Intramedullary versus crossed pinning for paediatric distal radius fractures: clinical decision patterns and biomechanical validation integrating real-world practice with growth plate stress finite element analysis.

OBJECTIVE: This observational study characterized clinical outcomes and decision patterns for intramedullary (IM) versus crossed pinning (CP) in paediatric distal radius fractures, integrating real-world practice with growth plate-inclusive finite element analysis (FEA) to elucidate biomechanical profiles, particularly physeal safety. METHODS: Retrospective analysis of 102 children (43 IM, 59 CP) undergoing closed reduction and percutaneous fixation (2018-2023). Baseline differences included younger age (IM 8.3 ± 2.7 vs. CP 12.3 ± 3.3 years, p < 0.001) and more proximal fractures (IM 25.0 ± 6

Repairing a critical cranial defect using WISP1-pretreated chondrocyte scaffolds.

In cranial flat bone fractures, spontaneous bone repair will occur only when the fracture ends are in close contact. However, in cases wherein bone discontinuity is extensive, surgical interventions are often required. To this end, autologous bone is harvested and surgically integrated into the site of fracture. Here we propose to use cartilage, as an alternative autologous source, to promote cranial fracture repair. The advantage of this approach is the potential reduction in donor site morbidity, likely due to the avascular and aneural nature of cartilage. As a first step we attempted to ind

Treatment modalities and outcomes following acetabular fractures in the elderly: a systematic review.

PURPOSE: The treatment of geriatric acetabular fractures remains controversial. Treatment options include nonoperative management, open reduction and internal fixation (ORIF), total hip arthroplasty (THA) with or without internal fixation, and closed reduction with percutaneous pinning (CRPP). There is currently no consensus on the optimal treatment strategy for geriatric patients with acetabular fractures. The purpose of this study is to compare adverse event rates, functional and radiographic outcomes, and intraoperative results between the various treatment modalities in order to help guide

Flexor Tendon Rupture After a Cat Bite - A Case Report of An Uncommon Sequela.

INTRODUCTION: Distal radius fractures account for nearly one-third of all pediatric fractures, making it among the most common of all pediatric fractures. Studies report that distal radius physeal arrest rates following trauma to the distal radius seem to occur in only about 5% of patients. Significant ulnar variance can develop leading to notable pain and limitations in function. CASE REPORT: We present a case of a 17-year-old right-hand dominant adolescent Caucasian male with persistent ulnar wrist pain. The patient presented to our outpatient clinic for evaluation of his left wrist followi

Hold the Antibiotics: Are Preoperative Antibiotics Unnecessary in the Treatment of Pediatric Supracondylar Humerus Fractures?

BACKGROUND: Supracondylar humerus (SCH) fracture is the most common elbow injury in children and often treated with closed reduction and percutaneous pinning (CRPP). There is little published evidence supporting or refuting the use of perioperative prophylactic antibiotics for SCH CRPP in the pediatric population. The purpose of this study is to evaluate the rate of surgical site infection for patients with and without preoperative antibiotics. METHODS: A retrospective chart review was conducted of patients less than or equal to 16 years from 2012 to 2018 who underwent primary CRPP. Open frac

Safety and Outcomes of Inpatient Compared with Outpatient Surgical Procedures for Ankle Fractures.

BACKGROUND: As the cost of health-care delivery rises in the era of bundled payments for care, there is an impetus toward minimizing hospitalization. Evidence to support the safety of open reduction and internal fixation (ORIF) of ankle fractures in the outpatient setting is largely anecdotal. METHODS: Patients who underwent ORIF from 2005 to 2013 were identified via postoperative diagnoses of ankle fracture and Current Procedural Terminology codes; patients with open fractures and patients who were emergency cases were excluded. Patients undergoing inpatient and outpatient surgical procedure

Wound Infections Following Implant removal below the knee: the effect of antibiotic prophylaxis; the WIFI-trial, a multi-centre randomized controlled trial.

BACKGROUND: In the Netherlands about 18,000 procedures with implant removal are performed annually following open or closed reduction and fixation of fractures, of which 30-80% concern the foot, ankle and lower leg region. For clean surgical procedures, the rate of postoperative wound infections (POWI) should be less than ~2%. However, rates of 10-12% following implant removal have been reported, specifically after foot, ankle and lower leg fractures. Currently, surgeons individually decide if antibiotics prophylaxis is given, since no guideline exists. This leads to undesirable practice varia

A prospective randomized controlled trial of a bioresorbable calcium phosphate paste (alpha-BSM) in treatment of displaced intra-articular calcaneal fractures.

BACKGROUND: Displaced intra-articular calcaneal fractures are devastating injuries and pose a therapeutic challenge. The purpose of this study was to determine whether open reduction internal fixation (ORIF) plus an injectable bioresorbable calcium phosphate paste (alpha-BSM [bone substitute material]) is superior to ORIF alone in the treatment of calcaneal bone voids encountered after operative treatment of displaced intra-articular calcaneal fractures. METHODS: We prospectively randomized 47 patients with 52 closed displaced intra-articular fractures necessitating operative fixation to rece

Interfragmentary gap is a risk factor for nonunion after intramedullary nailing for femoral shaft fractures. About 190 cases.

INTRODUCTION: Intramedullary nailing (IMN) is the standard treatment for femoral shaft fractures (FSF). Complications include nonunion and rotational malunions, the contributing factors of which remain less clearly defined. The aim of this study was to find the relevant risk factors for complications after IMN for FSF. HYPOTHESIS: Complications after IMN for FSF are related to several risk factors. MATERIALS AND METHODS: All patients who underwent surgery for open or closed FSF, performed as a first-line treatment or after temporary external fixation, between 2014 and 2020 were included. The

Functional and microstructural biomarkers of repetitive mild head injury in a conscious momentum exchange mouse model.

BACKGROUND: Repetitive mild traumatic brain injury (rmTBI) represents a significant public health concern, yet preclinical models have been limited by methodological confounds including anesthesia, restricted head motion, and skull fracture. NEW METHOD: This study adapted the momentum exchange model, previously validated only in rats, for use in awake mice during their active circadian phase, creating the first closed-head, acceleration-driven mTBI model in conscious mice. Adult C57BL/6 mice (N = 18, 50% female) received either repetitive head impacts (0.34 J energy input at 3.7 m/s) or sham

Total Elbow Arthroplasty as the Treatment of Choice for a Young Man with Neglected Terrible Triad of the Elbow Joint and Schizophrenia: A Case Report.

In young patients, the use of total elbow arthroplasty (TEA) is rarely preferred due to its high rate of mechanical failure. Poor compliance and psychological problems encountered may lead to increased difficulty in management. A 38-year-old male complained stiffness and pain on his left elbow. History of trauma was present 10 months ago, when he fell down from a tree of 6m high. Immediate closed reduction and immobilisation with backslab was performed, but he was lost to follow-up due to Schizophrenia. In physical examination, we found varus and recurvatum deformity with inability to flex the

Medial approach to the peroneal vessels as recipients for free flap reconstruction of the leg.

The anatomic variation in the branching pattern of the popliteal vessel has been well-established. Little has been written in the literature regarding recipient vessel selection for microvascular reconstruction in the lower extremity as it pertains to aberrant vascular anatomy. We present the case of a 57-year-old male patient who sustained a closed right lower extremity pilon fracture in a motor vehicle accident. The patient was initially treated with external fixation, which was followed by definitive open reduction and internal fixation 3 weeks later. Over the next 2 weeks, the patient deve

Bilateral Traumatic Anterior Dislocation of Shoulder - A Rare Entity.

INTRODUCTION: Bilateral shoulder dislocation are most commonly posterior type. These are most commonly due to seizure disorder and electrocution. Anterior shoulder dislocations occurring bilaterally without any predisposing factors are very rare. These types of injuries are due to trauma with a unique mechanism of injury. To best of our knowledge there are only few cases of similar kind are reported in literature. We hereby report a interesting case of posttraumatic, bilateral anterior dislocation of shoulder without associated fracture in a 45 old women without any predisposing pathoanatomy.
