Frontomaxillary facial angle in chromosomally normal fetuses at 11 + 0 to 13 + 6 weeks.

OBJECTIVE: To establish the normal range of the frontomaxillary facial (FMF) angle at 11 + 0 to 13 + 6 weeks of gestation. METHODS: In this prospective study, three-dimensional (3D) volumes of the fetal head were obtained from 500 pregnancies before fetal karyotyping by chorionic villus sampling (CVS), after screening by fetal nuchal translucency (NT) thickness and maternal serum free beta-human chorionic gonadotropin (beta-hCG) and pregnancy-associated plasma protein-A (PAPP-A) at 11 + 0 to 13 + 6 weeks. Only cases with a normal karyotype were included in this study. The FMF angle was measur

Cephalometric assessment of cranial abnormalities in patients with acromegaly.

OBJECTIVE AND PATIENTS: Patients with acromegaly (12 women, 26 men) and a control group (36 women, 50 men) were chosen for cephalometry to assess the size, shape and positional characteristics of the craniofacial bones and the upper airways. RESULTS: When compared with the controls, patients of both sexes with acromegaly were found to have significant anomalies in the orofacial skeleton: increased facial height, elongated ascending ramus mandibulae and greater basion-supramentale distance, a negative difference between maxillary and mandibular protrusions, enlarged lower part of the gonion an

Early combined management of frontal sinus and orbital and facial fractures.

BACKGROUND: Historically, frontal sinus fractures and associated maxillofacial fractures were addressed at different times. We retrospectively reviewed our experience with a series of 79 consecutive patients with bitable frontal sinus fractures, most with associated facial fractures, who underwent early combined cranial and maxillofacial procedures. METHODS: The operations were performed emergently for patients with open or depressed skull fractures and hematomas with mass effect, and electively within 2 weeks for those with closed fractures. The cranial procedures were performed first. The n

Neurosurgical treatment of traumatic frontobasal CSF fistulae in 300 patients (1967-1989).

300 patients with traumatic fronto-basal CSF fistulae were treated neurosurgically from 1967 to 1989, i.e. via a transcranial approach. An additional intracranial space occupying lesion, a direct open endocranium and/or a large basal brain hernia was found in 68%. The following aspects were discussed: progress in establishing the exact anatomical diagnosis, especially neuroradiological advances; questions of the indication for surgery; the problem of the optimal surgical approach. Both the cranial and the rhinological approach have advantages and disadvantages. Their indications only partly co

Microscopic minimally invasive approach to nonsyndromic craniosynostosis.

PURPOSE: The purpose of this retrospective study was to present the results of the authors' microscopic minimally invasive approach in the treatment of nonsyndromic craniosynostosis. METHODS: From 2001 to 2007, the authors treated a cohort of 67 infants with nonsyndromic sagittal, unicoronal, bicoronal, and metopic craniosynostosis, either with the microscopic (n = 40) or the open (n = 27) approach. In the microscopic approach, incisions were placed over the premature suture, and using a surgical microscope, the appropriate synostectomy was performed. The open approach used a traditional coro

Acute isovolaemic haemodilution in two Jehovah's Witnesses presenting for major intracranial surgery.

The Jehovah's Witnesses (JWS) is a religious sect with strong convictions against acceptance of blood and its products for medical care, including surgery. We present two cases of JWS, aged 24 and 19 years old, who had craniectomy for trauma and craniotomy for tumour excision, respectively. A team approach was employed in the care of both patients. During exhaustive preoperative discussions they re-affirmed their religious convictions but agreed to some modifications. Both patients accepted Acute Isovolaemic Haemodilution (AIH). However one of them insisted on non-discontinuation of the line u

Traumatic perilymphatic fistula with pneumolabyrinth: diagnosis and management.

A perilymphatic fistula (PLF) is an abnormal communication between the inner and middle ear resulting in vestibular or cochlear symptoms. We review three pediatric traumatic temporal bone fractures with pneumolabyrinth, confirmed radiologically by the presence of air within the cochlea (pneumocochlea) or vestibule (pneumovestibule). Patients were treated conservatively with complete resolution of vestibulopathy. Hearing outcome was variable and worse in two patients with pneumocochlea. A pneumolabyrinth on radiologic imaging confirms a PLF and obviates the need for exploration to reach a diagn

On equal footing: Trends in ankle/foot injuries for men vs. women.

OBJECTIVES: The objective of the current study was to examine trends in ankle/foot (A/F) injuries during the period 2001-2014, in order to determine whether the incidence of these injuries has changed and whether a previously identified difference in risk by gender still existed. In addition, other driver and crash-related risk factors were examined separately for men and women. METHODS: Passenger vehicle drivers aged 16+ were identified from NASS-CDS; weighted data were analyzed for model years 2001-2014. Model years (MY) were grouped as 2001-2004 (older) vs. 2005-2014 (newer), and drivers i

Morphology and topography of the supraorbital notch and foramen in medieval and contemporary skulls.

BACKGROUND: The supraorbital foramen or notch constitutes an important anatomical landmark for maxillofacial surgery, regional anesthesia, a … hological variant in both medieval and contemporary groups. Although certain differences in lateral frontal bone measurements were observed, the general topography of the supraorbital foramen appears relatively stable. The observed variability and asymmetry should be considered during surgical and anesthetic procedures involving the supraorbital region.

Parry-Romberg Syndrome: Radioclinical Dissociation in a Paucisymptomatic Form and a Proposed Diagnostic Framework.

Background and Clinical Significance: Parry-Romberg syndrome (PRS), also known as progressive hemifacial atrophy, is a rare disorder characterized by progressive unilateral hemifacial atrophy, with potential involvement of the cranial bones and the brain. Although neurological manifestations are frequently described, central nervous system involvement may be subclinical and detectable only through neuroimaging. Owing to its rarity and the lack of standardized diagnostic criteria, most available data derive from case reports and small case series. Case Presentation: We report the case of a 38-y

Solitary brain metastasis from laryngeal squamous cell carcinoma in a 68-year-old male: A Case Report.

INTRODUCTION AND IMPORTANCE: Laryngeal squamous cell carcinoma (LSCC) commonly metastasizes to regional lymph nodes, lungs, liver, and bones. Intracranial metastasis from LSCC is exceedingly rare, reported in only 0.4% of cases. The atypical presentation can delay diagnosis and negatively impact prognosis. We report a rare case of solitary brain metastasis from LSCC, highlighting the diagnostic challenges and clinical considerations. CASE PRESENTATION: A 68-year-old Pakistani male with a prior diagnosis of LSCC presented with new-onset generalized tonic-clonic seizures. He had previously unde

An Investigation of Hyperostosis Frontalis Interna in a Modern Anatomical Body Donor Population.

This research sought to examine the prevalence and severity of hyperostosis frontalis interna (HFI) in the Chicagoland anatomical body donor population. The study further aimed to elucidate potential demographic risk factors for HFI, including sex, age at death, and structural vulnerability index (SVI), as well as any common comorbidities, as gleaned from death certificates. HFI is an irregular bony overgrowth of the endocranial surface of the frontal bone. It is most often observed in postmenopausal women or in individuals with growth hormone disorders. This work investigated the distribution…

Learning curves of two surgical robot systems for assisted total knee arthroplasty and their impact on early patient clinical outcomes: a retrospective study.

As robotic-assisted total knee arthroplasty (TKA) continues to evolve, a key clinical question is whether the learning curve and clinical outcomes differ between CT-dependent and image-independent robotic systems. This retrospective study compared the learning curves and early clinical outcomes of 101 patients undergoing TKA with either a CT-dependent robotic system (Beijing HURWA, Group A) or an image-independent system (Smith & Nephew CORI, Group B). A statistically significant intergroup disparity (P < 0.05) was noted in the rates of transition to conventional treatment, with Group A exhibi

Biomechanical evaluation of zygomatic-orbital-maxillary complex fractures following internal fixation.

PURPOSE: The aim of this study is to develop effective treatment strategies for zygomatic-orbital-maxillary complex (ZOMC) fractures using finite element analysis (FEA) and investigate the mechanical properties and stress distribution of different internal fixators. METHODS: A three-dimensional (3D) model was created from CT images of a patient with a unilateral ZOMC fracture, and the preinjury craniofacial structure was reconstructed. A cylindrical impactor was used to simulate facial impact. Biomechanical analysis was performed to assess the stability of internal fixation of the fracture li

Cerebral Metastasis of a Gastrointestinal Stromal Tumor: A Case Report and Literature Review.

Gastrointestinal stromal tumors (GISTs) are rare mesenchymal neoplasms originating from the gastrointestinal tract. These tumors frequently harbor activating mutations in the receptor tyrosine kinase (KIT) or in the platelet-derived growth factor receptor alpha (PDGFRA) gene, which guide treatment with tyrosine kinase inhibitors (TKIs). Although GISTs commonly metastasize to the liver and peritoneum, involvement of the central nervous system (CNS) is exceptionally uncommon. We report a case of a 58-year-old male with metastatic gastric GIST, treated with imatinib, followed by sunitinib and reg

3D printing and acute maxillofacial trauma: an overview of the literature.

BACKGROUND: The consequences of maxillofacial injuries are functionals and aesthetics. The initial treatment must lead to consolidation without any sequelae (complete healing with no clinical symptoms). 3D printing is increasingly used in maxillofacial surgery, but data on its use in the initial phase of trauma is scarce. METHOD: We conducted a bibliographic search in PubMed’s electronic database with the following terms: maxillofacial traumatology, maxillofacial fracture, mandible fracture, maxillary fracture, zygoma fracture, LeFort fracture, fracture of the naso-ethmoido-maxillo-fronto-orbi
