Assessment of clinical outcome of percutaneous needle quadriceps tenotomy in the treatment of congenital knee dislocation.

PURPOSE: Treatment of congenital knee dislocation (CDK) depends on the severity and flexibility of the deformity. Various modalities of treatment ranging from serial cast, open quadricepsplasty and minimally invasive quadricepsplasty have been described. We describe percutaneous needle quadriceps tenotomy for treatment of flexible CDK and present our result of retrospective case series. METHODS: This was a retrospective study of 12 patients (20 knees) with flexible CDK. Eight patients with bilateral and four with unilateral deformities. The mean age of intervention was 14.5 days (range, 4-55

In vivo evaluation of femoral and tibial graft tunnel placement following all-inside arthroscopic tibial inlay reconstruction of the posterior cruciate ligament.

BACKGROUND: The arthroscopic all-inside tibial inlay technique represents a novel procedure for posterior cruciate ligament (PCL) reconstruction. However, in vivo investigations that evaluate the accuracy of this technique regarding anatomic graft tunnel placement are few. The objective of this study was to analyse the femoral and tibial tunnel apertures using computed tomography (CT) and compare these findings to recommendations in the literature. METHODS: CT scans were obtained in 45 patients following single-bundle PCL reconstruction. The centres of the tibial and femoral tunnel apertures

What is the ED95 of prilocaine for femoral nerve block using ultrasound?

BACKGROUND: Our aim was to estimate the ED95 of prilocaine 1% w/v for femoral nerve block. METHODS: This two-stage dose-finding sequential clinical trial followed an adaptive design based on the continual reassessment method (CRM). Adult patients undergoing Vastus medialis muscle biopsy under ultrasound-guided femoral nerve block were recruited. Data from previously published studies and our own previous experience were used to set the dose levels and their guesstimate probabilities of response. RESULTS: Forty patients were recruited in the trial (n=26 in the first stage and n=14 in the seco

The hip antero-superior labral tear with avulsion of rectus femoris (HALTAR) lesion: does the SLAP equivalent in the hip exist?

BACKGROUND: The purpose of this report is to describe a new lesion, the "Hip Antero-superior Labral Tear with Avulsion of Rectus femoris (HALTAR)". This injury may be seen in both adolescent and skeletally mature athletes and shares several characteristics with the "Superior Labral tear from Anterior to Posterior (SLAP)" lesion in the shoulder. METHODS: We present a case example, as well as a detailed anatomic description of the pathological condition. RESULTS: As the rectus femoris crosses two joints it is exposed to substantial forces during muscle contraction. Considering the peri-articul

Timing of Muscle Activation Is Altered During Single-Leg Landing Tasks After Anterior Cruciate Ligament Reconstruction at the Time of Return to Sport.

OBJECTIVES: It is well known that alterations in landing mechanics persist for years after anterior cruciate ligament reconstruction (ACL-R). Nevertheless, existing literature is controversial in reporting successful or unsuccessful recovery of prelanding muscle activation timing after ACL-R. The study aimed at comparing myoelectric and kinematic patterns during landing tasks between ACL-R and healthy subjects. DESIGN: Cross-sectional study. SETTING: Institutional research laboratory. PATIENTS AND INTERVENTION: Fifteen male athletes after ACL-R using patellar tendon and 11 using hamstrings

Multimodal magnetic resonance imaging of the vastus medialis for quantitative diagnosis and grading in early-stage knee osteoarthritis and its correlation with severity.

Early-stage knee osteoarthritis (EKOA) requires timely diagnosis. The recent diagnostic criteria for EKOA by the bone and joint branch of the Chinese geriatric care association remain limited. These limitations necessitate the development of novel, clinical non-invasive imaging modalities to quantitatively evaluate knee joint structures and improve diagnostic accuracy in EKOA. We aimed to investigate whether the quantitative parameters of intravoxel incoherent motion diffusion-weighted imaging (IVIM-DWI) and fat analysis calculation technique (FACT) may differentiate between patients with EKOA

Focus of Attention Impacts Brain Activity and Connectivity Early After Anterior Cruciate Ligament Reconstruction.

CONTEXT: Individuals who undergo anterior cruciate ligament reconstruction (ACLR) have altered sensorimotor brain activity that can persist for years. Directing an individual's focus of attention (FoA) using instructional cues during rehabilitation and motor control training can impact movement performance, but the direct effects on sensorimotor brain activity and network-level relationships in an ACLR population are less understood. This can have important implications for understanding the neural underpinnings of automatic control processes for direct application to motor learning. OBJECTIV

Point-of-Care Ultrasound-Based Assessment of Sarcopenia to Predict Revisits or Mortality in Older Adults at the Emergency Department: A Prospective Observational Study.

BACKGROUND: Older patients visiting the emergency department (ED) are at increased risk of adverse outcomes, including ED revisits and mortality. Sarcopenia quantification by point-of-care ultrasound (POCUS) may be a useful bedside screening tool, especially when traditional frailty screening instruments, reliant on history taking, cannot be used. This study evaluated whether POCUS measurement of rectus femoris cross-sectional area (RFcsa) can predict adverse outcomes in older patients visiting the ED. METHODS: In this single-centre prospective study, patients aged ≥70 years presenting to the

Quadriceps tendon autograft ACL reconstruction has less pivot shift laxity and lower failure rates than hamstring tendon autografts.

PURPOSE: Quadriceps tendon (QT) autograft ACL reconstruction was hypothesized to possess less anterior knee laxity, pivot shift laxity, and lower failure rates than hamstring tendon (HT) autografts. METHODS: Terms "hamstring tendon autograft" and "ACL reconstruction" or "quadriceps tendon autograft" and "ACL reconstruction" were searched in Embase and PubMed. Inclusion criteria required that studies included patients treated for primary ACL injury with reconstruction using either a QT autograft (Group 1) or a HT autograft (Group 2) and instrumented anterior knee laxity assessment. Extracted i

Relationships between Quadriceps Tendon Elasticity and Knee Flexion Angle in Young Healthy Adults.

Background and objectives: Although tendon elasticity by elastography is useful for diagnosing tendon disorders and planning rehabilitation regimens of the tendon, there are few reports on the quadriceps tendon. Moreover, relationships between the quadriceps tendon elasticity and knee angle have not been investigated. The purpose of this study was to clarify the relationship between quadriceps tendon elasticity and knee flexion angle in young healthy adults using elastography, and to investigate the difference in elasticity by sex and leg dominance. Materials and Methods: A total of 40 knees i

Performance of stair negotiation in patients with cerebral palsy and stiff knee gait.

BACKGROUND: Due to the limited knee range of motion, achieving adequate foot clearance while walking on level ground constitutes a major problem for patients with cerebral palsy and stiff knee gait. Stair negotiation as an activity of daily life requires a considerably higher knee range of motion than level ground walking, but little is known yet as to whether such patients are able to walk stairs. RESEARCH QUESTION: The aim of this study was to investigate how patients with a limited knee range of motion negotiate stairs. Do they increase their peak knee flexion and use the same pattern as i

Non-uniformity in the healthy patellar tendon is greater in males and similar in different age groups.

There is increasing evidence that tendons are heterogeneous and take advantage of structural mechanisms to enhance performance and reduce injury. Fascicle-sliding, for example, is used by energy-storing tendons to enable them to undergo large extensions while protecting the fascicles from damage. Reductions in fascicle-sliding capacity may thus predispose certain populations to tendinopathy. Evidence from the Achilles tendon of significant superficial-to-deep non-uniformity that is reduced with age supports this theory. Similar patellar tendon non-uniformity has been observed, but the effects
