Factors associated with malocclusions in children and adolescents with Down syndrome.

INTRODUCTION: Our aims in this study were to determine the prevalence of malocclusion stemming from vertical or transversal occlusal alterations in subjects with Down syndrome (DS) and the associations with individual, socioeconomic, and behavioral factors. METHODS: A cross-sectional study was carried out with 112 pairs of mothers and their children with DS between 3 and 18 years of age attending a genetics clinic at a public hospital in Rio de Janeiro, Brazil. Data were collected with a questionnaire given to the mothers and a clinical examination of the child or adolescent. Univariate, biva

Surgical-orthodontic treatment of a patient with temporomandibular disorder stabilized with a gnathologic splint.

The condylar position was stabilized with a gnathologic splint designed to achieve a mutually protected occlusion in a patient with internally deranged joints whose symptoms had not been resolved with an anterior repositioning splint. A diagnosis was made in the stable condylar position from which hinge axis movement of the mandible could be performed. After preoperative orthodontic treatment, the mandibular position was restabilized with splint therapy; then, a sagittal split ramus osteotomy was performed, by using an overcorrection splint. As a result, the stable position of the condyles was

Craniofacial computerized tomography analysis of the midface of patients with repaired complete unilateral cleft lip and palate.

INTRODUCTION: Computerized tomography (CT) makes regional anatomy available as a series of cross-sectional axial images that can be analyzed unobstructed by other anatomic features. In this study, we aimed to analyze the maxillary dentoalveolar and deeper structures in patients with surgically repaired complete unilateral cleft lip and palate (CLP) and to investigate whether significant asymmetry existed in the deeper midfacial structures, using axial CT. METHODS: Axial CT scans of 15 children (7 boys, 8 girls; mean age, 11.59 +/- 1.34 years) with repaired complete unilateral CLP were acquire

Adult patient with mandibular protrusion and unstable occlusion treated with titanium screw anchorage.

Orthodontic treatment for adults can be difficult because they often have several problems, including some associated with aging. This article reports the successful use of miniscrews to treat a 51-year-old woman with Class III malocclusion, unstable occlusion, periodontal disease, and temporomandibular disorder. The patient had a skeletal Class III jaw-base relationship, anterior and unilateral posterior crossbites, and a deviated midline. It was therefore necessary to confirm her mandibular position with a stabilization splint. Miniscrews were implanted in the retromolar area and used to mov

Surgical positioning of orthodontic mini-implants with guides fabricated on models replicated with cone-beam computed tomography.

INTRODUCTION: This article illustrates a new surgical guide system that uses cone-beam computed tomography (CBCT) images to replicate dental models; surgical guides for the proper positioning of orthodontic mini-implants were fabricated on the replicas, and the guides were used for precise placement. The indications, efficacy, and possible complications of this method are discussed. METHODS: Patients who were planning to have orthodontic mini-implant treatment were recruited for this study. A CBCT system (PSR 9000N, Asahi Roentgen, Kyoto, Japan) was used to acquire virtual slices of the poste

Determining the cessation of vertical growth of the craniofacial structures to facilitate placement of single-tooth implants.

INTRODUCTION: Single-tooth implants are commonly used to replace congenitally missing teeth in adolescent orthodontic patients. However, if implants are placed before cessation of facial growth, they will submerge relative to the adjacent erupting teeth. Therefore, it is important to know when facial growth is complete in postpubertal orthodontic patients. The purposes of this study were to determine and quantify the amount of vertical growth of the facial skeleton and the amount of eruption of the central incisors and the maxillary first molars after puberty. METHODS: Two or 3 lateral cephal

Idiopathic failure of eruption of multiple permanent teeth: report of 2 adults with a highlight on molecular biology.

Multiple unerupted teeth with no obvious etiology is a rare dental anomaly. Various local and systemic factors have been implicated in the failure of eruption of multiple permanent teeth. But the localization of the genetic defect in the phenotype of failure of eruption is largely unknown at present. Our aims in this article were to consolidate and organize the available information regarding the molecular biology of tooth eruption and to corroborate the current evidence with the report of 2 adult cases of failure of eruption of multiple permanent teeth without a known cause.

Longitudinal study of facial skeletal growth completion in 3 dimensions.

INTRODUCTION: Previous authors have suggested that transverse facial skeletal growth is completed before either anteroposterior or vertical growth and that anteroposterior growth is completed before vertical growth. Our purpose in this study was to examine this concept. METHODS: Twenty-four subjects (11 male, 13 female) who had annual lateral and posteroanterior cephalograms up to and including age 17 or 18 and again at age 25 or older were identified from the Iowa Facial Growth Study. Transverse, anteroposterior, and vertical facial dimensions were measured longitudinally into adulthood by u

Effect of cephalometer misalignment on calculations of facial asymmetry.

INTRODUCTION: In this study, we evaluated errors introduced into the interpretation of facial asymmetry on posteroanterior (PA) cephalograms due to malpositioning of the x-ray emitter focal spot. We tested the hypothesis that horizontal displacements of the emitter from its ideal position would produce systematic displacements of skull landmarks that could be fully accounted for by the rules of projective geometry alone. METHODS: A representative dry skull with 22 metal markers was used to generate a series of PA images from different emitter positions by using a fully calibrated stereo cepha

Anterior open bite with temporomandibular disorder treated with titanium screw anchorage: evaluation of morphological and functional improvement.

Skeletal anterior open bite is one of the most challenging malocclusions to correct because it is difficult to establish absolute anchorage for molar intrusion with traditional orthodontic mechanics. In addition, patients with anterior open bite sometimes have signs and symptoms of temporomandibular disorders (TMD). In this article, we report the successful treatment of a patient with severe skeletal anterior open bite and TMD; we used titanium screw anchorage. The patient, a woman, age 19 years 11 months, had an open bite of -4.0 mm and increased anterior lower facial height. The titanium scr
