International Normalized Ratio (INR) Sample Rejection in Neck of Femur Fracture Patients: A Retrospective Closed-Loop Study From a Major UK Trauma Centre.

Background Patients presenting with neck-of-femur (NOF) fractures often require urgent surgery, as prolonged delays beyond 36 hours are associated with increased morbidity, mortality, and length of hospital stay, whereas shorter time-to-surgery intervals have been shown to improve outcomes. Many of these patients are elderly and on anticoagulant therapy; therefore, making accurate International Normalized Ratio (INR) assessment is crucial for determining surgical readiness and anaesthetic safety. The INR reflects the extrinsic pathway of coagulation and is prolonged in patients on warfarin or

Two-stage surgical reconstruction for cubitus valgus secondary to neglected lateral condylar fracture non-union in older children.

BACKGROUND: This study aimed to evaluate the outcomes of a two-stage surgical protocol-comprising open reduction and internal fixation (ORIF) with bone grafting followed by corrective osteotomy-for managing cubitus valgus deformity secondary to neglected lateral condylar fracture non-union in older children. METHODS: We retrospectively reviewed nine patients (5 males, 4 females; mean age, 11.1 years; range, 10-14 years) who underwent this two-stage procedure between January 2015 and January 2023. The mean interval from initial injury to first surgery was 7.7 years (range, 6-10 years). All pat

Treatment of intertrochanteric femur fracture with closed external fixation in high-risk geriatric patients: can it be the most reliable method that reduces mortality to minimum compared to proximal femoral nail and hemiarthroplasty?

The optimal surgical treatment of intertrochanteric femur fractures (ITF) to minimize the increased mortality in geriatric patients with high anesthetic risk was investigated by comparing closed external fixation (EF), a minimal invasive and biological osteosynthesis technique, proximal femoral nail (PFN) and hemiarthroplasty (HA) methods.Three different surgical methods were performed to 167 patients who were admitted to our clinic for ITF between 2014 and 2020 and considered at high risk (American Society of Anesthesiologists III - American Society of Anesthesiologists IV) by the Anesthesiol

[Palmar dislocation of the metacarpophalangeal (MCP) joint of the long finger. A case report].

UNLABELLED: Palmar dislocation of the metacarpophalangeal (MCP) joint of the long finger represents a rare event. The first case was published by Mc Laughlin in 1965, and only nine other cases have been reported. We present one more case and discuss the mechanisms of these dislocations and treatment. CASE REPORT: A 22-year-old biker presented with complex injuries of his right hand after a road accident.Examination revealed an extensive palmar wound on the ulnar side of his right hand, with another on the MCP joint of the little finger, and shortening of the little and the ring fingers. Any a

Is Capitate Shift Reliable as a Single Indicator for Failure of Non-operative Management in Distal Radius Fractures?

Background Distal radius fractures (DRFs) are the most commonly treated fracture; however, their treatment remains controversial. There is significant variation in the rate of surgical intervention related to a lack of consensus regarding the displacement threshold for surgery. Although studies have advocated that carpal malalignment is the most important radiographic parameter for surgical correction, it is rarely considered in general clinical practice and remains poorly studied. Recently, capitate shift was identified as the most useful measure of carpal malalignment, and a capitate shift t

[Posterior shoulder joint instability. Classification, pathomechanism, diagnosis, conservative and surgical management].

The posterior instability of the shoulder is a more difficult diagnostic and therapeutic challenge than the anterior instability. There are many etiologies and causes of posterior instability. Most studies in the literature are retrospective and yield a great variation in therapeutic recommendations. Generally it has to be separated in traumatic and atraumatic instabilities. Most of the traumatic dislocations are impaction fractures of the humeral head against the dorsal glenoid. Therapy is depending on the size of the humeral defect, the duration of dislocation and the functional demand of th

Matrix Rhythm Therapy as a Novel Clinical Approach in the Rehabilitation of Surgically Treated Distal Radius Fracture: A Single Case Study.

Distal radius fractures (DRFs) are prevalent among all hand injuries, commonly due to a fall on an outstretched hand. Not being treated properly can cause many complications such as malunion, non-union, reduced range of motion, and muscle strength. This case report presents a multidisciplinary approach to the physiotherapeutic management of a DRF treated with closed reduction internal fixation using K-wires. The rehabilitation protocol incorporated matrix rhythm therapy (MRT), a novel therapeutic technique, in combination with targeted therapeutic exercises. The study outlines the patient's jo

[Osteosynthesis of extra-articular proximal tibial fractures].

The treatment of extra-articular proximal tibial fractures is a therapeutic challenge due to the frequently significant soft tissue injury, the effect of the deforming forces and the need for an exact restoration of the bony alignment. Various methods of osteosynthesis are available for surgical stabilization. The locking plate osteosynthesis is the most frequently used procedure because of its good biomechanical stability, especially in osteoporotic bones, and the protection of the periosteal blood flow. Depending on the extent and stability of the defect zone, especially in the case of a med

Revision total hip arthroplasty using an alumina-on-alumina bearing surface in patients with osteolysis.

We evaluated the outcomes of 64 consecutive revision total hip arthroplasties with an alumina-on-alumina bearing surface in 61 patients with osteolysis. No implants had been rerevised nor was osteolysis detected at a mean of 9.8 years (range, 7.0-13.1 years) postoperatively. There was 1 case of stem loosening but no cup loosening or alumina bearing fractures. Two surgical procedures were performed for an infection in 1 patient. Three dislocations occurred in 3 hips; all were treated with closed reduction and abduction bracing for 3 months. No further dislocations occurred. With any reoperation

Supraclavicular brachial plexus block as a sole anaesthetic technique in children: an analysis of 200 cases.

Classical supraclavicular brachial plexus block was used as the sole anaesthetic technique in 200 children aged between 5 and 12 years undergoing closed reduction of arm fractures. The local anaesthetic used was lidocaine 1.5% with epinephrine. The block was graded as satisfactory if surgical manipulation could be performed without discomfort and unsatisfactory if general anaesthesia had to be given. In 182 children, the procedure was carried out under the block alone, whereas the remaining 18 patients required general anaesthesia. The mean (SD) time required for performing the block was 9.1 (

[Use of the tumor prosthesis in secondary neoplastic destruction of the proximal end of the femur].

Total hip arthroplasty was performed with PMMA-augmented tumor prostheses in 42 individuals suffering from metastatic destruction of the proximal end of the femur at the Orthopedic University Hospital Mainz from 1980 to 1992. Neoplastic lesions had been triggered by carcinomas of the breast in 25 female patients. 24 of our patients presented with pathologic fractures. In all patients capable of walking preoperatively early mobilization could be attained by total hip arthroplasty. Pain relief was achieved in each patient. Resection of the major trochanter and, partially, of the pelvitrochanteri

Nerve complications in closed fractures of the lower end of the radius.

Nineteen cases of nerve complications following closed fractures of the lower end of the radius (17 lesions of the median nerve, 4 of which were associated with lesions of the ulnar nerve, and 2 isolated lesions of the ulnar) were observed. A therapeutic approach may be suggested for lesions of the median nerve and/or the ulnar nerve: immediate lesions: immediate reduction, gently if possible and avoiding the position of volar flexion and ulnar deviation. Early secondary lesions associated with immobilization in flexion and ulnar deviation: modify the position and complementary pinning when ne

The Effects of Denosumab Treatment on Pain and Function Beyond Bone Density in Patients With Postmenopausal Osteoporosis: A Prospective Study.

OBJECTIVE: Postmenopausal osteoporosis (PMOP) leads to bone loss, fragility, and fractures, causing pain and reduced function. Effective treatment should improve bone mineral density (BMD), prevent fractures, and enhance quality of life. Denosumab is FDA-approved for osteoporosis, but its effects on pain and function in Chinese patients with PMOP remain underexplored. This study investigates the impact and influencing factors of denosumab treatment on pain and function to support a broader evaluation of osteoporosis treatment. METHODS: This prospective study included 200 patients with PMOP, t

[Late sequelae of fractures of the distal third of the forearm during the growth period].

Fractures to the distal third of the forearm are the most common fractures of the upper extremity, with the majority occurring between the age of ten and 14 years. With the exception of the rare epiphyseal fractures, they have a favourable prognosis. The present study investigates the frequency and extent of potential clinical and radiological late sequelae of fractures in the distal third of the forearm during growth. Of the patients treated at the Innsbruck University Department of Traumatology from 1980 to 1992, 220 patients of a growing age with 232 closed fractures in the distal third of

Retrospective analysis of external pin fixation of mandibular fractures: A 25-year single institution experience.

External fixation of mandible fractures is an infrequently utilized fixation treatment modality in comparison to open reduction internal fixation (ORIF) or closed reduction techniques. However, external fixation still provides a necessary modality in the management of select mandibular fractures. Therefore, we aim to draw trends, outcomes, and treatment timelines through a retrospective analysis to better guide, advise and predict realistic treatment outcomes in the future for our patients. This abstract shows trends in indications, complications, airway management and timeline to definitive r

What Effect Have Emerging Technologies Had on Distal Radius Fracture Treatment? A Cross-sectional Analysis 2002 to 2018.

Since the year 2000, there have been numerous advances in the indications for surgery and technical/procedural fixation methods regarding the treatment of distal radius fractures (DRFs). The purpose of this study is to analyze the epidemiological trends surrounding their management. The authors hypothesize that the reported treatment for DRF will correlate with the advancing technologies in plating. Through cross-sectional analysis of a state healthcare hospital database from 2002 to 2018, 79,481 treated DRFs were identified. Operative management of DRFs significantly increased from 2002 to 20

Thrombosis of the Brachial Artery After Closed Elbow Dislocation.

Posterior dislocation of the elbow joint is the second commonest large joint dislocation that can be experienced due to various traumatic incidents. Although it may be associated with fractures and vascular lesions, in this case report, we describe a patient who encountered a posterolateral elbow dislocation following a fall on their arm with an extended elbow. This dislocation was followed by delayed thrombosis of the brachial artery, necessitating a revascularization surgery. For optimal patient care, physicians should remain vigilant, being cautious about potential vascular injuries both be
