Determinants of Functional Outcome after Pediatric Hemispherotomy.

OBJECTIVE: We aimed to evaluate determinants of functional outcome after pediatric hemispherotomy in a large and recent multicenter cohort. METHODS: We retrospectively investigated the functional outcomes of 455 children who underwent hemispherotomy at 5 epilepsy centers in 2000-2016. We identified determinants of unaided walking, voluntary grasping with the hemiplegic hand, and speaking through Bayesian multivariable regression modeling using missing data imputation. RESULTS: Seventy-five percent of children were seizure-free, and 44% stopped antiseizure medication at a 5.1-year mean follow

Incidence of Gastrointestinal Bleeding after Transesophageal Echocardiography in Patients with Gastroesophageal Varices: A Systematic Review and Meta-Analysis.

BACKGROUND: Transesophageal echocardiography (TEE) is useful for cardiac assessment and intraoperative monitoring. However, the safety of TEE in patients with cirrhosis and gastroesophageal varices has remained uncertain. The aim of this meta-analysis was to determine the incidence of gastrointestinal bleeding after TEE in patients with varices. The secondary objectives were to compare bleeding risks between patients with and without varices and to determine the incidence of TEE-related esophageal perforation and mortality. METHODS: A systematic literature search was conducted on MEDLINE, Emb

Physical Activity as a Predictor of Cognitive Decline in an Elderly Essential Tremor Cohort: A Prospective, Longitudinal Study.

Background: Essential tremor (ET), one of the most common neurological diseases, is associated with cognitive impairment. Surprisingly, predictors of cognitive decline in ET remain largely unidentified, as longitudinal studies are rare. In the general population, however, lower physical activity has been linked to cognitive decline. Objectives: To determine whether baseline physical activity level is a predictor of cognitive decline in ET. Methods: One hundred and twenty-seven ET cases (78.1 ± 9.5 years, range = 55-95), enrolled in a prospective, longitudinal study of cognition. At baseline, e

Sex-Dependent Cortical Volume Changes in Patients with Degenerative Cervical Myelopathy.

Degenerative cervical myelopathy (DCM) is a progressive condition characterized by degeneration of osseocartilaginous structures within the cervical spine resulting in compression of the spinal cord and presentation of clinical symptoms. Compared to healthy controls (HCs), studies have shown DCM patients experience structural and functional reorganization in the brain; however, sex-dependent cortical differences in DCM patients remains largely unexplored. In the present study, we investigate the role of sex differences on the structure of the cerebral cortex in DCM and determine how structural

Comparison of manual compression and vascular hemostasis devices after coronary angiography or percutaneous coronary intervention through femoral artery access: A meta-analysis of randomized controlled trials.

OBJECTIVES: To compare the efficacy and safety of manual compression (MC) with vascular hemostasis devices (VHD) in patients undergoing coronary angiography (CA) or percutaneous coronary intervention (PCI) through femoral artery access. INTRODUCTION: The use of femoral artery access for coronary procedures may result in access-related complications, prolonged immobility and discomfort for the patients. MC results in longer time-to-hemostasis (TTH) and time-to-ambulation (TTA) compared to VHDs but its role in access-related complications remains unclear in patients undergoing coronary procedur

Racial Inequities and Access to COVID-19 Treatment.

IMPORTANCE: Timely outpatient treatment for COVID-19 reduces the risk of hospitalization and death in high-risk patients. However, large racial and ethnic inequities in treatment have been observed across the US. The relative contributions of care mechanisms that create barriers (eg, access to home tests or telemedicine) are unknown. OBJECTIVE: To examine the relative contribution of structural and encounter-level factors associated with racial and ethnic disparities in oral antiviral prescribing for COVID-19. DESIGN, SETTING, AND PARTICIPANTS: In this cross-sectional study using electronic

Multiregional characterisation of collaboration between clinical and community-based organisations for postpartum transition care delivery: A mixed methods study protocol.

INTRODUCTION: The USA exhibits the highest rate of maternal mortality among developed high-income countries. These deaths most commonly occur during the postpartum year and are largely preventable. Individuals that live in medically underserved areas and lack access to transitional care following childbirth are at higher risk for maternal mortality, severe maternal morbidity and adverse pregnancy outcomes. National US organisations have proposed implementation of innovative models, including coordinated postpartum service delivery across clinical and community-based organisations (CBOs), to he

A Qualitative Exploration of Clinicians' Perspectives on Obstructive Sleep Apnea Facilitators and Barriers to Care.

BACKGROUND: Literature has suggested that children experience wide variation in diagnosis and treatment of obstructive sleep-disordered breathing (SDB), however, multidisciplinary perspectives on barriers and facilitators to care are not well delineated among clinicians. METHODS: This qualitative study adhered to the Consolidated Criteria for Reporting Qualitative Research (COREQ) guidelines. Participants were recruited until thematic saturation was achieved across the overall dataset. Key informant interviews were conducted with physicians and advanced practice providers from primary care, o

Support for Transgender and Nonbinary Individuals Seeking Vaginoplasty (STRIVE) study: protocol for a national randomised pragmatic trial.

INTRODUCTION: Transgender and gender-diverse individuals often face significant barriers to accessing gender-affirming care, such as hormones and/or surgery, leading to poorer mental health, lower quality of life, and higher rates of substance use and suicidal ideation. Vaginoplasty, the most commonly sought genital gender-affirming surgery (GGAS), is desired by over half of all trans women but has been performed in only a minority. This is due largely to limited surgeon availability and long wait times. Peer support has been shown to improve health outcomes and reduce stigma in marginalised p

Pre-operative concerns and post-operative satisfaction: comparing attitudes toward bariatric surgery and medication intervention for weight loss on Reddit.

BACKGROUND: Patient perceptions influence the success of bariatric surgery and pharmacologic weight loss therapies, yet many concerns never reach providers during clinic visits. Reddit, a popular online community, offers a window to explore these concerns outside clinical settings. We leveraged natural language processing (NLP) to compare user-submitted content across communities in surgical and non-surgical weight loss communities on Reddit. METHODS: We extracted the top 500 posts and all comments from three Reddit communities: /r/GastricBypass (/r/GB), /r/GastricSleeve (/r/GS), and /r/Ozemp

Audit-First Recombination for Test-Time LLM Optimization Under Deterministic Verifiers.

Deterministic verifiers make test-time optimization of large language model (LLM) outputs practical, but modern search pipelines still waste budget on candidates that are structurally ineligible for semantic evaluation. Ineligibility includes parse failures, schema violations, and dependency breaks. These dead-on-arrival candidates consume the same scarce backend capacity as informative trials, reduce throughput under rate limits, and make cross-candidate reuse brittle under strict verifiers. We introduce audit-first recombination, a primitive for verifier-backed generation whose central opera
