Influence of preoperative factors on outcome of shoulder arthroplasty for glenohumeral osteoarthritis.

BACKGROUND: The results of shoulder arthroplasty for osteoarthritis have been reported to be excellent or good for the majority of patients, but the value of using a glenoid component and the anatomic factors that affect outcome are still debated. The purpose of this study was to evaluate the influence of an operatively confirmed full-thickness tear of the rotator cuff, the severity of preoperative erosion of glenoid bone, preoperative radiographic evidence of subluxation of the humeral head, and the severity of preoperative loss of the passive range of motion on the outcome of total shoulder

Myocardial Infarction Within 6 Months Before Arthroscopic Rotator Cuff Repair Is Associated With Significantly Increased Risk of Postoperative Myocardial Infarction.

PURPOSE: To evaluate the association between myocardial infarction (MI) timing within 2 years prior to arthroscopic rotator cuff repair (aRCR) and postoperative cardiac, medical, and surgical complications. METHODS: The PearlDiver Mariner Database (2010-2022) was queried to identify patients undergoing aRCR (CPT-29827). Patients with an MI within 2 years before surgery were propensity matched 1:4 to controls without MI based on demographics and comorbidities. Subgroup analyses evaluated MI timing (0-6, 6-12, 12-18, and 18-24 months), MI type (type 1 vs type 2, ST-elevation MI vs non-ST-elevat

Evaluation of animal models and methods for assessing shoulder function after rotator cuff tear: A systematic review.

BACKGROUND AND OBJECTIVE: Restoring the shoulder function is a crucial demand of patients with rotator cuff (RC) tears. Most preclinical studies only focused on biological and mechanical measurements. Functional assessment was less investigated in the preclinical studies. This study aims to review the literature of shoulder function in animal models for RC tears and evaluate the strengths and weaknesses of different shoulder functional assessments and animal models. METHOD: A literature search for studies used RC tear animal models to evaluate changes in shoulder function was performed. We se

Description of outcomes of upper-extremity musculoskeletal disorders in workers highly exposed to repetitive work.

PURPOSE: The outcomes of workers with upper-extremity musculoskeletal (UEMS) symptoms or disorders were evaluated over a 3-year period in a population highly exposed to repetitive work. METHODS: Data were derived from a survey focused on the health effects of repetitive tasks, with a standardized physical examination by an occupational physician, in 1993 to 1994 and again in 1996 to 1997. All workers with UEMS symptoms or disorders in 1993 to 1994 were included. We classified 3-year outcomes into 3 categories in 1996 to 1997: neither UEMS disorder nor symptoms, UEMS symptoms only, and UEMS di

Systematic review of shoulder arthroplasty outcomes: what sample size is meaningful?

BACKGROUND: Shoulder arthroplasty is increasingly performed for shoulder conditions such as arthritis, rotator cuff arthropathy, and traumatic injuries. Registries and other compilations of patient data provide the opportunity to detect meaningful differences in outcomes between alternative techniques and implants. A wide range of outcome measurements are reported after shoulder arthroplasty, but the sample sizes needed to identify meaningful differences have not been studied systematically. This review systematically analyzes common clinical outcomes reported for shoulder arthroplasty and rep

Association of post-stroke hemiplegic shoulder pain with sleep quality, mood, and quality of life.

BACKGROUND: Hemiplegic Shoulder Pain (HSP) is a prevalent post-stroke complication, characterized by paralysis, spasticity, altered sensation, neuropathic pain, shoulder subluxation, and soft tissue injuries such as rotator cuff tears and bicipital tendonitis. Muscle imbalances, weakness, and altered scapular positioning further contribute to the exacerbation of HSP. These factors lead to poorer functional outcomes, reduced hand function, and difficulties in performing daily activities. HSP is often associated with substantial mental and physical health burdens. The disorder significantly impa

Joint-preserving treatment options for irreparable rotator cuff tears.

An irreparable cuff tear is defined as the inability to achieve direct repair of native tendon to the great tuberosity despite intra- and extra-articular release of the remaining tissue. Three distinct anatomic patterns are identified: posterosuperior cuff tears which involve the supraspinatus, infraspinatus and teres minor; anterosuperior tears which involve the supraspinatus and subscapularis; and global tears which comprise both. Subacromial debridement and tenotomy or tenodesis of the long head of the biceps are proposed for older patients with a functional but very painful shoulder. Parti

Combined SLAP repair and biceps tenodesis for superior labral anterior-posterior tears.

PURPOSE: Long-head biceps tenodesis has been suggested as an alternative to superior labral anterior-posterior (SLAP) repair. However, an unrepaired superior labral tear may increase glenohumeral translation, and thus, labral repair may be considered in the setting of biceps tenodesis. METHODS: Patients who underwent tenodesis, SLAP repair, or combined tenodesis and labral repair for SLAP tears were included. The indication for combined tenodesis and labral repair was biceps tendonitis in the setting of a SLAP lesion with labral instability. Demographics, range of motion, return to work, retu

A new technique for reconstruction of the proximal humerus after three- and four-part fractures.

The results of proximal humeral replacement following trauma are substantially worse than for osteoarthritis or rheumatoid arthritis. The stable reattachment of the lesser and greater tuberosity fragments to the rotator cuff and the restoration of shoulder biomechanics are difficult. In 1992 we developed a prosthesis designed to improve fixation of the tuberosity fragments in comminuted fractures of the proximal humerus. The implant enables fixation of the fragments to the shaft of the prosthesis and the diaphyseal fragment using screws, washers and a special toothed plate. Between 1992 and 20
