Outcomes of early versus late tracheostomy: 2008-2010.

OBJECTIVES/HYPOTHESIS: The ideal timing of tracheostomy varies. This study sought to determine demographic, management, and outcome differences in patients undergoing early tracheostomy (ET) versus late tracheostomy (LT) (<10 days vs. >10 days postintubation, respectively). STUDY DESIGN: Retrospective review of the 2008 to 2010 Nationwide Inpatient Sample for patients with extreme severity of illness who underwent tracheostomy. METHODS: Patients were subdivided based on the timing of tracheostomy placement (days 1-5, 6-10, 11-15, 16-20, 21-25). ET and LT were defined using a 10-day cutoff. D

Solar ultraviolet-B radiation and vitamin D: a cross-sectional population-based study using data from the 2007 to 2009 Canadian Health Measures Survey.

BACKGROUND: Exposure to solar ultraviolet-B (UV-B) radiation is a major source of vitamin D3. Chemistry climate models project decreases in ground-level solar erythemal UV over the current century. It is unclear what impact this will have on vitamin D status at the population level. The purpose of this study was to measure the association between ground-level solar UV-B and serum concentrations of 25-hydroxyvitamin D (25(OH)D) using a secondary analysis of the 2007 to 2009 Canadian Health Measures Survey (CHMS). METHODS: Blood samples collected from individuals aged 12 to 79 years sampled acr…

Different methodologies to quantify uncertainties of air emissions.

Characterization of the uncertainty associated with air emission estimates is of critical importance especially in the compilation of air emission inventories. In this paper, two different theories are discussed and applied to evaluate air emissions uncertainty. In addition to numerical analysis, which is also recommended in the framework of the United Nation Convention on Climate Change guidelines with reference to Monte Carlo and Bootstrap simulation models, fuzzy analysis is also proposed. The methodologies are discussed and applied to an Italian example case study. Air concentration values

The role of particulate size and chemistry in the association between summertime ambient air pollution and hospitalization for cardiorespiratory diseases.

In order to address the role that the ambient air pollution mix, comprised of gaseous pollutants and various physical and chemical measures of particulate matter, plays in exacerbating cardiorespiratory disease, daily measures of fine and coarse particulate mass, aerosol chemistry (sulfates and acidity), and gaseous pollution (ozone, nitrogen dioxide, sulfur dioxide, and carbon monoxide) were collected in Toronto, Ontario, Canada, in the summers of 1992, 1993, and 1994. These time series were then compared with concurrent data on the number of daily admissions to hospitals for either cardiac d

Age patterns and transmission characteristics of hand, foot and mouth disease in China.

BACKGROUND: Hand, foot and mouth disease (HFMD) has circulated in China and caused yearly outbreak. To understand the transmission of the disease and to assess the spatial variation in cases reported, we examined age-specific transmission characteristics and reporting rates of HFMD for 31 provinces in mainland China. METHODS: We first analyzed incidence spatial patterns and age-specific incidence patterns using dataset from 2008 to 2012. Transmission characteristics were estimated based on catalytic model. Reporting rates were estimated using a simple mass action model from "Time Series Susce

Environmental risk factors for Pneumocystis pneumonia hospitalizations in HIV patients.

BACKGROUND: Pneumocystis pneumonia (PcP) is the second leading cause of morbidity and mortality in human immunodeficiency virus (HIV)-infected patients in the United States. Although the host risk factors for the development of PcP are well established, the environmental (climatological, air pollution) risk factors are poorly understood. The major goal of this study was to determine the environmental risk factors for admissions of HIV-positive patients with PcP to a single medical center. METHODS: Between 1997 and 2008, 457 HIV-positive patients with microscopically confirmed PcP were admitte

Drivers of tick distributions in Latvia: a multi‑species assessment of joint and single‑species distribution models (2017-2019).

The ecological factors shaping the distribution of key tick vector species remain only partially understood. We analysed the spatial patterns of three medically important ticks, Dermacentor reticulatus, Ixodes persulcatus, and Ixodes ricinus in Latvia, using extensive field records from a single survey combined with environmental predictors harmonised to a 1 km spatial resolution. We pursued two complementary objectives: (i) to model and map the spatial distribution of three epidemiologically important tick species in Latvia, and (ii) to evaluate whether a joint species distribution modelling

Assessing rectal temperature with a novel non-invasive sensor.

Athletes, soldiers, and workers who perform intense physical activities under extreme hot conditions might encounter increased physiological thermal strain. Consequently, the increase in body core temperature (Tc) might result in heat exhaustion and heatstroke. Thus, continuously following changes in Tc is of utmost importance. Recently, the Tcore sensor (Dräger, Germany), which employs a unique dual-sensor heat flux technology, became commercially available to measure Tc, in a hospital-controlled environment. This study aimed to evaluate the possibility of using the Tcore sensor to accurately

Impact of Increasing Ambient Temperatures on Emergency Department Trauma Presentations at a Level I Trauma Center.

INTRODUCTION: Global temperatures are increasing because of climate change, affecting human health. Elevated ambient temperature has been shown to increase rates of trauma presentations in emergency departments and to increase antisocial behavior, such as violent crime. To better understand how to reduce the burden on trauma hospitals, we must better understand the relationship between ambient temperature, trauma patients, and intentional injury. We sought to examine the relationship between trauma rates and minimum and maximum daily temperatures and assess whether the assault trauma mechanism

Practice Environment and Job Outcomes Among Primary Care Nurse Practitioners Caring for Patients With Dementia.

BACKGROUND: As the number of Americans living with dementia increases, healthcare delivery systems face increasing pressure to provide dementia-care services to improve patient and family outcomes. The primary care nurse practitioner (NP) workforce is critical to dementia care; however, many NPs practice in environments that lack organizational attributes needed to maximize their contributions. We examined the associations between the NP practice environment and key indicators of workforce sustainability-burnout, job satisfaction, and intent to leave. PARTICIPANTS AND SETTING: We conducted a

Biodiversity surrogates in freshwater environments: What determines their effectiveness?

Biodiversity surrogates, such as the Indicator Species Approach (ISA) and Higher-Taxon Approach (HTA), offer cost-effective alternatives to species-level identification. However, their reliability varies across studies. This article presents a large-scale evaluation of ISA and HTA using nearly 10,000 freshwater macroinvertebrate assemblages. We focused on two common surrogates: the EPT group (Ephemeroptera, Plecoptera, Trichoptera) as a proxy for the entire macroinvertebrate community (ISA), and family-level data as a proxy for species-level data (HTA). We tested whether sampling effort, the s

A Müller Glia-Retinal Pigment Epithelium Apical Network Surrounding Cone Outer Segments in Fish and Mouse.

Understanding how diverse headwater streams contribute water downstream is critical for accurate modeling of seasonal flow dynamics in larger systems. This study investigated how headwater catchments, with diverse subsurface storage, influence downstream flows within Lookout Creek-a 62 km2, 5th-order catchment in the rain-snow transition zone in western Oregon, USA. We analyzed one year of hydrometric and water stable isotope data collected at ten stream locations, complemented by a decade of precipitation isotopic data. As expected, isotopic data revealed that most of the streamflow was sourc

Taller Trees Experienced Less Crown Damage During a Severe Hurricane in a Tropical Forest.

Hurricanes strongly regulate vegetation structure and dynamics in coastal tropical forests by causing tree mortality and damaging the crowns of surviving trees. Despite the importance of this damage for long-term forest demography and carbon dynamics, the patterns and drivers of variation in hurricane-induced damage across heterogeneous landscapes remain poorly quantified. Here, we integrate field inventory data with repeated airborne LiDAR measurements to quantify tree crown damage following Hurricane María (Category 4) in the Luquillo Experimental Forest, Puerto Rico. We applied two approach
