTHESIS/PURPOSE: The goal of this study was to report the long-term results of a patient cohort whose medium-term results have been reported and to evaluate whether the results are maintained in the long term or deteriorate after a certain period. The hypothesis was that the subjective, clinical, and radiographic results are consistent in the long term without significant deterioration. STUDY DESIGN: Case series; Level of evidenc

Arthroscopic treatment of acute septic arthritis after meniscal allograft transplantation.

We present the 30-month follow-up results of an acute septic arthritis of the knee after meniscal allograft transplantation, which was successfully treated with graft retention. A 21-year-old man presented with a 4-month history of right knee pain following arthroscopic subtotal lateral meniscectomy. Plain radiographs showed there was no arthritic change with neutral limb alignment. Fourteen days after meniscal allograft transplantation, septic arthritis was confirmed with positive cultures for Staphylococcus epidermidis, and arthroscopic debridement and irrigation were performed. The suggeste

Prevalence of tibiofemoral osteoarthritis 15 years after nonoperative treatment of anterior cruciate ligament injury: a prospective cohort study.

BACKGROUND: The occurrence of osteoarthritis (OA), associated meniscal injuries, meniscectomy, and patient-related measures for patients treated nonoperatively after anterior cruciate ligament (ACL) injuries have not been well described in the literature in terms of natural history. HYPOTHESIS: Patients with ACL injury can achieve a low occurrence of tibiofemoral OA and good knee function when treated without ACL reconstruction. STUDY DESIGN: Cohort study (prognosis); Level of evidence, 2. METHODS: One hundred consecutive patients with an acute, complete ACL injury were observed for 15 year

Localized pigmented villonodular synovitis of the knee: diagnostic challenge and arthroscopic treatment: a report of three cases.

The localized form of pigmented villonodular synovitis (LPVS) is a lesion characterized by focal involvement of the synovial membrane. The knee is the most commonly affected joint. We report three cases of LPVS of the knee which were not diagnosed upon clinical evaluation. The aim is to bring the attention of clinicians to this pathological entity, which is often regarded as extremely rare and is therefore not considered in the early differential diagnosis of various knee derangements. Diagnostic and therapeutic arthroscopy was performed. The lesions were completely resected and patohistologic

Cost-effectiveness of Early Surgery versus Conservative Treatment with Optional Delayed Meniscectomy for Patients over 45 years with non-obstructive meniscal tears (ESCAPE study): protocol of a randomised controlled trial.

INTRODUCTION: Recent studies show similar outcome between surgery and conservative treatment in patients with non-obstructive meniscal tears. However, surgery is still often preferred over conservative treatment. When conservative treatment is non-inferior to surgery, shifting the current standard treatment choice to conservative treatment alone could save over €30 millions of direct medical costs on an annual basis. Economic evaluation studies comparing surgery to conservative treatment are lacking. METHODS AND ANALYSIS: A multicentre randomised controlled trial (RCT) with an economic evalua

Outcome of meniscal allograft transplantation related to articular cartilage status: advanced chondral damage should not be a contraindication.

PURPOSE: Advanced chondral damage (bare bone) at presentation is considered a contraindication to meniscal allograft transplantation (MAT), yet there are few other options for young patients where arthroplasty is not appropriate. This study hypothesis is that MAT in patients with advanced chondral damage can obtain good clinical outcomes, equivalent to patients with minimal chondral damage. METHOD: A prospective longitudinal study of 99 consecutive patients who underwent MAT between May 2005 and Feb 2013, with a minimum of 1-year follow-up. Patients were categorised into two groups: 60 in Gro

Arthroscopic all-inside suture of symptomatic Baker's cysts: a technical option for surgical treatment in adults.

Synovial fluid caused by repeated effusions may replete the gastrocnemius-semimembranosus bursa (GSB) communicating with the knee joint. Fluid trapped inside the GSB through an alleged unidirectional valve-like mechanism forms a so-called Baker's cyst. Since a significant association of Baker's cysts with knee joint disorders has been reported, treatment should primarily address articular lesions causing recurrent effusions. Arthroscopic surgery provides an effective treatment in that both the cyst and associated joint disorders can be optimally visualized and accordingly treated. In the prese

[Injuries in women soccer - results of a prospective study - in cooperation with the German Football Association (DFB)].

Within the twelve female first division soccer teams 131 players of all 254 were injured during the season 2000/01. A total of 216 injuries was found, among them 74 minor (less than a week absence from sport), 84 moderate (max. 6 weeks) and 58 major injuries (more than 6 weeks). The average time to recover after an injury was 26.5 days. Within 116 joint injuries 16 sprains and 22 ligamental ruptures of the ankle, 16 meniscal lesions and 11 ACL ruptures could be found. Women soccer shows - compared to men - a higher selective risk of severe injuries, especially to the ankle joint and cruciate l
