Surgical management of metastatic bone disease.

Metastatic bone disease is a major contributor to the deterioration of the quality of life of patients with cancer; it causes pain, impending and actual pathological fractures, and loss of function and may also be associated with considerable metabolic alterations. Operative treatment may be required for an impending or existing fracture and intractable pain. The goals of surgery are to provide local tumor control and allow immediate weight-bearing and function. Radiation therapy is often indicated postoperatively. Detailed preoperative evaluation is required to assess the local extent of bone

Time-to-Surgery for Definitive Fixation of Hip Fractures: A Look at Outcomes Based Upon Delay.

The morbidity and mortality after hip fracture in the elderly are influenced by non-modifiable comorbidities. Time-to-surgery is a modifiable factor that may play a role in postoperative morbidity. This study investigates the outcomes and complications in the elderly hip fracture surgery as a function of time-to-surgery. Using the American College of Surgeons-National Surgical Quality Improvement Program data from 2011 to 2012, a study population was generated using the Current Procedural Terminology codes for percutaneous or open treatment of femoral neck fractures (27235, 27236) and fixation

Intertrochanteric fractures treated by diaphyseal support arthroplasty with hook plate vs cerclage wires only: A retrospective cohort study.

BACKGROUND: The treatment of unstable fractures of the intertrochanteric region of the hip in the elderly is controversial. Conventionally, internal fixation with intramedullary nail or a dynamic hip screw is the treatment of choice in intertrochanteric fractures. Nowadays, some authors encouraged the use hip arthroplasty for management of these fractures with good outcome. The aim of this study is to compare total hip arthroplasty with hook plate against total hip arthroplasty with cerclage cables in the management of unstable intertrochanteric fractures. MATERIALS AND METHODS: Out of 100 ad

Less invasive percutaneous wave plating of simple femur shaft fractures: A prospective series.

In developing nations, fixation of femoral shaft fractures with intramedullary (IM) nails can pose significant challenges. Use of IM implants is commonly limited by availability, funds or patient's physique. Conversely, traditional compression plates are usually readily available at a much lower cost, making bridge plating of femur fractures a frequently used surgical technique. We hypothesised that less invasive percutaneous plate osteosynthesis (MIPPO) of femoral shaft fractures has a similar outcome compared to IM nailing. The study is designed as a prospective case series at a Level 1 univ

Outcome of the intramedullary diaphyseal segmental defect fixation system for bone tumors.

BACKGROUND: Resection of diaphyseal malignant bone tumors is indicated for local control and impending pathological fracture or failure of prophylactic internal fixation. However, there are no large, long-term studies analyzing the results of intercalary reconstruction using segmental metallic spacers. MATERIALS AND METHODS: We present 24 patients treated with wide resection for primary or metastatic bone tumors involving the diaphysis of the femur, tibia, or humerus and reconstruction using a modular intramedullary diaphyseal segmental defect fixation system. The mean length of bone resectio

Outcomes of hip arthroplasty with concomitant hardware removal: influence of the type of implant retrieved and impact of positive intraoperative cultures.

INTRODUCTION: The impact of residual internal fixation devices on subsequent procedures about the hip has not been clearly well defined. The objective of the current study is to evaluate the outcome of hip arthroplasty after hardware retrieval as a one-stage replacement, to analyze possible differences related to the type of removed implant, and to assess the impact of unexpected intraoperative cultures during implant retrieval. MATERIALS AND METHODS: We present a retrospective study including all those cases undergoing hip arthroplasty with concomitant hardware removal (cannulated screws, in

Ilizarov fixator combined with an intramedullary nail for tibial nonunions with bone loss: is it effective?

BACKGROUND: Treatment of tibial nonunion with bone loss is extremely difficult. A variety of techniques have been described, but each has shortcomings, in particular prolonged external fixation time as well as serious complications such as nonunion and infection. Accordingly, we developed a technique that seeks to reduce these complications by using a circular external fixator in addition to an intramedullary nail to achieve union, limb lengthening, and stability of the regenerated segment. DESCRIPTION OF TECHNIQUE: First, the pseudoarthrosis area is resected, and acute compression is continu

Multidirectional thoracic wall stabilization: a new device on the scene.

PURPOSE: Stabilization and replacement of ribs is still a challenge, because most available systems for intramedullary and extramedullary fixation are less than perfect. We present our experience with a modified device, which compensates for several disadvantages in other methods. DESCRIPTION: Originating from the Strasbourg Thoracic Osteosyntheses System (STRATOS [MedXpert GmbH, Heitersheim, Germany]), the multidirectional thoracic wall stabilization system uses tripodal clips with sharp clasping legs. They can be placed without dissecting the ribs, and bridge fractures or defects with titan

Association between time to weight bearing and rates of nonunion, delayed union, and malunion after intramedullary nailing of tibial shaft fractures.

INTRODUCTION: Intermedullary nailing (IMN) is the gold standard for tibial shaft fractures in adults with the goal of allowing early weight bearing. The purpose of this study was to determine the association between time to weight bearing as tolerated (WBAT) after tibial shaft IMN fixation and fracture nonunion, delayed union, and tibial deformity at final follow-up. METHODS: A retrospective cohort of patients who sustained extra-articular tibial shaft fractures treated with IMN at two level 1 trauma centers were collected from 2010 to 2021. A minimum follow-up of six months was required unle
