Familial cavernous angiomas: natural history and genetic study over a 5-year period.

In a kindred of 122 individuals we found 5 individuals with cerebral vascular malformation, 3 representing typical cavernous angiomas. The condition was inherited as an autosomal dominant trait with variable expressivity. Forty-three relatives were examined prospectively by cranial computed tomography (CCT) and lesions were found in 15; 7 were followed prospectively with CCT scans for 5 years. Angiography in 5 of these cases failed to demonstrate the lesion. In 3 patients with previously normal CCT scans a change in blood volume or membrane permeability allowed visualization of the lesion on c

Visual Ordinal Coronary Calcium Scoring on Routine PET/CT for Predicting Perioperative Major Clinical Events After Noncardiac Surgery.

BACKGROUND. PET/CT is commonly performed during oncologic workup and provides an opportunity for coronary artery calcification (CAC) evaluation. However, technical constraints typically preclude standard Agatston scoring using the attenuation-correction CT images obtained during PET/CT examinations. OBJECTIVE. The purpose of this study was to assess the prognostic utility of a simple visual ordinal CAC scoring system applied to PET/CT for perioperative risk stratification after noncardiac surgery. METHODS. This retrospective study included 972 patients (559 men, 413 women; median age, 58 years

Clinical significance of serum calcium-phosphorus product, parathyroid hormone, vitamin K2 in chronic kidney disease patients with coronary artery calcification.

Coronary artery calcification (CAC) is a major cardiovascular complication in chronic kidney disease (CKD) driven by disrupted calcium-phosphorus metabolism, secondary hyperparathyroidism, and vitamin K2 deficiency. Although CAC predicts cardiovascular events, the roles of calcium-phosphorus product (Ca × P), parathyroid hormone (PTH), and vitamin K2 in CAC progression and diagnosis remain underexplored. This study examines clinical and biochemical factors linked to CAC in CKD and evaluates the diagnostic potential of these biomarkers individually and in combination. This retrospective analysi

Clinical implications of calcification severity adjacent to calcified nodule: Its association with first and recurrent risks of target lesion revascularization after percutaneous coronary intervention.

BACKGROUND AND AIMS: Calcified nodule (CN) is a plaque phenotype characterized by protruding calcification, associated with repeat revascularization after percutaneous coronary intervention (PCI). The severity of calcification increases the risk of future target lesion revascularization (TLR). This study was conducted to determine whether calcification severity in the adjacent zone is associated with TLR. METHODS: We analyzed 204 patients who received PCI for de-novo CN using intravascular ultrasound (IVUS). The calcium volume index (CVI) was calculated for each 1-mm cross-sectional frame in

Intravascular Lithotripsy Versus Rotational Atherectomy in the Management of Calcific Coronary Lesions: A Systematic Review and Meta-Analysis.

BACKGROUND AND AIMS: Calcific coronary lesions pose significant challenges to percutaneous coronary intervention (PCI), limiting stent delivery and expansion. Intravascular lithotripsy (IVL) and rotational atherectomy (RA) are widely used plaque modification techniques; however, comparative data on their effectiveness remain limited. We aimed to compare clinical and procedural outcomes between IVL and RA in the management of calcific coronary lesions. METHODS: PubMed, Embase, Scopus, and Cochrane Library were searched through January 2025 for randomized controlled trials (RCTs) and observatio

Diagnostic performance of a vessel-length-based method to compute the instantaneous wave-free ratio in coronary arteries.

The instantaneous wave-free ratio (iFR) is a recently introduced vasodilator-free index to assess the functional severity of coronary stenosis in the resting state, while fractional flow reserve (FFR) is the gold standard index in hyperemia. The computed instantaneous wave-free ratio (CT-iFR) is a noninvasive method to estimate iFR using computer simulations. Here, we developed a vessel-length-based CT-iFR method in patient-specific models of coronary arteries. This method was implemented by coupling a three-dimensional computational fluid dynamics model with a lumped parameter model (LPM) of
