The accuracy of 3T magnetic resonance cholangiopancreatography in suspected choledocholithiasis.

BACKGROUND: Femoral neck fractures (FNF) are associated to patient's disability, reduced quality of life and mortality. None of the fixation devices commonly used for extracapsular (EC) FNF (i.e., dynamic hip screws (DHS) and intramedullary nails (IN)) is clearly superior to the other, especially in case of unstable fractures (31.A2 and 31.A3 according to AO/OTA classification). The aim of our study was to identify a sub-population of patients with EC fractures in which better outcomes could be obtainable using total hip arthroplasty (THA). METHODS: All patients with EC unstable fractures tre

Subtrochanteric fracture: the effect of cerclage wire on fracture reduction and outcome.

INTRODUCTION: Subtrochanteric neck of femur fractures are a challenge to treat due to anatomical and biomechanical factors. Poor reduction, varus deformity, nonunion and return to theatre risks are high. A cerclage wire can augment an intramedullary nail to help fracture reduction and construct stability. Concerns exist regarding the use of cerclage wire on fracture zone vascularity. The aim of this study was to assess the benefits and adverse outcomes associated with the use of cerclage wiring. PATIENTS AND METHODS: A 7-year retrospective review of all subtrochanteric fractures at a Level 1

Anatomic Structures at Risk With Antegrade Intramedullary Fixation of the First Metacarpal Using Three Start Points: Is There A Safe Zone?

PURPOSE: Intramedullary fixation of extraarticular first metacarpal fractures using a percutaneous technique may risk injury to important anatomic structures. The purpose of this study is to evaluate potential safe zones for antegrade implant entry of the first metacarpal and describe the anatomical structures at risk. METHODS: Ten cadaveric hands were used to evaluate three start points located at the first metacarpal base: volar-radial, dorsal-radial, and dorsal-ulnar. A fluoroscopic imaging system was used to guide percutaneous placement of 0.8 mm × 100 mm guidewires and 2.0 mm diameter sc

Intramedullary locking mini-plate screw system in Lindgren osteotomy: a more effective fixation technique in mild and moderate hallux valgus cases.

OBJECTIVE: In the treatment of mild and moderate hallux valgus (HV) cases, the Lindgren distal osteotomy (LDO) technique has been one of the most frequently preferred methods, and screw fixation has been the most preferred material for surgery. However, considering the implant removal operations due to the reaction to the material,  the risk of unsteadiness, retarded union, and malunion has limited the amount of safe application of lateralization of the distal fragment in distal osteotomy surgery and has yielded the researchers for a better alternative. This study compared the results of the L

Management of a Nonunion Type IIC Monteggia Fracture.

INTRODUCTION: Infected non-union of long bones represents one of the most challenging problems in orthopedic surgery. Conventional two-stage approaches involving debridement, antibiotic spacers, and subsequent fixation are associated with high morbidity and prolonged treatment duration. MATERIALS AND METHODS: We present a prospective case series of 10 patients with infected non-union of various long bones (tibia, femur, humerus) treated with antibiotic cement-coated intramedullary nails (AC-CIMN). The nail was coated with polymethylmethacrylate mixed with culture-sensitive antibiotics in a si

Deformity Correction and Limb Lengthening in Maffucci Syndrome - A Case Report.

INTRODUCTION: ?Maffucci syndrome (MS) is a rare disorder with enchondromatosis associated with multiple hemangiomas in soft tissue and internal organs. Enchondromas in the long bone lead to limb length discrepancy, deformity, and pathological fractures that usually need corrective osteotomies and limb lengthening. CASE REPORT: A female in her late adolescence with MS presented with significant varus deformity of the distal femur and left lower limb shortening of 11.5 cm. She underwent an acute correction of the varus with a distal femur osteotomy and gradual femoral lengthening using a monola

Posteromedial elastic stable intra-medullary nailing (ESIN) in volarly displaced metaphyso-diaphyseal distal radius fractures in child.

INTRODUCTION: The distal metaphyso-diaphyseal fractures of the radius with volar angulation are generally considered unstable. Too distal to be treated with classic elastic stable intramedullary nailing (ESIN) via a laterodistal approach and too proximal to be stabilized with simple conservative treatment, they are sometimes treated by plating. PATIENTS AND METHODS: To avoid the disadvantages of the open exposure necessary for this latter fixation and to prevent volar angulation while respecting the curvature of the radius, radial elastic nailing with posteromedial distal entry was used in 16

Minimal invasive long PHILOS®-plate osteosynthesis in metadiaphyseal fractures of the proximal humerus.

Minimal invasive plate osteosynthesis (MIPO) not only meets the criteria of a "biological" osteosynthesis by minimising invasivity as well as iatrogenic soft tissue damage, but can also provide adequate stability for fracture healing and early functional aftertreatment. Up to date, only few publications report on MIPO of humeral shaft fractures mainly using the anterolateral deltopectoral approach for proximal plate insertion. Objective of the present study was to assess the feasibility and clinical outcome of MIPO for metadiaphyseal fractures of the proximal humerus through a lateral approach

Anatomic evaluation of radiographic landmarks for accurate straight antegrade intramedullary nail placement in the humerus.

BACKGROUND: Neurovascular insult, nonunion, and iatrogenic rotator cuff injury are concerns when using an intramedullary nail (IMN) for proximal humerus fracture. The purpose of this study was to identify a reproducible starting point and intraoperative imaging for nail insertion optimizing nail depth, tuberosity screw position, and protecting the axillary nerve and rotator cuff insertion. Our hypothesis was that a more medialized starting point would protect soft tissue structures and improve locking screw positioning. METHODS: Ten fresh-frozen cadavers underwent antegrade IMN with Grashey a

Numerical analysis of an osseointegrated prosthesis fixation with reduced bone failure risk and periprosthetic bone loss.

Currently available implants for direct attachment of prosthesis to the skeletal system after transfemoral amputation (OPRA system, Integrum AB, Sweden and ISP Endo/Exo prosthesis, ESKA Implants AG, Germany) show many advantages over the conventional socket fixation. However, restraining biomechanical issues such as considerable bone loss around the stem and peri-prosthetic bone fractures are present. To overcome these limiting issues a new concept of the direct intramedullary fixation was developed. We hypothesize that the new design will reduce the peri-prosthetic bone failure risk and adver

Perioperative Selective Serotonin Reuptake Inhibitor (SSRI) Use and Postoperative Fracture Fixation Outcomes: A Multi-institutional Retrospective Cohort Study.

INTRODUCTION: Selective serotonin reuptake inhibitors (SSRIs) have been implicated in impaired bone metabolism and delayed fracture healing. This study evaluates the association between perioperative SSRI use and complications following open reduction and internal fixation (ORIF) and intramedullary nailing (IMN) of various long bone shaft fractures. The study hypothesis is that perioperative SSRI use negatively affects fracture healing and increases adverse postoperative outcomes across fixation constructs. METHODS: The TriNetX database was used to identify patients (≥18 years) undergoing hum

Reaming-induced joint surface destruction in tibiotalocalcaneal nailing: a cadaveric comparison of straight and valgus-curved nails.

BACKGROUND: Tibiotalocalcaneal (TTC) nailing has become an increasingly preferred option for the treatment of complex ankle pathologies and fragility fractures, particularly in geriatric patients. Despite its growing clinical use, limited data are available regarding how different nail designs impact joint surface integrity during intraoperative preparation. This cadaveric study aimed to compare the articular surface destruction caused by straight and valgus-curved TTC nails. METHODS: 20 fresh-frozen cadaveric lower limbs (mean age 70.1 ± 4.1 years) were randomized to receive either a straigh

Treatment of Upper Cervical Spinal Cord Injury (Unstable C1-C2) by Direct Visualization and Nailing Technique and the Advantages of Early MRI.

Источник пометил публикацию как отозванную

BACKGROUND: The treatment of C1-C2 fractures mainly depends on fracture type and the stability of the atlantoaxial joint. Disruption of the C1-C2 combination is a big challenge, especially in avoiding vertebral artery, nerve, and vein sinus injury during the operation. PURPOSE: This study aims to show the benefit of using the posterior approach and pedicle screw insertion by nailing technique and direct visualization to treat unstable C1-C2 and, moreover, to determine the advantages of performing early MRI in patients with limited neck movement after trauma. METHOD: Between Jan 2017-Feb 2019

3D Optical Investigation of 2 Nail Systems Used in Tibiotalocalcaneal Arthrodesis: A Biomechanical Study.

BACKGROUND: Although retrograde intramedullary nails for tibiotalocalcaneal arthrodesis (TTCA) are an established fixation method, few studies have evaluated the stability of the available nail systems. The purpose of this study was to compare biomechanically the primary stability of 2 nail-systems, A3 (Small Bone Innovations) and HAN (Synthes), in human cadavers and analyze the exact point of instability in TTCA by means of optical measurement. METHODS: In 6 pairs of lower legs (n = 12) of fresh-frozen human cadavers with osteoporotic bone structure, bone mineral density (BMD) was determined
