Frequency of skeletal injuries in children with inflicted burns.

BACKGROUND: It is estimated that inflicted burn injuries in physically abused children occur with a prevalence of approximately 6-20%. Ident … en, classic metaphyseal lesions in three, healing shaft fractures in six, skull fracture in one and clavicle fracture in two children. Fractures in the indeterminate group included shaft fractures in two, one of which was healing. CONCLUSION: Intentional burns in children appear to be more common than previously known, occurring in 40% of the children in …

Scapular neck fracture--the influence of permanent malalignment of the glenoid neck on clinical outcome.

A scapular neck fracture is considered unstable if it is associated with an ipsilateral clavicular fracture or an acromioclavicular (AC) joi … usion, severe displacement of the glenoid neck may occur with or without associated fracture of the clavicle or dislocation of the AC joint and can be identified as a GPA less than 20 degrees. Scapular neck fractures with such malalignment have a less favorable long-term outcome compared with otherwise comparable cases with absence of glenoid malalignment …

Open reduction and internal fixation of clavicular fractures after a delay of three months is associated with an increased risk of complications and revision surgery.

AIMS: The primary aim of this study was to determine if delayed clavicular fixation results in a greater risk of operative complications and revision surgery. PATIENTS AND METHODS: A retrospective case series was undertaken of all displaced clavicular fractures that underwent plate fixation over a ten-year period (2007 to 2017). Patient demographics, time to surgery, complications, and mode of failure were collected. Logistic regression was used to identify independent risk factors contributing towards operative complications. Receiver operating characteristic (ROC) curve analysis was used to

High-grade metachronous osteosarcoma. A case report over a 23-year period.

This is a case report on the remarkable 23-year course of a metachronous osteogenic sarcoma in a 31-year-old man. Histology invariably showed the features of a high-grade osteogenic sarcoma with predominantly chondroblastic cells. During the observed period the patient developed nine osseous metastases. The quiescent clinical course of some metastases was in sharp contrast to the histological pattern. The patient finally died from symptoms of increasing cervical spinal cord compression without ever developing lung metastases.

Combined Clavicular Hook Plate and Coracoid Screw Fixation for Coracoid Process Fractures Associated with Acromioclavicular Joint Dislocation.

Background and Objectives: Coracoid process (CP) fractures combined with acromioclavicular (AC) joint dislocation are extremely rare, and evidence guiding optimal surgical management remains limited. This retrospective, single-center case series study evaluated clinical and radiologic outcomes after simultaneous fixation of both lesions using a clavicular hook plate and a coracoid screw. Materials and Methods: We retrospectively reviewed 15 consecutive patients with Ogawa type I CP fractures combined with AC joint dislocation who underwent clavicular hook plate and coracoid screw fixation betw

A novel endovascular treatment for long-standing high-flow arteriovenous fistula.

We describe a novel use of an Amplatzer III plug device (St. Jude Medical, St. Paul, Minn) in the treatment of a traumatic arteriovenous fistula between the innominate vein and the origin of the left common carotid artery causing symptomatic right heart failure. A review of the literature indicates this is the first case to describe the use of the Amplatzer III device in this way. We demonstrate immediate closure of fistula radiologically and complete symptomatic resolution at short-term follow-up.

Shoulder dystocia.

The frequency of shoulder dystocia in different reports has varied, ranging 0.2-3% of all vaginal deliveries. Once a shoulder dystocia occurs, even if all actions are appropriately taken, there is an increased frequency of complications, including third- or fourth-degree perineal lacerations, postpartum hemorrhage, and neonatal brachial plexus palsies. Health care providers have a poor ability to predict shoulder dystocia for most patients and there remains no commonly accepted model to accurately predict this obstetric emergency. Consequently, optimal management of shoulder dystocia requires

Nodal ratios in node-positive breast cancer--long-term study to clarify discrepancy of role of supraclavicular and axillary regional radiotherapy.

PURPOSE: To study the absolute number of involved nodes/the number of nodes examined or the nodal ratio (NR) in breast cancer. The primary study endpoint was to evaluate the role of supraclavicular and axillary radiotherapy (SART) according to the NR. METHODS AND MATERIALS: From the Saskatchewan provincial registry of 1981-1995, the charts of 5,996 consecutive patients were retrieved to collect detailed prognostic factors. Among these patients, 1,985 were node positive. Because the NRs are more reliable the greater the number of nodes examined, we analyzed 1,255 patients with > or =10 nodes e

Whole body MR imaging in ankylosing spondylitis: a descriptive pilot study in patients with suspected early and active confirmed ankylosing spondylitis.

BACKGROUND: Ankylosing spondylitis is a chronic inflammatory rheumatic disorder which usually begins in early adulthood. The diagnosis is often delayed by many years. MR imaging has become the preferred imaging method for detection of early inflammation of the axial skeleton in ankylosing spondylitis. The goal of this study was to assess the frequency and distribution of abnormalities on whole body MR imaging in patients with suspected early ankylosing spondylitis and with active confirmed ankylosing spondylitis. METHODS: Ten patients with suspected early ankylosing spondylitis and ten patien

Supraclavicular and axillary lymphadenopathy as the initial manifestation in Wegener's granulomatosis.

Wegener's granulomatosis (WG) is a systemic granulomatous vasculitis that typically affects the upper airways, lungs and kidneys. Lymphadenopathy is rare in patients with WG. Here, we present the first case of WG whose initial manifestation was superficial lymphadenopathy (i.e. supraclavicular and axillary lymphadenopathy). One year after the initial presentation, a mass appeared in the lung. Biopsy specimens obtained from supraclavicular lymph nodes and lung demonstrated granulomatous vasculitis. This patient was negative for classic antineutrophil cytoplasmic antibodies (c-ANCA). Treatment w

Distal clavicular excision: a detailed functional assessment.

Distal clavicular excision is commonly performed for a number of clinical conditions. The results of the procedure are not well described. Using the Constant, American Shoulder and Elbow Surgeons (ASES), University of California at Los Angeles (UCLA) and Neer scoring systems, the medium term results of distal clavicular excision were assessed. Over an 8 year period, 39 patients underwent distal clavicular excision for conditions other than acromioclavicular instability (subluxation or dislocation). A single technique was employed. A retrospective study was performed of all 18 patients who coul

Chronic Nonbacterial Osteomyelitis of the Sternocostoclavicular Region in Adults: A Single-Center Dutch Cohort Study.

Sternocostoclavicular hyperostosis (SCCH) is a rare autoinflammatory bone disorder caused by chronic nonbacterial osteomyelitis (CNO), which is associated with sclerosis and hyperostosis primarily affecting the sternum, the medial end of the clavicles, and the first ribs. Other areas of the axial skeleton may also be affected. The more severe synovitis-acne-pustulosis-hyperostosis-osteitis (SAPHO) syndrome is additionally associated with dermatoses and joint manifestations. This Dutch retrospective cross-sectional single-center cohort study characterizes the spectrum of clinical features in ad…

[Ultrasound-guided intermediate cervical plexus block. Anatomical study].

BACKGROUND AND OBJECTIVES: The innervation of the human cervical region is complex and subject to relevant anatomical variability involving … iate cervical plexus blocks (selective block of the supraclavicular nerves, e.g. for surgery of the clavicle) must be investigated in clinical trials. The permanent anastomosis (superficial cervical ansa) between the cervical plexus and the ramus colli of the facial nerve provides an anatomically reasonable explanation for inadequate cervical plexus blocks …
