Treatment of otorhinolaryngological manifestations of three rare genetic syndromes: Branchio-Oculo-Facial (BOF), Ectrodactyly Ectodermal dysplasia Clefting (EEC) and focal dermal hypoplasia (Goltz syndrome).

Genetic background and characteristic symptoms of three children with rare genetic syndromes: Ectrodactyly Ectodermal dysplasia Clefting (EEC), Branchio-Oculo-Facial (BOF) and focal dermal hypoplasia (Goltz syndrome) were outlined. All patients presented common otorhinolaryngological features of bilateral hearing impairment and dermal problems. Diagnostic protocol and treatment strategies for all three syndromes were presented and discussed. Skin lesions of the head and neck and degree of hearing loss were identified in clinical examination and by audiological methods. Treatment of hypoacousis

The Multifaceted Human Round Window Anatomical Aspects and Clinical Relevance.

BACKGROUND: There is an increasing clinical interest in the human round window region through the arise of novel medical treatments such as cochlear implantation (CI), middle ear pharmacotherapy, and gene therapy strategies. Here, we analyzed anatomic variations of the round window niche (RWN) and membrane (RWM) using synchrotron phase-contrast imaging (SR-PCI) of human cadaveric specimens. Results were combined with light (LM) and electron microscopy (TEM) investigations of optimally preserved human tissue. MATERIALS AND METHODS: SR-PCI and 3D reconstructions of the round window region were …

Electromagnetic stimulation of the auditory system: effects and side-effects.

Extracranial electromagnetic stimulation (EMS) is a recently developed clinical technique which may be used in place of conventional transcutaneous electrical stimulation to activate the central and peripheral nervous systems. This technique is widely used in neurology and otolaryngology for non-invasive stimulation of the brain and facial nerve. EMS uses electromagnetic field pulses which pass unimpeded through the cranium and soft tissues to activate excitable membranes of volume conductors. In this series of studies, the effects and side-effects of electromagnetic stimulation on the auditor

Role of tumor necrosis factor-alpha in sensorineural hearing loss after bacterial meningitis.

HYPOTHESIS: Blockade of tumor necrosis factor-alpha with tumor necrosis factor-alpha antibody will reduce the extent of cochlear injury and hearing loss associated with Streptococcus pneumoniae meningitis. BACKGROUND: Inflammatory mediators play a significant role in the morbidity associated with bacterial meningitis, including hearing loss and labyrinthitis ossificans. Previous studies have shown the attenuation of hearing loss by the nonspecific blockade of such pathways. METHODS: Fifty Mongolian gerbils were divided into four groups. Auditory brainstem response testing was conducted to me

Stronger efferent suppression of cochlear neural potentials by contralateral acoustic stimulation in awake than in anesthetized chinchilla.

There are two types of sensory cells in the mammalian cochlea, inner hair cells, which make synaptic contact with auditory-nerve afferent fibers, and outer hair cells that are innervated by crossed and uncrossed medial olivocochlear (MOC) efferent fibers. Contralateral acoustic stimulation activates the uncrossed efferent MOC fibers reducing cochlear neural responses, thus modifying the input to the central auditory system. The chinchilla, among all studied mammals, displays the lowest percentage of uncrossed MOC fibers raising questions about the strength and frequency distribution of the con

Perceptual consequences of disrupted auditory nerve activity.

Perceptual consequences of disrupted auditory nerve activity were systematically studied in 21 subjects who had been clinically diagnosed with auditory neuropathy (AN), a recently defined disorder characterized by normal outer hair cell function but disrupted auditory nerve function. Neurological and electrophysical evidence suggests that disrupted auditory nerve activity is due to desynchronized or reduced neural activity or both. Psychophysical measures showed that the disrupted neural activity has minimal effects on intensity-related perception, such as loudness discrimination, pitch discri

[Diseases of the middle ear in childhood].

Middle ear diseases in childhood play an important role in daily ENT practice due to their high incidence. Some of these like acute otitis media or otitis media with effusion have been studied extensively within the last decades. In this article, we present a selection of important childhood middle ear diseases and discuss the actual literature concerning their treatment, management of complications and outcome. Another main topic of this paper deals with the possibilities of surgical hearing rehabilitation in childhood. The bone-anchored hearing aid BAHA® and the active partially implantable

A guide to the positioning of brainstem implants using intraoperative electrical auditory brainstem responses.

The number of electrodes that elicit usable auditory sensations with an auditory brainstem implant varies significantly between subjects. For those with only very few, movement of the array by only a few millimetres could make a significant improvement to their outcome, but yet the point at which this is normally discovered is during activation, weeks after the surgery. The number of the electrodes that are able to stimulate the auditory system can be more reliably assured by the use of electrophysiologic guidance in the placement of the implanted electrode array. This chapter describes a proc

[Routine diagnostics prior to ear surgery-results of a survey among otorhinolaryngology departments in German-speaking countries. German version].

BACKGROUND: Diagnostic testing prior to ear surgery is an important part of the work of ear specialists in hospitals. To date, there is no national or international diagnostic standard. Rather, the test batteries used are dependent on hospitals' own doctrinal opinions. OBJECTIVE: The aim of this study was to conduct a survey in German-speaking countries to determine the routine diagnostic methods used prior to various ear surgery procedures. METHODS: An online questionnaire with 18 general questions on the hospital's equipment and availability of tests and 6 questions on specific ear operati

Sulfobetaine-based ultrathin coatings as effective antifouling layers for implantable neuroprosthetic devices.

Foreign body response (FBR), inflammation, and fibrotic encapsulation of neural implants remain major problems affecting the impedance of the electrode-tissue interface and altering the device performance. Adhesion of proteins and cells (e.g., pro-inflammatory macrophages, and fibroblasts) triggers the FBR cascade and can be diminished by applying antifouling coatings onto the implanted devices. In this paper, we report the deposition and characterization of a thin (±6 nm) sulfobetaine-based coating onto microfabricated platinum electrodes and cochlear implant (CI) electrode arrays. We found t

[Inpatient rehabilitation of adult CI users: Results in dependency of duration of deafness, CI experience and age].

BACKGROUND: Cochlear implants (CI) have proven to be a highly effective treatment for severe hearing loss or deafness. Inpatient rehabilitation therapy is frequently discussed as a means to increase the speech perception abilities achieved by CI. However, thus far there exists no quantitative evaluation of the effect of these therapies. METHODS: A retrospective analysis of audiometric data obtained from 1355 CI users compared standardized and qualitative speech intelligibility tests conducted at two time points (admission to and discharge from inpatient hearing therapy, duration 3-5 weeks). [Fitness to dive in the otorhinolaryngological field].

In line with the rising number of recreational divers, the otorhinolaryngologist has to deal with growing numbers of diving-associated disorders of the ear, nose and throat (ENT). Nevertheless, the majority of divers present to their ENT doctor for assessment of their fitness to dive. On the basis of long-term follow-up examinations and increasing experience in diving medicine, even divers with a history of ENT problems can be considered fit to dive. Therefore, diving is possible after tympanoplasty, surgery to improve hearing including stapesplasty, after implantation of middle ear amplifiers

[Electric stimulation of auditory nerve fibers. Experimental study].

The authors tested in animal experiments the effectiveness of electrical stimulation of acoustic nerve fibres. To guinea pigs platinum-iridium electrodes were implanted in the round window of the cochlea. The receptors of the inner ear were functionally eliminated by the local application of the ototoxic antibiotic gentamicin. Cortical responses produced by electric stimulation of the contralateral ear and acoustic stimulation of the ipsilateral intact ear were recorded. The threshold for producing a cortical response by the electric stimulus was lowest at the frequency of 0.5 kHz and rose wit

Some practical considerations in development of multichannel scala tympani prostheses.

There are several basic issues regarding the safety and feasibility of implanting multichannel scala tympani cochlear prostheses in humans. Physiological and technical studies have been designed to resolve some of these questions. Results of physiological investigations demonstrate that (1) nerve viability is not affected by the presence (without activation) of a multichannel array; (2) use of a highly specified bipolar electrode configuration permits discrete and predictable stimulation of sectors of the auditory nerve array, and (3) some deleterious effects (i.e., nerve damage and calcificat

Implantation of the semicircular canals with preservation of hearing and rotational sensitivity: a vestibular neurostimulator suitable for clinical research.

HYPOTHESIS: It is possible to implant a stimulating electrode array in the semicircular canals without damaging rotational sensitivity or hearing. The electrodes will evoke robust and precisely controlled eye movements. BACKGROUND: A number of groups are attempting to develop a neural prosthesis to ameliorate abnormal vestibular function. Animal studies demonstrate that electrodes near the canal ampullae can produce electrically evoked eye movements. The target condition of these studies is typically bilateral vestibular hypofunction. Such a device could potentially be more widely useful clin

Chronic electrical stimulation of the auditory nerve using high surface area (HiQ) platinum electrodes.

High surface area cochlear implant electrodes with much smaller geometric surface areas than current designs might be used in the future to increase the number of stimulating electrodes along the carrier. Potential problems with an increase in charge density for a common stimulus resulting from decreasing the geometric surface area would be reduced by the enlarged real surface area of such electrodes. Electrochemically modified (HiQ) platinum (Pt) electrodes, with a real surface area approximately 75 times greater than the current standard Pt electrodes of the same geometric size, had shown in
