Intensity-Specific Physical Activity Measured by Accelerometer, Genetic Susceptibility, and the Risk of Kidney Stone Disease: Results From the UK Biobank.

RATIONALE & OBJECTIVE: Kidney stone disease (KSD), a significant health care problem within both developed and developing countries, has been associated with genetic risk factors. An association between physical activity and KSD risk also has been hypothesized, but studies have yielded inconsistent findings. This study investigated the association between the intensity of physical activity and the incidence of KSD accounting for genetic risk. STUDY DESIGN: Prospective cohort study. SETTING & PARTICIPANTS: A total of 80,473 participants from the UK Biobank Study. EXPOSURE: Physical activity

Patients who underwent TIPS face increases in hernia incarceration rates compared to patients managed with paracentesis: An analysis of prevalence and risk factors.

TIPS is commonly used to treat complications of portal hypertension. Hernia incarceration is a complication of the TIPS procedure that is poorly understood and not well documented. We examine this complication in a large, single-center cohort of TIPS patients and compared it to a cohort of patients with refractory ascites managed with serial large-volume paracentesis alone to determine whether TIPS itself increases incarceration risk. We identified 259 adults who underwent TIPS and 644 adults with refractory ascites managed with serial large-volume paracentesis at the University of California,

Development and Validation of a Model to Identify Critical Brain Injuries Using Natural Language Processing of Text Computed Tomography Reports.

IMPORTANCE: Clinical text reports from head computed tomography (CT) represent rich, incompletely utilized information regarding acute brain injuries and neurologic outcomes. CT reports are unstructured; thus, extracting information at scale requires automated natural language processing (NLP). However, designing new NLP algorithms for each individual injury category is an unwieldy proposition. An NLP tool that summarizes all injuries in head CT reports would facilitate exploration of large data sets for clinical significance of neuroradiological findings. OBJECTIVE: To automatically extract

The impact of national cultural framework on college students' willingness to communicate in oral English tasks in flipped classroom mode: a quasi experimental study.

Against the backdrop of "telling Chinese stories well in English," a core value orientation of college English education in the new era, college oral English teaching undertakes the dual mission of fostering language proficiency and shaping cultural confidence. This study takes the flipped classroom model as the implementation carrier, constructs a national cultural framework with three dimensions of cultural cognition, cultural expression and cultural identity, and adopts a non-equivalent control group pretest-posttest quasi-experimental design to explore its effects and mechanisms on college

Enhancing tumor T cell antigen prediction by integrating deep protein representations.

BACKGROUND: Cancer is a major threat to human health, and cancer immunotherapy is considered the most promising treatment option. It offers high effectiveness and precision, with fewer side effects than traditional treatments. Tumor T cell antigens (TTCA) are the proteins or protein fragments that stand on the surface of cancer cells and that are recognized by the immune system. However, physical experimental approaches to infer them might be costly and time-consuming, even though they are important for cancer immunotherapy. RESULTS: We propose DEEPTHYBRID to predict TTCAs by combining chemic

Automatic phenotyping of emergency department patients with incidental hepatic steatosis: A machine learning clustering analysis.

BACKGROUND: Incidental findings in the Emergency Department (ED) are often not acted upon due to acuity of care and lack of referral pathways. Artificial intelligence (AI) can be used to combine heterogeneous data collected during ED encounters to computationally identify patients with specific disease phenotypes allowing for directed care-paths. We identified ED patients with undiagnosed Metabolic Dysfunction-Associated Liver Disease (MASLD) to demonstrate the feasibility of automated algorithms for clinical phenotypes. METHODS: We identified adults in 5 EDs with abdominal imaging between 1/

Surgeon-Performed Point-of-Care Ultrasound in Complex Breast Lesions: Implications for Oncologic Diagnosis and Surgical Planning.

Surgeon-performed point-of-care breast ultrasound (POCUS) has become a practical adjunct in the evaluation and management of breast lesions, allowing real-time imaging to be incorporated directly into clinical and operative decision-making. In the oncologic setting, complex breast lesions often require timely characterization, tissue diagnosis, localization, and surgical planning, and the separation of imaging and surgical workflows may contribute to delay, fragmented care, and reduced procedural efficiency. This narrative review examines the technical principles, diagnostic applications, surg

Limbic-Visual Disintegration and Salience-Control Specialization Characterize Tinnitus Network Topology.

Subjective tinnitus (ST) has been hypothesized to arise from large-scale network reorganization, but the affected circuits and their symptom scaling remain unclear. In a normal-hearing cohort (N = 114; 57 ST, 57 matched controls), we combined resting-state fMRI graph topology, ROI-to-ROI connectivity, voxel-based morphometry (VBM), and multivariate modelling using a harmonized 50-ROI parcellation from the CONN atlas, spanning 15 functional networks. Node-wise analyses (covarying age, sex, education, and motion; multiple-comparison control) showed selective, not global, reconfiguration: reduced

A bias-accounting meta-analytic approach refines and expands the cerebellar behavioral topography.

The cerebellum plays important roles in motor, cognitive, and emotional behaviors. Previous cerebellar coordinate-based meta-analyses (CBMAs) have complemented precision-mapping and parcellation approaches by finding generalizable cerebellar activations across the largest possible set of behaviors. However, cerebellar CBMAs face challenges due to inherent methodological limitations, exacerbated by historical cerebellar neglect in neuroimaging studies. Here, we show overrepresentation of superior activations, rendering the null hypothesis of standard activation likelihood estimation (ALE) unsui

Artificial intelligence versus human expertise: ECG-based detection of occlusive myocardial infarction after cardiac arrest.

BACKGROUND/AIM: Accurate electrocardiogram (ECG) interpretation after cardiac arrest is essential for identifying occlusive myocardial infarction (OMI), but post-resuscitation artifacts make this challenging. While artificial intelligence (AI) offers promising support, its diagnostic performance in this critical setting remains uncertain. METHODS: This single-centre study included 97 adult patients resuscitated from cardiac arrest (CA). Post-return of spontaneous circulation (ROSC) ECG were evaluated by four methods: human experts (HE), a validated deep neural network [Queen of Hearts (QoH)],

Exploring Pain on Social Media: Observational Study on Perceptions and Discussions of Chronic Pain Conditions.

BACKGROUND: Chronic pain, affecting 30.3% of the global population, constitutes a major public health and social challenge. It is associated with disability, emotional distress, and diminished quality of life. Conditions, such as fibromyalgia, headache, paraplegia, neuropathy, and multiple sclerosis are characterized by persistent pain and limited social and medical understanding. This contributes to patient isolation and increases mental health burden. In recent years, social media, particularly X (formerly Twitter), has emerged as a key space for analyzing health-related perceptions and expe

The impact of COVID-19 vaccine spring boosters on COVID-19 hospital admissions in England 2022/23.

BACKGROUND: In the spring of 2022 and 2023 COVID-19 vaccine boosters were recommended for those aged ≥75 years in England as well as those in an immunosuppression risk group. The aim was to reduce severe COVID-19 disease in these groups. METHODS: The large difference in coverage between those above and below age 75 years was the basis for applying an age-discontinuity approach for measuring the impact of vaccination on COVID-19 hospitalisations in both spring 2022 and 2023. Hospitalisations in individuals positive by PCR for COVID-19 were obtained from the national secondary user service hosp

Assessing processing speed and its neural correlates in the three variants of primary progressive aphasia with a non-verbal tablet-based task.

Prior research has revealed distinctive patterns of impaired language abilities across the three variants of Primary Progressive Aphasia (PPA): nonfluent/agrammatic (nfvPPA), logopenic (lvPPA) and semantic (svPPA). However, little is known about whether, and to what extent, non-verbal cognitive abilities, such as processing speed, are impacted in PPA patients. This is because neuropsychological tests typically contain linguistic stimuli and require spoken output, being therefore sensitive to verbal deficits in aphasic patients. The aim of this study is to investigate potential differences in p
