Necrotizing Fasciitis and Targeted Muscle Reinnervation in the Upper Extremity: A Case Report.

BACKGROUND Necrotizing fasciitis is a rare but increasingly prevalent, rapidly progressive soft tissue infection with high morbidity and mortality. While cases of lower limb involvement are more frequent, cases affecting the upper limb, especially leading to proximal major amputations, such as transhumeral amputation, are exceedingly rare. Targeted muscle reinnervation (TMR) is an innovative approach, aimed at improving postoperative outcomes and pain control. We report a case with sudden clinical presentation that was managed initially with emergency amputation. Postoperative neuroma-related

The relationship between Coronavirus Disease-19 (COVID-19) and ischemic priapism: a case-control study.

This multicentre retrospective study was conducted in 3 university hospitals in Egypt between April 2020 and June 2022. The aim was to assess the relation between Coronavirus Disease-19 (COVID-19) and ischemic priapism. Forty-three ischemic priapism patients were diagnosed and divided into two groups (30 in group I with ischemic priapism only, and 13 in group II with both ischemic priapism and COVID-19). Further sub-classification of COVID-19 patients according to the severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) infection severity was done. Cavernosal aspiration was successful

Fixation and Implant Retention of Extracapsular Femoral Neck Periprosthetic Fractures Around Hip Resurfacing Arthroplasty - A Case Series.

BACKGROUND: Extracapsular femoral neck fractures in the presence of a resurfacing hip arthroplasty (RHA) appear to be independent of suboptimal technique during the initial implantation of the RHA and present with a similar etiology as native hip fractures - that is, a fragility fracture related to pathological or age-related osteoporosis, as a consequence of trauma. In the presence of a well-fixed and previously well-functioning RHA, the options for management include revision arthroplasty or open reduction and internal fixation (ORIF). In the absence of loosening through mechanisms of wear,

Is the use of antibiotic-loaded bone cement associated with a lower risk of revision after primary total hip arthroplasty?

AIMS: The aim of this study was to investigate whether the use of antibiotic-loaded bone cement influenced the risk of revision surgery after primary total hip arthroplasty (THA) for osteoarthritis. METHODS: The study involved data collected by the National Joint Registry (NJR) for England and Wales, Northern Ireland and the Isle of Man between 1 September 2005 and 31 August 2017. Cox proportional hazards were used to investigate the association between use of antibiotic-loaded bone cement and the risk of revision due to prosthetic joint infection (PJI), with adjustments made for the year of

Management of Chronically Infected Total Knee Arthroplasty With Severe Bone Loss Using Static Spacers With Intramedullary Rods.

BACKGROUND: Two-stage revision with static antibiotic spacers is the preferred treatment for chronically infected total knee arthroplasty (TKA) associated with severe bone loss. Intramedullary rods to reinforce static spacers have been described. On those, however, bacterial colonization may occur and hamper infection control. This study reports the microbiological findings on the spacer rods and the treatment outcome among these patients. METHODS: We reviewed 97 infected TKA with extensive bone loss treated with antibiotic-loaded cement spacers reinforced with intramedullary rods. Mean inter

Surgical treatment of double valve endocarditis.

BACKGROUND: We have retrospectively analyzed the results of the operations made for aortic infective endocarditis with mitral involvement in a single center in 19 years. METHODS: From May 1992 to January 2011, we have operated on 72 patients with infective endocarditis of the aortic valve with mitral valve involvement. Fifty-two patients (72.2%) were male and the mean age was 40.5 ± 15.5 (9-73) years. The blood cultures were positive in 33 patients (45.8%) and the most commonly identified microorganism was Streptococcus. Nine patients (12.5%) had prosthetic valve endocarditis. The mean durati

Revision hip replacement in patients 55 years of age and younger.

The purpose of this study was to assess the efficacy and complications associated with revision hip arthroplasty undertaken in patients under 55 years of age. A review of our revision hip database showed that 430 revision hip arthroplasties undertaken over the past 9 years had a minimum follow-up of 2 years. Of these, 84 (46 female, 38 male) patients were under 55 years of age. The index procedures were preformed at an average age of 46 years (range 15 years to 54 years). The etiology of failure was aseptic loosening (74%), followed by infection (8%) and recurrent dislocation (7%). In the majo

Management of a pelvic mass following a worn uncemented total hip arthroplasty.

Intrapelvic granulomatous masses from polyethylene wear debris can easily be misdiagnosed if orthopaedic etiology is not considered. This article presents the case of a 50 year old woman with history of hip joint trauma and total hip arthroplasty (THA) revisions who presented to her gynecologist with a large intrapelvic cyst. Prolonged use of an indwelling drain and failure to recognize the orthopaedic origin of the mass resulted in articular infection that required an antibiotic spacer and subsequent THA revision. This patient did not have pain or evidence of medial wall defects, but did have

A modified cement spacer technique for infected total hip arthroplasties with significant bone loss.

Complications related to femoral spacers are common during addressing infected total hip arthroplasties by 2-stage revision. We evaluated 11 patients who had 2-stage revisions with massive bone loss after removal of the infected components by a trochanteric osteotomy. All femoral cement spacers were assembled on intramedullary nails. Femurs were protected by a plate, whereas acetabuli were augmented by a cage or roof ring depending on the remaining bone stock. This additional hardware was covered with antibiotic-impregnated cement. Infection was eradicated in 10 (90.9%) of 11 patients within 3

Reoperation for failed prosthetic replacement used for limb salvage.

Patients with segmental bone and joint replacement prostheses because of tumors increasingly need revision surgery because of their long term survival. Between 1970 and 1990, 208 custom prosthetic replacements were performed for limb salvage in patients with tumors. Reoperations were required in 52 patients. The mean time to reoperation was 37 months. The reoperation procedures included 35 prosthetic revisions, 11 amputations, four arthrodeses, one vascularized fibular graft, and one open reduction and internal fixation of a fracture with supplemental bone graft. Functional assessment using th

The Ross operation: results of early experience including treatment for endocarditis.

The Ross operation has been performed for more than 25 years and its popularity has increased dramatically in recent years. We developed an interest in this procedure through a combination of a basic dissatisfaction with a device that requires life-long anticoagulation and the belief that a vital, autologous tissue valve with normal valve morphology and hemodynamics would prove to be superior to the mechanical valve, and that these advantages would outweigh the potential drawbacks related to the operation's technical difficulty and the risk of autograft or homograft dysfunction. From December

Clinical and Radiographic Outcomes of Total Elbow Arthroplasty Using a Semi-constrained Prosthesis with a Triceps-preserving Approach over a Minimum Follow-up Period of 4 Years.

OBJECTIVE: Complications related to triceps after total elbow arthroplasty (TEA) have become a major surgical concern. The triceps-preserving approach has the advantage of not disturbing the insertion of triceps but is disadvantaged by the reduced exposure of the elbow joint. The aim of this study was to investigate the clinical and radiological outcomes of TEA with a triceps-preserving approach and to compare the outcomes of TEA to treat arthropathy with that of TEA to treat acute distal humerus fracture. METHODS: From January 2010 to December 2018, 23 patients undergoing primary TEAs were r

Use of a polymethacrylate radial head spacer in temporary reconstruction of complex radial head fracture with associated elbow instability.

Radial head replacement in complex elbow fractures (Mason III) with either bony or ligamentous injuries or interosseous membrane rupture is unquestionable. Actual modular and metallic prosthesis ease the mounting technique and ensure durability. Nevertheless, these types of prostheses are not always available in a short time in our daily practice. We present the use of a transient polymethacrylate spacer as an alternative in nonreconstructable complex radial head fractures with a unstable elbow. We assessed 38 patients between 2006 and 2007, with a median follow-up of 53.8 months. We included

Pathologic and clinical aspects of repair of large incisional hernias after implant of a polytetrafluoroethylene prosthesis.

One of the alternatives for the repair of large incisional hernias is the use of a prosthetic material. The present retrospective study relates the experience acquired for treatment of large incisional hernias (hernial orifice > 10 cm) with ePTFE prostheses. Thirty-eight massive incisional hernias were repaired using ePTFE 1 mm thick Soft Tissue Patches. Twenty-four patients received a prosthetic patch of 10 x 15 cm, 13 a patch of 15 x 20 cm, and 1 a patch of 20 x 30 cm. In 30 cases the ePTFE was sutured to the recipient tissues with a double row of polypropylene stitches. For the first eight

Rare case of pacemaker infection with Mycobacterium abscessus.

Though prevalent in the environment, nontuberculous mycobacteria (NTM) have been increasingly identified as pathogenic. Sporadic reports of NTM infection of cardiac implantable electronic devices (CIEDs) have appeared but remain rare. This case describes a CIED infection with Mycobacterium abscessus, the third reported case in the literature. A 63-year-old male presented with a 3-day history of drainage from his pacemaker extraction site. An aspirate grew Mycobacterium abscessus Together with National Jewish Health, a treatment plan was developed, consisting of an induction phase with amikacin

Deep brain stimulator infection by a novel rapid growing mycobacterium.

Devise-related infections after deep brain stimulator implantation are not uncommon. However, infections due to mycobacteria have not been reported in the medical literature. We describe the first reported case of DBS infection due to a novel rapidly growing mycobacteria, most closely resembling Mycobacterium goodii, by rpoB gene sequencing.

Are homografts superior to conventional prosthetic valves in the setting of infective endocarditis involving the aortic valve?

BACKGROUND: Surgical dogma suggests that homografts should be used preferentially, compared with conventional xenograft or mechanical prostheses, in the setting of infective endocarditis (IE), because they have greater resistance to infection. However, comparative data that support this notion are limited. METHODS: From the prospective databases of 2 tertiary academic centers, we identified 304 consecutive adult patients (age ≥17 years) who underwent surgery for active IE involving the aortic valve (AV), in the period 2002 to 2014. Short- and long-term outcomes were evaluated using propensity
