High-Resolution Models of Human Cochlea for a Study of Neural Activation.

With more than one million implanted devices, the cochlear implant (CI) is the most successful neuroprosthesis and one of the most successful achievements of medical technology in general. While CI users usually regain the ability to understand and use spoken language with or without visual aids, CI performance is widely variable after implantation. After reconstructing anatomically realistic cochlea models from high-resolution X-ray microtomography scans of eight human temporal bone specimens contrast-enhanced with osmium tetroxide, we conducted an in-depth analysis of the stimulus amplitudes

Unveiling the safety landscape: A comprehensive review of the toxicological profile of facial aesthetic implants and biomaterials.

Exploring diverse biomaterials and implants in the ear, nose, and throat by understanding adverse effects and post-usage events. Literature was obtained from Scopus, PubMed, Google Scholar, and Web of Science. A comprehensive analysis was conducted on original research studies, case reports, and case series spanning from December 2010 to May 2022. Our analysis underscores that the effectiveness of cochlear implants (CIs) relies on factors such as biocompatibility, anti-inflammatory measures, and fibrosis reduction. Although silicone is employed in otologic applications, allergic reactions lead

Osseointegration and growth effects of temporal bone percutaneous pedestals.

The percutaneous temporal bone pedestal has shown significant utility for the attachment of bone-anchored hearing aids, attachment of cosmetic auricular prostheses, and as connector between external sound processors and implanted cochlear implants. The biological acceptance of these implants by temporal bone hosts is affected by many factors including the maturity of the bone, the design of the pedestal-fixation system, and the pedestal-construction material. The first phase of this study evaluated the effect of the pyrolized graphite pedestal fixation on maturing temporal bones. Pedestals wer

Photobiomodulation Can Enhance Stem Cell Viability in Cochlea with Auditory Neuropathy but Does Not Restore Hearing.

Sensorineural hearing loss is very difficult to treat. Currently, one of the techniques used for hearing rehabilitation is a cochlear implant that can transform sound into electrical signals instead of inner ear hair cells. However, the prognosis remains very poor if sufficient auditory nerve cells are not secured. In this study, the effect of mouse embryonic stem cells (mESC) and photobiomodulation (PBM) combined treatment on auditory function and auditory nerve cells in a secondary neuropathy animal model was investigated. To confirm the engraftment of stem cells in vitro, cochlear explants

[Management of the acoustic tumor in an only/better hearing ear].

Because profound bilateral hearing impairment is a catastrophic event, the management and care of an individual with an 8th nerve lesion in an only/better hearing ear remains a significant challenge for both patient and physician. Current options regarding the treatment of the acoustic tumor in an only/better hearing ear include: observation, attempted hearing preservation surgery and stereotactically guided radiation therapy. In this article, we present 3 cases of acoustic tumor within the internal auditory canal in an only/better hearing ear diagnosed by gadolinium-enhanced MRI and discuss t

Promontory electrical stimulation in postoperative acoustic tumor patients.

Complete deafness can follow acoustic tumor surgery and results from labyrinthine injury, auditory nerve trauma, and/or vascular compromise. A central auditory prosthesis is one potential rehabilitative strategy in such patients. Anatomical studies suggest that some spiral ganglion cells may survive after vascular occlusion, and we have demonstrated responses to electrical stimulation in patients after labyrinthectomy. It was thus hypothesized that patients deafened after a hearing conservation attempt, but maintaining an intact auditory nerve, could utilize an intracochlear implant. To invest

Auditory brainstem implant outcomes and MAP parameters: report of experiences in adults and children.

UNLABELLED: The auditory brainstem implant (ABI) was first developed to help neurofibromatosis type 2 patients. Recently, its use has been recently extended to adults with non-tumor etiologies and children with profound hearing loss who were not candidates for a cochlear implant (CI). Although the results has been extensively reported, the stimulation parameters involved behind the outcomes have received less attention. OBJECTIVE: The aim of this study is to describe the audiologic outcomes and the MAP parameters in ABI adults and children at our center. METHODS: Retrospective chart review.

ABI-auditory brainstem implant.

The Auditory Brainstem Implant (ABI) is based on the classic cochlear implant (CI) but uses a different stimulation electrode. At MED-EL, the early development activities on ABI started in the year 1994, with the suggestion coming from J. Helms and J. Müller from Würzburg, Germany in collaboration with the Univ. of Innsbruck Austria. The first ABI surgery in a neuro-fibromatosis (NF2) patient with the MED-EL device took place in the year 1997. Later, the indication of ABI was expanded to non-NF2 patients with severe inner-ear malformation, for whom a regular CI will not be beneficial. Key tran

Retrosigmoid approach for auditory brainstem implant.

The present paper reports our experience with the surgical retrosigmoid-transmastoid (RS-TM) technique for implanting auditory brainstem implants (ABIs). From April 1997 to August 1998, four patients with neurofibromatosis type 2 (NF2) were operated on for vestibular schwannoma removal with ABI implantation. The subjects (three men and one women) ranged in age from 22 to 31 years. Tumour size ranged from 12 to 30 mm. A classical RS-TM approach was performed. After tumour excision, identification of landmarks (VIIth, VIIIth and IXth cranial nerves, choroid plexus) to the foramen of Luschka was

Systematic review of outcome domains and instruments used in designs of clinical trials for interventions that seek to restore bilateral and binaural hearing in adults with unilateral severe to profound sensorineural hearing loss ('single-sided deafness').

BACKGROUND: This systematic review aimed to identify, compare and contrast outcome domains and outcome instruments reported in studies investigating interventions that seek to restore bilateral (two-sided) and/or binaural (both ears) hearing in adults with single-sided deafness (SSD). Findings can inform the development of evidence-based guidance to facilitate design decisions for confirmatory trials. METHODS: Records were identified by searching MEDLINE, EMBASE, PubMed, CINAHL, ClinicalTrials.gov, ISRCTN, CENTRAL, WHO ICTRP and the NIHR UK clinical trials gateway. The search included records

Chronic intracochlear electrical stimulation at high charge densities: reducing platinum dissolution.

OBJECTIVE: Cochleae of long-term cochlear implant users have shown evidence of particulate platinum (Pt) corroded from the surface of Pt electrodes. The pathophysiological effect of Pt within the cochlea has not been extensively investigated. We previously evaluated the effects of Pt corrosion at high charge densities and reported negligible pathophysiological impact. The present study extends this work by examining techniques that may reduce Pt corrosion. APPROACH: Deafened guinea pigs were continuously stimulated for 28 d using biphasic current pulses at extreme charge densities using: (i)

Experience with a subcutaneous implantable bone conduction hearing aid (Xomed Audiant) in a district general hospital.

Patients with a conductive hearing loss may find conventional hearing aids unsatisfactory for a variety of reasons. Efforts to overcome some of these difficulties have led to the development of the osteointegrated hearing aid. Implantable bone conduction hearing aids are an important advance in the treatment of well selected patients. Only 18 patients, who have been implanted with the Xomed Audiant bone conduction hearing aid, have so far been reported on in the UK. This report concerns a further six patients who have been implanted in one centre. Five of these patients continue to use their A
