Mind the gap: Adherence to denosumab dosing and cessation guidelines in Australian residential aged care.

AIMS: Denosumab and bisphosphonates are the main treatments for osteoporosis in residential aged care (RAC). Strict adherence to 6-monthly denosumab dosing is critical as delayed doses can result in bone resorption and vertebral fractures. If denosumab is ceased, then a bisphosphonate is recommended to avoid vertebral fractures. We aimed to examine adherence to denosumab dosing guidelines, practices of ceasing or switching to bisphosphonate treatment and how these vary in Australian RAC. METHODS: A retrospective cohort study using routinely collected electronic medication administration data.

Relationship between bisphosphonate use and risks of recurrence and mortality among hormone receptor-positive and triple-negative breast cancer patients.

BACKGROUND: Bisphosphonates (BPs) are routinely used as an adjuvant therapy for breast cancer to reduce the risk of bone recurrences given their anti-resorptive effects on bones and antitumor characteristics. However, there remains limited evidence regarding their utility across different subtypes. METHODS: In this population-based cohort study, the authors assessed the effects of pre- and post-diagnostic BP use on recurrence, any second breast cancer event, all-cause mortality (ACM), and breast cancer-specific mortality (BCSM) using multivariable-adjusted Cox proportional hazards models amon

Factors Associated With Compliance and Persistence With Pharmacotherapy in Patients With Osteoporosis: A Nationwide Cohort Study in Korea.

BACKGROUND: Despite the necessity of long-term management for fracture risk reduction, adherence to osteoporosis pharmacotherapy remains poor. We investigated the factors influencing adherence to pharmacotherapy among Korean patients with osteoporosis, with a particular focus on treatment with bisphosphonates (BPs). METHODS: Data from 725,313 osteoporosis patients newly prescribed BPs or selective estrogen receptor modulators (SERMs) between 2012 and 2014, obtained from the Korean National Health Insurance Service, were analyzed. Adherence was assessed based on compliance and persistence over

Well-reduced bisphosphonate-associated atypical femur fractures have low rates of nonunion and delayed union.

INTRODUCTION: Bisphosphonate-associated atypical femur fractures have been reported to have high rates of delayed union and nonunion, as well as high rates of contralateral fracture. There have been limited studies looking at specific factors associated with delayed union or nonunion of atypical fractures. Many studies are limited by small numbers of patients. To date, no studies have examined the effect of reduction technique (closed versus open versus percutaneous) and reduction quality on healing. METHODS: This was a multicentered retrospective review of patients greater than 50 years of a

Radiographic Analysis of MRONJ in Osteoporotic and Oncologic Patients on Bisphosphonates or Denosumab.

INTRODUCTION: Medication-Related Osteonecrosis of the Jaw (MRONJ) is a severe adverse effect of antiresorptive and antiangiogenic treatment, primarily in osteoporotic and oncologic patients. The disease is characterized by persistent jawbone necrosis that results in significant impairment of the quality of life of involved patients. The aim of this study is to compare the radiographic characteristics of MRONJ between osteoporotic and oncologic patients and assess the differences based on treatment type, particularly bisphosphonates and denosumab (RANK-L inhibitors). METHODS: A retrospective a

Peri-implant inflammation increases the risk of osteonecrosis in mice treated with bisphosphonate.

BACKGROUND: Bisphosphonates (BPs) are effective in managing bone diseases due to their anti-resorptive properties but are linked to medication-related osteonecrosis of the jaw (MRONJ), particularly concerning dental implants. This study explored the combined impact of ligature-induced peri-implant inflammation and zoledronic acid (ZA), a BP, using a murine model. METHODS: Twenty-four mice underwent bilateral maxillary molar extractions and implant placements, with ZA or vehicle treatment and ligature placement on the left side. Two groups were defined: group 1 (vehicle-treated) with control (

Local delivery of pamidronate with collagen matrix mitigates buccal bone resorption following immediate implant placement - An experimental in vivo study.

AIMS: The aim of this experimental in vivo pilot study was to evaluate the effect of the local delivery of pamidronate within a collagen membrane on the changes in the buccal soft and hard tissue dimensions at the time of immediate implant placement and whether this effect was influenced by the placement of bone substitutes. METHODS: In six beagle dogs, the distal roots of the third and fourth premolars were extracted, and immediate implants were placed. Treatment groups were randomly allocated to each socket: (i) covering the buccal bone with pamidronate-soaked collagen membrane (BP group),

Outcome of COVID19 in Patients With Osteogenesis Imperfecta: A Retrospective Multicenter Study in Saudi Arabia.

BACKGROUND: Although genetic diseases are rare, children with such conditions who get infected with COVID-19 tend to have a severe illness requiring hospitalization. Osteogenesis imperfecta (OI) is a rare genetic disorder of collagen resulting in fractures and skeletal deformities. Kyphoscoliosis, restrictive lung disease, and pneumonia worsen the prognosis of patients with OI. The use of bisphosphonate improves bone mineral density (BMD) and reduces fractures in OI. There is no literature describing the impact of COVID-19 in patients with OI. METHODOLOGY: A retrospective multi-center study w

Medication use quality and safety in older adults: 2020 update.

Improving the quality of medication use and medication safety are important priorities for prescribers who care for older adults. The objective of this article was to identify four exemplary articles with this focus in 2020. We selected high-quality studies that moved the field of research forward and were not merely replication studies. The chosen articles cover domains related to deprescribing, medication safety, and optimizing medication use. The first study, a noninferiority randomized clinical trial in England, evaluated whether antihypertensive medication reduction is possible without si

Bisphosphonate Use and Breast Cancer Risk among Women with Ductal Carcinoma In Situ.

Women with a history of ductal carcinoma in situ (DCIS) have an elevated risk of a subsequent invasive breast cancer, but there are few established potentially modifiable factors known to lower this risk. Bisphosphonates are a commonly used treatment for patients with osteoporosis and have been shown to lower risks of recurrence and mortality in patients with invasive breast cancer; however, their use has not previously been investigated within the context of DCIS. Utilizing a population-based nested case-control design, we compared 301 cases of women diagnosed with DCIS and a subsequent breas

Sensory denervation increases potential of bisphosphonates to induce osteonecrosis via disproportionate expression of calcitonin gene-related peptide and substance P.

Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a serious side effect of systematic administration of bisphosphonates (BPs). Sensory innervation is crucial for bone healing. We established inferior alveolar nerve injury (IANI) and inferior alveolar nerve transection (IANT) models characterized by disorganized periosteum, increased osteoclasts, and unbalanced neuropeptide expression. Zoledronate injection disrupted neuropeptide expression in the IANI and IANT models by decreasing calcitonin gene-related peptide (CGRP) and increasing substance P (SP); associated with this, BRONJ preva

Does the use of preoperative bisphosphonates in patients with osteopenia and osteoporosis affect lumbar fusion rates? Analysis from a national spine registry.

OBJECTIVE: Bisphosphonates are used to increase bone strength in treating osteopenia and osteoporosis, but their use for increasing lumbar fusion rates has been controversial. The objective of this study was to determine if preoperative treatment with bisphosphonates affects the reoperation rates for nonunions (operative nonunion rates) following lumbar fusions in patients with osteopenia or osteoporosis. METHODS: The authors conducted a cohort study using data from the Kaiser Permanente Spine Registry. Patients (aged ≥ 50 years) with a diagnosis of osteopenia or osteoporosis who underwent pr

Osteogenesis Imperfecta: Skeletal Outcomes After Bisphosphonate Discontinuation at Final Height.

Intravenous cyclical bisphosphonates are widely used to treat children with moderate to severe osteogenesis imperfecta (OI). Bisphosphonates are often discontinued when growth is completed, but subsequent skeletal changes have not been studied in detail. We assessed 31 patients (22 females) with OI who had started intravenous bisphosphonates (either pamidronate or zoledronic acid) before 13 years of age, were treated for at least 2 years (range 4.7-15.7 years), and discontinued treatment after completion of growth, when their age ranged from 13.4 to 20.0 years (mean 16.4 years). At 4 years aft

Endothelial Progenitor Cells inhibit jaw osteonecrosis in a rat model: A major adverse effect of bisphosphonate therapy.

Medication-related osteonecrosis of the jaw (MRONJ) is a serious adverse effect of antiresorptive and antiangiogenic therapies. MRONJ is identified by chronic wounds in the oral mucosa associated with exposed necrotic bone. We hypothesized that zoledronic acid (ZOL) impairs keratinocyte and fibroblast function and reduces soft tissue vascularization; therefore, treating MRONJ with proangiogenic cells may benefit MRONJ patients. The effect of ZOL and dexamethasone (DEX) on gingival fibroblasts and keratinocytes was investigated. In-vitro, ZOL inhibited fibroblast and keratinocyte proliferation,

Idiopathic Inflammatory Myopathy and Antisynthetase Syndrome: Early Diagnosis by 99mTc-HDP Bone Scintigraphy.

We present a 44-year-old woman with scapular and pelvic muscle weakness, joint inflammation, and fever. Bone scintigraphy showed high uptake in proximal regions of upper and lower limbs, suspecting inflammatory myopathy and polyarticular damage. These features were the clue to request other complementary tests such as anti-aminoacyl-tRNA-synthetase antibodies and chest CT, which showed interstitial lung disease, defining an antisynthetase syndrome. Therefore, BS allows an earlier diagnosis of inflammatory muscle disease and to identify the optimal site for muscle biopsy. BS is an important ste
