Tricortical iliac crest wedge grafting with headless screw fixation for unstable scaphoid waist nonunion: Union rates and two-year functional outcomes.

BACKGROUND: Unstable scaphoid waist nonunion disrupts wrist biomechanics through scaphoid shortening, humpback deformity, and dorsal intercalated segment instability (DISI), predisposing to degenerative collapse. Structural bone grafting aims to restore scaphoid length and alignment in addition to achieving union. This study evaluated radiological and functional outcomes following tricortical iliac crest wedge grafting for unstable scaphoid waist nonunion. METHODS: A retrospective analysis was conducted on 40 adult patients with unstable scaphoid waist nonunion treated using a volar tricortic

Correction of lesser toe deformities: minimally invasive versus open surgery-a prospective randomised study.

OBJECTIVE: To compare soft and hard outcome measures after minimally invasive (MIS) and open (OS) surgical treatment of lesser toe deformities. It was hypothesised that MIS would be associated with fewer complications and comparable subjective and objective results. METHODS: A prospective randomised controlled study was designed. One hundred patients were included consecutively and allotted via block randomisation to two groups. The presence of co-pathologies at the forefoot and midfoot, which were treated simultaneously, was not an exclusion criterion. However, these cases were analysed to e

Outcomes of Pediatric Scaphoid Nonunions Treated With Distal Radius Cancellous Autograft.

BACKGROUND: The optimal management of pediatric scaphoid fracture nonunions is controversial. We hypothesize that pediatric patients with scaphoid fracture nonunions will have favorable functional outcomes with the utilization of nonvascularized distal radius cancellous autograft with open reduction and internal fixation (ORIF). METHODS: A review was performed from 2012 to 2017 identifying skeletally immature patients with scaphoid fracture nonunions treated with ORIF and nonvascularized distal radius cancellous autograft, including demographic data, mechanism of injury, length of time from i

Fine wire versus locking plate fixation of type C pilon fractures.

PURPOSE: The successful treatment of high energy pilon fractures (AO-OTA 43C) can be achieved with a fine wire circular external fixator (CEF) or locking plate construct (ORIF). There is no consensus on whether ORIF or CEF achieves superior outcomes, and both have unique complications. We report early to mid-term outcomes comparing type C pilon fractures treated with ORIF and CEF. METHODS: An 8-year retrospective review was performed on all patients who underwent ORIF or CEF for closed 43C fractures in a tertiary orthoplastic centre. Outcomes included unplanned return to theatre prior to unio

Biomechanical comparison of the pullout strengths of C1 lateral mass screws and C1 posterior arch screws.

BACKGROUND CONTEXT: Conditions of the atlantoaxial complex requiring internal stabilization can result from trauma, malignancy, inflammatory diseases, and congenital malformation. Several techniques have been used for stabilization and fusion. Posterior wiring is biomechanically inferior to screw fixation. C1 lateral mass screws and C1 posterior arch screws are used for instrumentation of the atlas. Previous studies have shown that unicortical C1 lateral mass screws are biomechanically stable for fixation. No study has evaluated the biomechanical stability of C1 posterior arch screws or compar

Percutaneous posterior-lateral lumbar interbody fusion for degenerative disc disease using a B-Twin expandable spinal spacer.

Degenerative disc disease (DDD) causes gradual intervertebral space collapse, concurrent discogenic or facet-induced pain, and possible compression radiculopathy. A new minimal invasion procedure of percutaneous posterior-lateral lumbar interbody fusion (PPLIF) using a B-Twin stand-alone expandable spinal spacer (ESS) was designed to treat this disease and evaluated by follow-up more than 1 year. 12 cases with chronic low back pain and compressive radiculopathy due to DDD refractory were selected to conservative treatment. Under fluoroscopy in the posterior-lateral position, a K-wire was advan

Application of periodontal tissue engineering using enamel matrix derivative and a human fibroblast-derived dermal substitute to stimulate periodontal wound healing in Class III furcation defects.

BACKGROUND: Enamel matrix derivative (EMD) has been shown to promote several aspects of periodontal regeneration in vitro and in vivo. Recently, a bioengineered tissue (DG) was developed to promote wound healing of chronic skin ulcers. This pilot study sought to assess the effects of EMD and DG, alone or in combination, on periodontal wound healing in surgically created Class III furcation defects. METHODS: Six female baboons received bilateral ostectomy of approximately 10 mm around the first and second mandibular molars to achieve Class III, subclass C furcation defects. Wire ligatures and

Mechanism of sternotomy dehiscence.

OBJECTIVES: Biomechanical modelling of the forces acting on a median sternotomy can explain the mechanism of sternotomy dehiscence, leading to improved closure techniques. METHODS: Chest wall forces on 40 kPa coughing were measured using a novel finite element analysis (FEA) ellipsoid chest model, based on average measurements of eight adult male thoracic computerized tomography (CT) scans, with Pearson's correlation coefficient used to assess the anatomical accuracy. Another FEA model was constructed representing the barrel chest of chronic obstructive pulmonary disease (COPD) patients. Six,

[Is there a need of an additional extramedullary fixation in transverse subtrochanteric shortening in primary total hip arthroplasty for patients with severe hip dysplasia? Short-term experience in seven patients with congenital dislocation].

AIM: Total hip arthroplasty for high developmental hip dislocations should restore the anatomic hip centre by implanting the acetabular cup in the true acetabulum. The normalisation of the position of the hip may require femoral shortening to avoid the risk of neurological traction injury. In this study group a subtrochanteric transverse shortening with a cement-free stem (Alloclassic-SL) with a rectangular square diameter was used to secure rotation stability. METHOD: With this technique seven primary hip arthroplasties were performed in Crowe grade types III and IV. The age at time of opera

[Management of finger-level avulsion injuries].

BACKGROUND: In avulsion-type injuries of the fingers recovery of blood circulation is one of the major obstacles. The indication for finger reconstruction is discussed controversely, being influenced by the patient's needs, the degree of damage to the soft tissue and the prospects of success of the healing process. In this study we present our results after reconstruction of avulsion-type injuries of the fingers. Indications for finger reconstruction will be assessed in consideration of the expected outcome. PATIENTS AND METHODS: From 1999 to 2006 we treated 18 patients with finger level avul
