Femoral de-escalation during revision total hip arthroplasty using a rectangular-taper shortened stem.

PURPOSE: Long stems with diaphyseal fixation are widely used in revision total hip arthroplasty (RTHA) but may increase surgical complexity and compromise femoral bone stock. Primary femoral stems, used as a de-escalation strategy, could simplify RTHA in selected patients as a bone preserving strategy. This study aimed to evaluate the clinical and radiographic outcomes of this technique using a cementless rectangular-taper shortened stem during both components RTHA. METHODS: Between January 2019 and January 2025, 301 both components RTHA were prospectively included in our institutional total

A Mid-Term Result of the Treatment of Intra-Articular Calcaneal Fractures with the Use of Intramedullary Nailing.

Background: Intra-articular calcaneal fracture (CF) treatment is associated with a high risk of complications, but closed reduction and internal fixation (CRIF) is a minimally invasive alternative for treatment. Methods: Forty-eight patients treated with CRIF and CALCAnail® due to intra-articular CF between 2016 and 2021 were analyzed to check union time, complication rate, and functionality after the intervention. Functional and pain outcomes were assessed, including the Maryland Foot Score (MFS), American Orthopedic Foot & Ankle Society (AOFAS) scale questionnaires, and the numerical pain sc

The pace of a trauma resuscitation: experience matters.

PURPOSE: Resuscitation quality and pace depend on effective team coordination, which can be facilitated by adequate leadership. Our primary … d during resuscitations led by Staff trauma surgeons (4 thorax drains, 1 endotracheal intubation, 1 closed fracture reduction). Moreover, a significant negative correlation (r:  - 0.698, p < 0.01) between Injury Severity Score and resuscitation time was found. CONCLUSION: Experienced trauma team leaders may positively influence the pace of the resuscitation. Moreover, we …

Instability after hip hemiarthroplasty for femoral neck fracture: an unresolved problem.

BACKGROUND: The dislocated hip hemiarthroplasty (HA) remains a difficult condition to treat owing to frailty, comorbidity, poor quality of bone and soft tissues. We aimed to identify parameters contributing to instability following hip HA and describe the operative management and patient outcomes. METHODS: We retrospectively reviewed consecutive cases of all patients with hip fracture treated between 2004 and 2019 at a single tertiary care institution. We propensity matched patients with and without hip dislocations on a 1:2 basis for age, sex, and approach. We reviewed risk factors for HA di

Instability after reverse total shoulder arthroplasty: risk factors and how to avoid them.

Instability after RTSA (4'7%) remains a complication with limited salvage options... or not? We conducted a study of the incidence, predisposing factors, and treatment of RTSA instability to risk stratify patient and identify the most reliable treatment methods. We retrospectively searched for RTSAs performed between 2008 and 2017 at our institution by one surgeon using the same technique. We identified post- operative dislocations or symptoms of instability. 103 patients underwent 103 RTSAs (97 primary, 6 revision). 6 patients had 5 dislocations (3 in primary RTSAs, 3 in revision RTSAs). Mean

Absence of radiographic abnormalities in nursemaid's elbows.

BACKGROUND: Disagreement exists between physicians on the usefulness of a prereduction radiograph for diagnosis and treatment of nursemaid's elbows in children. Some evidence suggests that nursemaid's elbows have identifying features on radiographs. This study compares the radiographs of nursemaid's elbows to normal, control elbows in children and hypothesizes that differentiating features do not exist on radiograph. METHODS: For this retrospective case-control study, hospital billing records were searched to identify all patients under age 6 treated with closed reduction for a nursemaid's el

Spontaneous modular femoral head dissociation complicating total hip arthroplasty.

Modular femoral heads have been used successfully for many years in total hip arthroplasty. Few complications have been reported for the modular Morse taper connection between the femoral head and trunnion of the stem in metal-on-polyethylene bearings. Although there has always been some concern over the potential for fretting, corrosion, and generation of particulate debris at the modular junction, this was not considered a significant clinical problem. More recently, concern has increased because fretting and corrosive debris have resulted in rare cases of pain, adverse local tissue reaction

Evaluation of the Relationship Between Radial Translation and Functional Outcomes in Nonsurgical and Surgically Treated Distal Radius Fractures.

PURPOSE: This study aimed to evaluate the prognostic importance of radial translation on functional outcomes in patients treated nonoperatively or surgically for distal radial fractures. It was hypothesized that increased radial translation would be associated with worse functional scores, regardless of treatment modality. METHODS: This retrospective comparative study included 130 adult patients (aged 18-70 years) with unilateral AO/OTA 2R3A2 distal radius fractures, treated between January 2020 and December 2024. Group 1 (n = 60) underwent closed reduction and plaster casting, whereas group

[Fixation of pediatric fractures with percutaneous Kirschner-wire: should we leave the wire exposed or buried?].

Introduction: The percutaneous Kirschner-wire fixation is a minimally invasive operative technique in pediatric fracture treatment. The advantage of the K-wire technique is that it requires little instrumentation and removing the wire is easy. The disadvantage is that it requires plaster cast or brace. Objective: Our aim was to examine the following: did exposed wires have more septic complications, were the total hospital stays and the number of narcosis during metal removal lower, than in the case of buried wires? Method: A retrospective observational cohort study was enrolled in Heim Pál Na

Condyle head fracture management: A systematic review of outcomes.

The management of Condylar Head fractures (CHFs) has historically been with closed treatments(CTR); however, contemporary studies suggest that Open Reduction and Internal Fixation(ORIF) may produce better clinical and radiographic results. Our primary aim was to review the literature on the clinical and radiological outcomes of open and closed management of CHFs. A systematic literature search was undertaken using EMBASE, MEDLINE and PubMed, using PRISMA guidelines, for all studies relating to outcomes following CHF. The inclusion criteria include 1) studies focused on CHF in adult patients th
