Randomized Trial of Sternal Closure for Low Risk Patients: Rigid Fixation versus Wire Closure.

BACKGROUND: A previous retrospective analysis of our cardiac surgery patients showed shortened ventilation time and hospital stay among patients receiving rigid sternal fixation compared to sternal wire fixation. We performed a prospective randomized study to further investigate these outcomes and determine if rigid closure can provide reduced pain after cardiac surgery. METHODS: Patients undergoing cardiac surgery between July 2011 and May 2014 were prospectively randomized into wire closure (Group C) or rigid fixation using sternal plates (Group R) groups. Age above 80, emergency surgery, r

Cardiothoracic injuries after CardioPump CPR: a report of two cases and review of the literature.

Although many clinical trials have demonstrated its efficacy during active compression-decompression cardiopulmonary resuscitation (ACD-CPR), the Ambu® CardioPump seems likely to cause severe and sometimes lethal injuries. In this paper, we report two cases observed at the Institute of Legal Medicine of Nancy, France. A 67-year-old man collapsed in the street, in the presence of witnesses, and without any sign of trauma. The autopsy revealed a flail chest, a wound of the left ventricle, a rupture of the right ventricle, and a wrenching of the inferior vena cava. A 71-year-old woman was found i

Noncontiguous multi-tiered spinal tuberculosis associated with sternal localization: a case report.

BACKGROUND: Tuberculous spondylodiscitis is a frequent localization of tuberculosis. Multi-tiered involvement and an association with sternal localization are rare. CASE PRESENTATION: We report a case of multi-tiered tuberculous spondylodiscitis with sternal localization in an immunocompetent 41-year-old Arab woman who had inflammatory bilateral sciatica L5 and S1 and a history of low back pain caused by a trauma. Radiography, computed tomography, and a vertebral biopsy were useful for diagnosis. She reacted well to anti-bacillary treatment despite the occurrence of multiple paravertebral and

Titanium plate fixation versus conventional approach in the treatment of deep sternal wound infection.

BACKGROUND: Deep sternal wound infection (DSWI) is a serious complication post cardiac surgery and associated with increased mortality, morbidity and cost. Sternal titanium plate fixation could be an effective approach to treat DSWI. We sought to compare the effectiveness of titanium plate fixation with conventional approach in the treatment of DSWI. METHODS: Retrospective data was analyzed from consecutive patients with DSWI post cardiac surgery who received either titanium plate fixation (sternal plate group) or conventional treatment with sternal debridement and rewiring (control group). Early and mid-term outcomes of trans-sternal and video-assisted thoracoscopic surgery for thymoma.

OBJECTIVES: Video-assisted thoracoscopic surgery (VATS) for thymoma has uncertain safety and effectiveness in comparison with trans-sternal resection. This feasibility study compared short- and mid-term outcomes for patients undergoing these two procedures, highlights weaknesses in current research and makes recommendations for long-term technological evaluations in this field. METHODS: Consecutive thymoma cases between 2004 and 2010 were identified. Patients were divided into two groups according to surgical approach (Group I trans-sternal; Group II VATS) and comparisons were made between gr

Thermoreactive nitinol clips: Propensity score comparison with Robicsek technique.

OBJECTIVE: To compare the use of thermoreactive nitinol clips with the classic Robicsek technique for treatment of sternal dehiscence after cardiac surgery. METHODS: Eighty-two (2.3%) of 3564 open heart surgery patients underwent reoperation for sternal dehiscence between October 2011 and 2012. Prospectively collected data from 26 (31%) consecutive patients who underwent reoperation using thermoreactive nitinol clips were compared with those of a retrospective cohort of 42 (51.2%) who were treated with the classic Robicsek technique. To overcome baseline and operative variations, we construct

Chest wall reconstruction for sternal dehiscence after open heart surgery.

BACKGROUND: Sternal dehiscence is a grave complication after open heart surgery. Sternal debridement and flap coverage are the mainstays of therapy, but no consensus exists regarding the appropriate level of debridement. More recently, the use of vacuum-assisted closure devices has been advocated as a bridge to definitive closure, but indications for use remain incompletely defined. MATERIALS AND METHODS: A retrospective review of all chest wall reconstructions performed from January 2000 to December 2010 was conducted. The type of operative management was evaluated to assess morbidity, morta

Nitinol flexigrip sternal closure system and chest wound infections: insight from a comparative analysis of complications and costs.

BACKGROUND: We sought to assess the efficiency of two different sternal closure techniques in preventing sternal wound complications (SWC). A cost analysis was also considered. METHODS: Between January 2008 and April 2010, 1,644 consecutive cardiac surgery patients who underwent cardiac surgery in our institute were prospectively collected. A total of 1,072 patients received a standard parasternal wiring technique (group A), and 572 patients received a new method of sternal closure based on the use of thermoreactive nitillium clips (Flexigrip; Praesidia SRL, Bologna, Italy [group B]). Cardiopulmonary bypass line sternal wrapping for protection and haemostasis.

Sternal marrow haemostasis is often obtained with the application of bone wax, with potential side effects pertaining to sternal wound healing. We illustrate an alternative technique which also offers some protection to sternal edges. Two lengths of tubing from the discarded cardiopulmonary bypass (CPB) circuit are cut longitudinally on one side. After sternotomy they are placed across each sternal edge and kept in position by two stitches and the spreader blades. They are removed just before sternal closure.

Is allogeneic blood transfusion a risk factor for sternal dehiscence following cardiac surgery? A prospective observational study.

Sternal dehiscence following cardiac surgery has a multifactorial etiology. Significant risk factors contributing to sternal dehiscence include chronic obstructive pulmonary disease (COPD), obesity, or re-exploration due to bleeding or pericardial tamponade. We have focused on the role of allogeneic blood transfusion as a factor leading to the poor healing of surgical wounds. A prospective observational study of 1553 elective and emergency cardiac surgery patients was performed between January 2003 and June 2007. All of the patients enrolled in this study underwent median sternotomy. We studie

Guideline for resuscitation in cardiac arrest after cardiac surgery.

The Clinical Guidelines Committee of the European Association for Cardio-Thoracic Surgery provides this professional view on resuscitation in cardiac arrest after cardiac surgery. This document was created using a multimodal methodology for evidence generation including the extrapolation of existing guidelines from the International Liaison Committee on Resuscitation where possible, our own structured literature reviews on issues particular to cardiac surgery, an international survey on resuscitation hosted by CTSNet and manikin simulations of potential protocols. This protocol differs from ex

Giant mass due to Castleman's disease causing superior vena cava syndrome.

Castleman's disease is an uncommon disorder occurring mostly in patients presenting with localized mediastinal lymphadenopathy. It is usually asymptomatic. With localized disease, surgical excision is curative. Castleman's disease can very rarely present as superior vena cava syndrome. We describe a case of mediastinal Castleman's disease which presented as vena cava superior syndrome; it is the largest mediastinal mass from Castleman's disease reported in the literature.

One-and-a-half ventricular repair through the right lateral thoracotomy: an alternative to midline approach in a patient with previous mediastinitis.

We report a patient who successfully underwent a one-and-a-half ventricular repair (1.5 VR) through a right lateral thoracotomy. In the case of possible hazardous complications at the sternal reentry because of previous mediastinitis, this approach was thought to be an option in selected patients to complete a functional correction by means of 1.5 VR.

Parathyroid subcutaneous pre-sternal transplantation after parathyroidectomy for renal hyperparathyroidism. Long-term graft function.

BACKGROUND: In the setting of total parathyroidectomy (TPT) and parathyroid transplantation (PTx) for renal hyperparathyroidism (RHP), we evaluated long-term parathyroid graft function after subcutaneous pre-sternal transplantation (SCPTx). Because parathyroid glands are surrounded by fatty tissue, we postulated that results of subcutaneous implantation of parathyroid tissue after total parathyroidectomy for renal hyperparathyroidism could be at least as successful as intramuscular grafting, but without its complications. PATIENTS AND METHODS: The study, a prospective open efficacy study of p

Surgical management of mediastinal goiters: when is a sternotomy required?

OBJECTIVE: Mediastinal goiters are frequently diagnosed, particularly in the elderly population. However, factors associated with an increased risk of median sternotomy have not been analyzed systematically. METHODS: Between 1980 and 2004, a total of 185 patients underwent surgery for mediastinal goiters in our institution. There were 126 women and 59 men with a median age of 68 years (range 24 to 94 years). The goiters were left-sided in 77 patients, right-sided in 69 patients, and bilateral in 39 patients. RESULTS: Clinical presentation was mainly dyspnea (37 %), palpation of a cervical ma

Haemopericardium in stable patients after penetrating injury: is subxiphoid pericardial window and drainage enough? A prospective study.

AIM: This prospective study was undertaken to evaluate whether stable patients with haemopericardium could safely be managed with subxiphoid pericardial window (SPW) and drainage only. PATIENTS AND METHODS: From July to December 2001, all stable patients with haemopericardium diagnosed by SPW, who did not have immediate active bleeding, were subjected to sternotomy to grade the injury using the American Association for the Surgery of Trauma (AAST)-cardiac injury score. The data of all patients with penetrating cardiac injuries for the year 2001 is presented to place into perspective the spect

Comparison of limited and full sternotomy in aortic valve replacement.

OBJECTIVE: The practice of minimally invasive valve surgery remains controversial. The aim of this study was to evaluate the technical feasibility and postoperative course of aortic valve replacement through limited upper sternotomy compared to conventional full sternotomy. METHODS: From May 1998 to August 2000, we performed 24 cases of isolated aortic valve replacements through the limited upper sternotomy approach (group M). During the same period, 18 patients received isolated aortic valve replacements through the conventional full sternotomy approach (group C). Operation duration, postope

[Macro-creatine kinase: an interesting occurrence that can cause diagnostic mistake].

INTRODUCTION: Macroenzymes have been known for a long time. Macro-creatine kinase is important among them, because it can cause serious diagnostic misunderstanding. OBJECTIVE: The authors describe a case in which creatine kinase MB isoenzyme activity measured by immunoinhibition was much higher than total creatine kinase activity. METHOD: The presence and type of possible macro-creatine kinase was examined by the authors with heat-stability test and electrophoresis. RESULTS: Total creatine kinase and creatine kinase MB isoenzyme activity measured by immunoinhibition of macro-creatine kinase

[Minimally invasive cardiac surgery--the efficacy of right parasterna approach].

The recent concepts of minimally invasive surgery have affected even cardiovascular surgery. Especially, the desire to lessen incisional pain and hospital stay has made minimally invasive cardiac surgery (MICS) desirable. However, its efficacy is still controversial. To investigate this goal, we assessed the efficacy of avoidance of median sternotomy through right parasternal approach in view of the postoperative bleeding, % transfusion, postoperative intubation period, degree of incisional pain and serum level of cytokines. Patients with mitral valve disease or atrial septal defects were divi

[The transmanubrial approach for tumors of the superior thoracic aperture].

A considerable difficulty in surgical management of apical chest tumors is represented by the potential involvement of the thoracic outlet anatomic structures (i.e.: vertebral body, subclavian vessels, first rib). Among the various techniques proposed, the anterior trans-cervical approach popularized by Dartevelle significantly improved the radical treatment of these tumors. This approach offers a wide access to the thoracic outlet with satisfactory control of subclavian vessels, safe dissection of brachial plexus and upper part of the mediastinum, permitting a radical treatment. However, this
