Interlocking Nail Fixation for the Treatment of Displaced Intra-Articular Calcaneal Fractures.

INTRODUCTION: Less invasive restoration of joint congruity and calcaneal shape in displaced intra-articular calcaneal fractures via a sinus tarsi approach followed by percutaneous internal fixation with an interlocking nail results in a low rate of soft-tissue complications and good short-term outcomes1 (Video 1). STEP 1 PATIENT PLACEMENT: Place the patient in the lateral decubitus position, supporting the involved extremity with a soft radiolucent pillow, flex the contralateral knee, check with fluoroscopy before draping, and obtain lateral radiographs. STEP 2 INCISION: Use a sinus tarsi ap

Femoral malrotation after trochanteric fracture nailing: what is the safety zone of limb rotation during closed reduction?

INTRODUCTION: Improper closed trochanteric fracture reduction can cause rotational malposition which can lead to serious consequences. The primary objective of this study was to assess the hypothetical threshold at which excessive rotation becomes hazardous due to a significant postoperative malrotation. MATERIALS AND METHODS: We conducted a prospective study focused on closed reduction in intramedullary nailing of trochanteric fractures (AO 31A1-3) in 100 consecutive patients and its influence on final malrotation. Immediately after the closed reduction, the rotation of affected limb was mea

Evaluation of the effect of eccentric nailing on femoral diaphyseal union delay.

OBJECTIVES: In this study, we aimed to investigate whether there was a relationship between deformity at the fracture line/eccentric placement of the nail, and union time (UT) in isthmal diaphyseal femoral fractures treated with intramedullary nails. PATIENTS AND METHODS: Between September 2017 and December 2020, a total of 61 patients (38 males, 23 females; median age: 47 years; range, 23 to 62 years) with closed femoral shaft fractures who underwent antegrade nailing were retrospectively analyzed. The following parameters were examined: (i) amount of angulation in the fracture line (AFL), (

The outcomes of Ilizarov treatment in aseptic nonunions of the tibia stratified by treatment strategies and surgical techniques.

Nonunions of the tibia, particularly those located in the distal third of the bone, are relatively common in clinical practice. There is no gold standard for the treatment of nonunions of the tibia. The purpose of our study was to assess the results of treatment with the Ilizarov method in patients with aseptic nonunions of the tibia, depending on the employed treatment strategies and surgical techniques. A total of 75 patients with Ilizarov treatment of aseptic nonunions of the tibia were evaluated in the study. The patients's mean age at the beginning of treatment was 46 years. The mean foll

Risk factors for refracture of the forearm in children treated with elastic stable intramedullary nailing.

PURPOSE: This study aims to investigate risk factors for refracture of the forearm in children treated with elastic stable intramedullary nailing (ESIN). METHODS: Clinical data of 267 patients who had been treated for forearm fractures by ESIN in our hospital from January 2010 to December 2014 were retrospectively reviewed. Risk factors for forearm refractures were determined using logistic regression analysis. RESULTS: Forearm refractures occurred in 11 children. Univariate analysis revealed that age, body weight, number of fractures, open fracture, nail diameter, and immobilization time we

Retrograde entry portal for femoral interlocking nailing in femoral nonunion after plate failure: a prospective comparative study with antergrade portal.

The piriformis fossa is the ideal portal of entry for antegrade interlocking nailing. Localizing this portal can be difficult and its eccentricity leads to complications. This prospective comparative study was designed to compare an innovative way to obtain the ideal portal from inside the medullary canal in cases of plate failure and compare it to the classic antegrade portal. It included 41 cases (19 antegrade and 22 retrograde). The retrograde portal was significantly better in terms of entry time, radiation time, blood-loss, and wound length. The proper portal was rapidly and easily achiev

Is it safe to place a tibial intramedullary nail through a traumatic knee arthrotomy?

We conducted a study to compare postoperative infection and nonunion rates in tibial intramedullary nails (IMNs) placed through either uninjured knees or traumatic knee arthrotomies (KAs). We reviewed all adult tibial diaphyseal fractures (n = 1378) treated with an IMN between 1998 and 2010. Fourteen of these nails were placed through a traumatic KA. Each patient in the study group was assigned 4 separate matched controls for comparison. Controls were matched on age, sex, diabetes, smoking, and fracture classification (closed or open with Gustilo-Anderson). There were no postoperative infectio

Insufficiency fracture associated with oncogenic osteomalacia.

Oncogenic osteomalacia is a rare paraneoplastic syndrome of systemic bone demineralization secondary to a tumor-induced dysregulation of phosphorus metabolism. The tumor's low prevalence, small size, and variable location often result in years of muscular weakness and bone pain before diagnosis. With complete treatment, patient's symptoms swiftly dissipate. We report the case of a 63-year-old previously healthy man with a 20-month course of musculoskeletal symptoms before diagnosis and resection of a posterior tibial tumor. Postoperatively, the patient had returned to his previous lifestyle wh

Intramedullary osteosynthesis for fracture associated with osteogenesis imperfecta.

Our aim was to analyse the complications associated with intramedullary stabilisation of the bone fractures which are a common complication of osteogenesis imperfecta. A total of 12 fractures among six individuals with osteogenesis imperfecta were treated by intramedullary stabilisation. The mean age of the patients (three male and three female) was 19.4 years, range 7-42 years. The most common fracture site was the femoral midshaft (seven fractures). After implant removal, one new fracture and one re-fracture occurred. Operative stabilisation of fractures is a safe treatment option for osteog

Proximal femoral nail in intertrochanteric femoral fractures.

INTRODUCTION: Proximal femoral nails (PFN) have been introduced recently but have begun to compete the traditional dynamic hip screw. The mechanical strength of the nail and less invasive procedure has made the procedure preferable. This is a short retrospective review of 26 cases operated in the last two years. METHODS: A retrospective review was conducted of 26 cases operated during two years of study which had completed at least a year of follow up. All had been treated using a proximal femoral nail for unstable intertrochanteric fractures of femur. A radiological assessment was made with

Predictors of reoperation following operative management of fractures of the tibial shaft.

BACKGROUND: Accurate prediction of likelihood of reoperation in patients with tibial shaft fractures would facilitate optimal management. Previous studies were limited by small sample sizes and noncomprehensive examination of possible risk factors. OBJECTIVE: We conducted an observational study to determine which prognostic factors were associated with an increased risk of reoperation following operative treatment in a heterogeneous population of patients with tibial shaft fractures. DESIGN: Retrospective observational study. SETTING: Level 1 trauma center. METHODS: We identified 200 patie

Retrograde intramedullary nailing of supracondylar femoral fractures: design and development of a new implant.

Since 1992 we have developed an implant in which the distal (condylar) screws have a diagonal configuration so that the screws can be closer to the distal end of the nail, allowing more distal fractures to be fixed. It also utilises the denser bone of the posterior condyles for more secure fixation in osteoporotic patients. The new implant was used for 24 extra-articular fractures from September 1994 to September 1997, and for 14 articular fractures from February 1995 to December 2000. There was no significant difficulty with obtaining fixation in very distal fractures and in osteoporotic bone

Primary intramedullary nailing of open femoral shaft fractures.

The cases of 60 patients with 63 open femoral fractures treated by primary reamed intramedullary nailing were retrospectively reviewed. Twenty-two were classified as Type I open fractures, 26 as Type II and 15 as Type III open fractures by Gustilo's classification. All fractures were treated by wound debridement followed by immediate reamed intramedullary nailing. There were five early soft-tissue infections and three late deep infections. Of the late infections, only one was osteomyelitis (1.6%). There were three nonunions and seven malunions. All of the complications were dealt with effectiv
