Management of large aortopulmonary collateral arteries in patients with ventricular septal defect and pulmonary atresia: simultaneous ligation through median sternotomy during intracardiac repair.

Nine consecutive patients with ventricular septal defect (VSD) and pulmonary atresia associated with large aortopulmonary collateral arteries underwent primary repair with simultaneous ligation of these collateral arteries. The patients ranged from 1 year to 20 years old (average, 8.0 years). The average number of large aortopulmonary collateral arteries was 1.9 per patient. Arborization abnormality was found in 5 patients. The aortopulmonary collateral arteries were reached solely through a median sternotomy, with dissection of the posterior pericardium or anterior mediastinal pleura before o

The manubriosternal joint--ananatomicoradiological survey.

Cameron, H. U. and Fornasier, V. L. (1974).Thorax, 29, 472-474. The manubriosternal joint—an anatomicoradiological survey. The movement of the manubriosternal joint is said to be a significant factor in respiration. The incidence of fusion of the manubriosternal joint in necropsy cases has been investigated with fine detail radiography. In over 600 cases, fusion of the joint was found in 14% and partial fusion in 8%. In total, the joint was found to be abnormal in 66% of cases. There was an increase in fusion with increasing age. Little relationship was found with degenerative disc disease, an

Upper Sternal Cleft Associated With Congenital Cardiac Defect: Single Stage Surgical Correction.

Sternal cleft is a rare congenital malformation of anterior chest wall resulting from the failure of mesodermal migration to the midline during early embryological development. It may occur as an isolated anomaly or in connection with other congenital defects, most commonly of the cardiovascular system. The authors report the case of a 6-month-old girl with an upper sternal cleft combined with ventricular and atrial septal defects, who underwent single-stage surgical correction. The cosmetic and functional outcomes of the reconstruction were very good, with a stable anterior chest wall. The ch

"Anchor-type" incision for surgical stabilization of anterior rib fractures: a single center experience and evaluation.

PURPOSE: Rib fixation is increasingly used, but bilateral anterior fractures are difficult to access. We assessed feasibility and outcomes of a single muscle-sparing "anchor-type" anterior exposure for bilateral parasternal fractures with or without sternal fractures. METHODS: Single-center retrospective study at a level I trauma center from January 1, 2016 to April 30, 2024. Adults with bilateral parasternal rib fractures who underwent surgical stabilization of rib fractures (SSRF) through an anchor-type incision were included; sternal plating was performed when indicated. Collected variable

Orthoplastic management of delayed sternal osteomyelitis and non-union: a retrospective case series applying fracture-related infection principles.

BACKGROUND: Sternal osteomyelitis (OM), particularly in the context of late-presenting non-union following cardiothoracic surgery, remains a poorly understood and inconsistently managed condition. Delayed presentations involve established infection, sternal instability, and biofilm formation, requiring a multidisciplinary approach. BOAST guidelines for fracture-related infection (FRI) recommend a combined orthoplastic approach to manage these complex cases. METHODS: This retrospective case series identified all patients referred to a specialist bone infection unit with delayed onset sternal O

Minimally invasive repair of pectus excavatum in children: Results of a modified Nuss procedure.

INTRODUCTION: Pectus excavatum (PE) is the most common deformity of the anterior thoracic wall. The Nuss technique allows the thorax to be reshaped with the aid of a retrosternal metallic bar. The aim of this study is to evaluate and compare the complication rate between the original Nuss technique and a lightly modified approach. MATERIAL AND METHOD: We performed a retrospective single-center observational study based on the medical files of patients operated for PE in the Pediatric Surgery Unit between July 2004 and July 2015. We divided two patient groups according to the operating techniq

Introducer Development for Coronary Sinus Access From Parasternal Mediastinotomy.

OBJECTIVE: Right parasternal mediastinotomy with right atriotomy has been used clinically for pacemaker insertion. A similar approach might facilitate access to the coronary sinus for biventricular pacing and other manipulations when more conventional approaches are not feasible. The primary barrier to this is lack of appropriate introducers and techniques. METHODS: Anatomically derived introducers were developed in 2 anesthetized domestic pigs using data from computerized axial thoracic tomography. Each digitized tomogram defined a unique introducer shape and was constructed using 3-dimensio

Elevation of serum lactate dehydrogenase in patients with pectus excavatum.

INTRODUCTION: Pectus excavatum is the most common congenital chest wall deformity and the depression of the anterior chest wall, which compresses the internal organs. The aim of the present study is to investigate the effects of pectus excavatum on blood laboratory findings. MATERIAL AND METHODS: From March 2011 to December 2011, 71 patients with pectus excavatum who visited Seoul Saint Mary Hospital for Nuss procedure were reviewed and analyzed. The blood samples were routinely taken at the day before surgery and pectus bar removal was usually performed in 2 to 3 years after Nuss procedure.

Presternal dermoid cyst mimicking lymphatic malformation: a case report and review of the literature.

We describe an 11-month-old boy with an unusually large presternal mass present since birth. The large size, fluctuant properties, transillumination, compressibility, and imaging of this lesion were characteristic of a lymphatic malformation. Although four treatments with sclerotherapy markedly reduced its size, it was not until definitive treatment with surgical excision and the final pathology report that we arrived at the ultimate diagnosis of dermoid cyst. Dermoid cysts, although appearing along embryologic lines of closure, are rarely presternal. They are usually small, thick walled, and

Optimal prediction of the central venous catheter insertion depth on a routine chest x-ray.

OBJECTIVE: Cardiac tamponade is a serious complication of central venous catheter (CVC) insertion. The position of the carina has been shown to be near the pericardial reflection and can easily be identified as a landmark on routine chest x-ray (CXR). The purpose of this study was to show a simple way to predict optimal CVC depth, thereby facilitating safe positioning of the CVC tip. METHODS: Subjects included 119 inpatients undergoing gastroenterologic surgery. Central venous catheterization was performed through the right internal jugular vein or the right subclavian vein. The insertion dep

Laryngeal Mask Airway insertion with total intravenous anesthesia for transsternal thymectomy in patients with myasthenia gravis: report of 5 cases.

Myasthenia gravis is a chronic autoimmune disease characterized by a reduction of postsynaptic nicotinic acetylcholine receptors at the neuromuscular junction. Most myasthenia gravis patients require thymectomy. Intravenous (IV) anesthetics may be superior to inhalation agents in these patients. The Laryngeal Mask Airway (LMA), when compared with the endotracheal tube, causes less airway resistance, which in turn may lead to a decreased bronchoconstrictive reflex, less atelectasis, and fewer pulmonary infections. We report 5 patients with myasthenia gravis, who underwent transsternal thymectom

Titanium plate osteosynthesis for the correction of severe sternal deformity in a 13-year-old boy.

In a 13-year-old boy a correction of an atrioventricular septal defect (AVSD) was performed at the age of one year and a mitral valve reconstruction at the age of two years. The patient developed, after two median sternotomies, a massive sternal deformity. At the age of 12 years another mitral valve reconstruction had to be performed. At this time titanium plates were used for sternal closure and correction of the deformity. Six months later, and after a CT-follow-up, the plates could be removed with a very satisfying result. Sternal plate osteosynthesis represents an excellent alternative for

Posterior pericardial ascending-to-descending aortic bypass through median sternotomy.

BACKGROUND AND AIM: Adult patients with complex forms of descending aortic disease remain a surgical challenge and have a high risk of postoperative mortality and morbidity. Surgical management may be complicated when there is an associated cardiac defect, necessitating repair, or a hostile anatomy exists. We present our experience with extra-anatomic bypass through posterior pericardial route at the same stage with intracardiac/ascending aortic aneurysm repair. METHODS: Patients that underwent one-stage surgery with posterior pericardial bypass between ascending and descending aorta during 2

Compressive orthotic bracing in the treatment of pectus carinatum: the use of radiographic markers to predict success.

PURPOSE: The treatment of pectus carinatum (PC) has classically been operative, though compressive orthotic braces have been used with good success in recent years. The purpose of this article is to evaluate the use of radiologic measurements in a successful bracing protocol. METHODS: Sixty-three patients with PC have been evaluated for an 8-year span. The average age is 13.3 +/- 2.5. Follow-up is from 4 to 60 months, with a median of 24 months. Seventeen patients with mild defects elected observation alone. The remaining 46 patients began the bracing protocol. Baseline chest computed tomogra

Use of pre-operative Tc99m-Sestamibi scintigraphy and intraoperative parathyroid hormone monitoring to eliminate neck exploration in mediastinal parathyroid adenocarcinoma.

A 66-year-old white woman was found to have an elevated serum calcium and parathyroid hormone (PTH) on routine health evaluation. Physical examination was unremarkable as was ultrasonography of the neck. A sestamibi parathyroid scan revealed abnormal uptake in the anterior mediastinum. Computed tomography of the chest demonstrated an anterior mediastinal mass compatible with a parathyroid adenoma but no neck masses. The patient underwent mediastinoscopy that was converted to a median sternotomy to fully access the mass. The mass was completely resected with surrounding thymus gland. Frozen sec

Surgical approach for Stanford type A aortic dissection in a patient with Marfan syndrome and pectus excavatum.

Acute Stanford type A aortic dissection in a patient with severe pectus excavatum constitutes a surgical emergency and presents a major challenge for the surgeon. Decisions must be made regarding the operative approach and whether the pectus excavatum should be corrected during the same session. Herein, we describe a case of acute aortic dissection in a patient who had Marfan syndrome with severe pectus excavatum. Combined partial upper sternotomy and left anterior thoracotomy provided excellent surgical exposure, and the aortic root and ascending aorta were completely replaced. The procedures
