Treatment of a case of serious occlusal instability associated with TMDs through a modern mini-invasive approach.

BACKGROUND: The latest trends in restorative dentistry are guided both by esthetic and mini-invasive procedures. New ceramic materials, such as lithium disilicate, allow the clinician to obtain invisible restorations with a minimum preparation thickness. SUMMARY: A young female patient presented at our practice with left temporomandibular closed locking and uncomfortable occlusal instability. After manual reduction, medical therapy, and reversible treatment with a stabilization splint, a mini-invasive lithium disilicate occlusal inferior posterior rehabilitation was conceived and performed. Short-Term Outcome of Tension Band Wiring for Medial Malleolar Fractures.

INTRODUCTION: The structural integrity of the medial malleolus is critical for maintaining the stability of the ankle mortise. While various internal fixation strategies exist, tension band wiring (TBW) leverages the principle of dynamic compression by converting tensile forces into compressive forces during early mobilization. This technique remains highly relevant in clinical practice, particularly for addressing small fracture fragments and transverse patterns. MATERIALS AND METHODS: This prospective observational study evaluated 20 skeletally mature patients presenting with closed Herscov

Atypical Femur Fractures-An Analysis of 69 Patients from 15 Years.

Background/Objectives: Bisphosphonates are effective in preventing osteoporotic fractures. However, the risk of atypical femur fractures (AFFs) increases with long-term bisphosphonate use. There are few existing publications on the analysis of clinical outcomes of atypical femur fracture cases in Chinese patients. Our objective was to review the clinical outcomes of AFF cases managed in a tertiary center in Hong Kong, China. Methods: Cases of AFF managed in the Prince of Wales Hospital from 2010 to 2024 were included. Data on demographics, type and duration of bisphosphonate use prior to AFF,

Successful management of respiratory distress in fat embolism syndrome: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Treatment of respiratory distress in fat embolism syndrome (FES) often ends in death. FES is generally associated with orthopedic trauma, with the highest incidence in long bone fractures. CASE PRESENTATION: We present a case, 20-year-old woman with an initial diagnosis of a closed fracture in the right femur and left cruris after traffic accidents, underwent open reduction and internal fixation 17 h after an incident. Thirty hours after the surgery, she experienced respiratory distress, loss of consciousness, and petechial rash. Thoracic radiography revealed bila

Forecasting effects of "fast-tracks" for surgery in the Swedish national guidelines for distal radius fractures.

BACKGROUND AND PURPOSE: National guidelines for treatment of distal radius fractures (DRFs) were presented in Sweden in 2021. In the guidelines, a fast-track is recommended for 4 subgroups of highly unstable DRFs. Regardless of the results of the closed reduction these are recommended for surgery within 1 week of injury. This study aims to evaluate the potential consequences of the newly presented national guidelines on incidence of surgical interventions. PATIENTS AND METHODS: In all, 1,609 patients (1,635 DRFs) with primary radiographs after a DRF between 2014 and 2017 at two Swedish hospit

Quadruple Versus Double Flexible Intramedullary Nails to Treat Pediatric Distal-third Tibial Shaft Fractures: A Biomechanical Comparison.

BACKGROUND: The vast majority of pediatric distal-third tibial shaft fractures can be treated with closed reduction and casting. If conservative measures fail, then these fractures are usually treated with 2 antegrade flexible intramedullary nails. A postoperative cast is usually applied because of the tenuous fixation of the 2 nails. Recent studies have described the use of 4 nails to increase the stability of the fixation, a technique that may preclude the need for postoperative casting. The purpose of this biomechanical study is to quantify the relative increase in stiffness and load to fai

Comparison of the outcomes of cannulated screws vs. modified tension band wiring fixation techniques in the management of mildly displaced patellar fractures.

BACKGROUND: K wire fixation with tension band wiring has conventionally been used for the open reduction and internal fixation of the patella. However, it suffers from distinct disadvantages such as implant irritation, need for open reduction, incidence of palpable implants, and need for subsequent implant removal. A smaller incision with percutaneous fixation may be an alternative to this established conventional technique. Thus, the purpose of this trial was to compare the treatment outcomes of patients with mildly displaced patellar fractures treated with closed reduction and percutaneous c

How long does it to achieve sagittal realignment of the displaced epiphysis in Salter-Harris type II distal radial fracture when treated by manual reduction?

This study aimed to investigate how long it takes for the dorsally displaced distal radial epiphysis to achieve realignment. We retrospectively reviewed 56 patients with dorsally displaced Salter-Harris type II distal radial epiphyseal fractures who were aged ≤15 years at the time of injury. All fractures were treated with closed reduction and immobilised using a sugar tong splint for 6 weeks. We evaluated the change in the displaced epiphysis position (%) until 12 weeks and the long-term clinical and radiological outcomes. We analysed significant differences in demographic factors and epiphys

Role of Toll-Like Receptor 4 on Osteoblast Metabolism and Function.

Inflammation is a process whose main function is to fight against invading pathogens or foreign agents. Nonetheless, it is widely accepted that inflammation takes part in multiple processes in a physiological or pathophysiological context. Among these processes the inflammation has been closely related to bone metabolism. It is well-known that in systemic inflammatory diseases such as rheumatoid arthritis the inflammatory environment contributes to the reduction of the bone mineral density. This has been further evidenced in different animals models of osteoporosis where the deletion of key in
