Myopathy with ragged red fibres following renal transplantation: possible role of cyclosporin-induced hypomagnesaemia.

A 65-year-old man developed a painless proximal myopathy 7 years after commencing cyclosporin therapy following a successful cadaveric renal transplant. Muscle histology showed ragged red fibres and biochemical studies suggested mitochondrial dysfunction. The possible pathogenesis of this mitochondrial myopathy, particularly its relationship to cyclosporin-induced hypomagnesaemia, is discussed.

Coronary arterial lesions in Mg-deficient hamsters.

Dietary Mg deficiency is thought to promote coronary artery spasm, probably through Ca overloading of vascular smooth muscle (VSM) cells. We now report the demonstration of a coronary arteriopathy in hamsters fed a Mg-deficient diet for 10 days or more. Affected arteries showed endothelial and VSM cell hyperplasia and pleomorphism, chronic inflammation of the media and adventitia, and fibrinoid necrosis. Lesions of myocardial ischemia, distinct from the well-known lesions of myocardial necrosis and calcification common in Mg deficient animals, were also present. Von Kossa positive granules wer

CNNM2 homozygous mutations cause severe refractory hypomagnesemia, epileptic encephalopathy and brain malformations.

Magnesium (Mg2+) plays a crucial role in many biological processes especially in the brain, heart and skeletal muscle. Mg2+ homeostasis is regulated by intestinal absorption and renal reabsorption, involving a combination of different epithelial transport pathways. Mutations in any of these transporters result in hypomagnesemia with variable clinical presentations. Among these, CNNM2 is found along the basolateral membrane of distal tubular segments where it is involved in Mg2+ reabsorption. To date, heterozygous mutations in CNNM2 have been associated with a variable phenotype, ranging from i

Exome sequencing identifies a novel frameshift variant causing hypomagnesemia with secondary hypocalcemia.

Hypomagnesemia with secondary hypocalcemia is a rare autosomal-recessive disorder characterized by intense hypomagnesemia associated with hypocalcemia (HSH). Mutations in the TRPM6 gene, encoding the epithelial Mg2+ channel TRPM6, have been proven to be the molecular cause of this disease. This study identified causal mutations in a 2-month-old male patient of hypomagnesemia from a consanguineous marriage. Biochemical analyses indicated the diagnosis of HSH due to primary gastrointestinal loss of magnesium. Whole exome sequencing of the trio (i.e. proband and both parents) was carried out with

Hypomagnesemia and the Risk of New-Onset Diabetes Mellitus after Kidney Transplantation.

Several studies suggest a link between post-transplant hypomagnesemia and new-onset diabetes after transplantation (NODAT), but this relationship remains controversial. We conducted a retrospective cohort study of 948 nondiabetic kidney transplant recipients from January 1, 2000, to December 31, 2011, to examine the association between serum magnesium level and NODAT. Multivariable Cox proportional hazards models were fitted to evaluate the risk of NODAT as a function of baseline (at 1 month), time-varying (every 3 months), and rolling-average (i.e., mean for 3 months moving at 3-month interva

New TRPM6 mutation and management of hypomagnesaemia with secondary hypocalcaemia.

BACKGROUND: TRPM6 gene mutation has been reported to cause hypomagnesemia with secondary hypocalcemia (HSH). However, the genotype-phenotype correlation for TRPM6 gene mutations has not been clarified. OBJECTIVE: To elucidate the factors underlying the severe neurological complications in HSH and evaluate the potential association between the location of TRPM6 gene mutations and clinical data of HSH. METHODS: A Japanese patient diagnosed with HSH at 10 weeks of age exhibited neurological damage and failed to thrive. Magnesium supplements were therefore started at 12 weeks of age. Mutational

Background potassium concentrations and epileptiform discharges. I. Electrophysiological characteristics of neuronal activity.

Intra- and extracellular recording techniques were used to study the epileptiform activity generated by guinea pig hippocampal slices perfused with free-magnesium artificial cerebrospinal fluid in the presence of physiologic (4 mM), reduced (2 mM) or elevated (8 mM) extracellular potassium concentrations ([K(+)](o)). Extracellular field potentials along with intracellular recordings were recorded in CA1 or CA3 region. Reduction of [K(+)](o) significantly increased the latency of epileptiform field potential (EFP) appearance as well as burst discharge duration and decreased EFP repetition rate.

[Hypomagnesemia and hypokalemia as risk factors of arrhythmia in acute phase myocardial infarction].

In a uniform group of 30 patients the A. have assessed the incidence of lower serum K+ and Mg++ concentrations on the probability of arrhythmias onset in the acute phase of myocardial infarction. The study shows that lower K+ and Mg++ levels significantly influence only the onset of ventricular tachycardia and monofocal PVC.
