Could Helicobacter pylori play a role in the aetiopathogenesis of tympanosclerosis?

AIM: The aetiology of tympanosclerosis is not yet clear. This prospective, controlled, clinical study investigated the relationship between Helicobacter pylori and tympanosclerosis aetiology. MATERIALS AND METHODS: The study included 14 patients with tympanosclerosis and 26 with other forms of chronic otitis media. All patients underwent surgery for chronic otitis media. Mucosal biopsies were taken, and examined for H pylori using the Campylobacter-Like Organism (CLO) test. RESULTS: Tympanoplasty was performed in 29 patients (72.5 per cent), radical mastoidectomy in eight (20 per cent) and m…

Liposarcoma of temporal bone: a case report.

This is the third case report of liposarcoma of the temporal bone. A 69-year-old man complained of left otalgia, left otorrhea, and dizziness for several months, with a past history of tympanoplasty for cholesteatoma 28 years previously. Otoscopy revealed debris in the attic. We performed tympanoplasty for recurrent cholesteatoma. The postoperative pathological diagnosis was well-differentiated liposarcoma. Palliative resection of liposarcoma was performed 3 months after the first surgery. Neither local recurrence nor distant metastasis has been detected for 24 months after the first surgery.

The temporal branch of the facial nerve: how reliably can we predict its path?

A thorough examination of the temporal branch of the facial nerve was performed to characterize precisely the number of rami crossing the zygomatic arch and their location with respect to bone and soft-tissue landmarks. Fresh cadaver dissection was performed in 12 facial halves, dissecting the facial nerve superiorly from the stylomastoid foramen to identify all branches crossing the zygomatic arch. There were a median of three (range two to four) rami of the temporal branch crossing the lower aspect of the zygomatic arch, with distinct anterior and posterior divisions identified in each disse…

Donor site morbidity of greater auricular nerve graft harvesting.

To better understand the risks of obtaining greater auricular nerve (GAN) grafts, a retrospective analysis of 29 patients who underwent GAN graft procurement between 1985 and 1990 was conducted. No short-term morbidity was noted. Thirteen patients developed symptomatic nerve injuries, of which 6 reported spontaneous resolution in an average of 4.6 months. Three patients developed neuromas and 1 formed a hypertrophic scar. Persistent nerve injury symptoms were well tolerated in all but one patient, who developed sympathetic-mediated pain.

Surgical obliteration of the tympanomastoid compartment and external auditory canal.

The ideal patient for a radical mastoidectomy with total tympanomastoid cavity obliteration is one with chronic granulomatous otomastoiditis without cholesteatoma, profound sensorineural hearing loss, and a normal ear on the opposite side. A meticulous and thorough classical radical mastoidectomy is required. The resultant cavity is eliminated by filling it with pedicled flaps and/or adipose tissue taken from the abdominal wall. Suturing the skin of the anterior and posterior membranous canal walls completes the procedure. Healing is rapid and requires minimal postoperative care. The absence o…

Vulnerable structures during intraoral sagittal split ramus osteotomy.

The aim of this study was to elucidate the anatomical structures that are vulnerable to injury during sagittal split ramus osteotomy (SSRO). … The FNT was located in the range of 11 to 14 mm medial to the PBM. The FNT emerged out of the stylomastoid foramen and ran anteroinferiorly in a direction of 45 degrees. In 10-mm ramus setback osteotomy, FNT was very close to the PBM, running almost vertically. Retromandibular vein was 5.5 to 8.6 mm posterior and 4.2 to 9.1 mm medial to the PBM. The ECA …

Long-term outcomes after tympanoplasty with and without mastoidectomy for perforated chronic otitis media.

The objectives of the retrospective study, performed at a tertiary referral center, were to examine the prognostic factors predicting long-term outcomes of tympanoplasty for perforated chronic otitis media (COM) and to determine whether mastoidectomy can be avoided during tympanoplasty for perforated COM. Between 1987 and 2002, 213 patients with perforated COM underwent tympanoplasty by the same surgeon and were followed for more than 5 years. Postoperative hearing outcomes were considered successful, if the postoperative air-bone gap was within 20 dB. Closure of perforation by a single surger…

[Improvement of registration accuracy for navigated-control drill in mastoidectomy (autopilot)].

BACKGROUND: The goal of this study is the improvement of the surgical accuracy of a navigate-controlled drill for mastoidectomy in a lab test. METHODS: For lab tests an artificial model of the temporal bone with color-coded injury identification of the facial nerve (solution of 0.5 mm) was used. Two different registration methods were examined: (group 1) navigation bow with 4 integrated markers at the upper jaw; (group 2) landmark registration with 4 titanium micro screws. An optical navigation system was used. The targets were illustrated by 3 titanium screws within the range of the planum m…

Tympanoplasty, with or without mastoidectomy, is highly effective for treatment of chronic otitis media in children.

CONCLUSION: The overall success rate of tympanoplasty, with or without mastoidectomy, in the treatment of chronic pediatric otitis media, was high and did not depend on patient age, the status of the contralateral ear, the inclusion or absence of surgical mastoidectomy, or the method of mastoidectomy (when this procedure was employed). Tympanoplasty may be expected to improve hearing in cases of chronic otitis media accompanied by perforation, but not in cases of cholesteatoma. OBJECTIVES: This study analyzed the clinical features of pediatric patents with chronic otitis media undergoing tymp…

Gold weight implantation for rehabilitation of the paralysed eyelid.

Patients with complete facial nerve palsy are at risk of severe eye complications due to corneal exposure. Gold weight implantation improves function, cosmetic appearance and morbidity. Three patients with nerve palsy underwent insertion of precisely prepared gold weights between May 2000 and December 2001. Individual gold weights were implanted into a small pocket between the orbicularis oculi and the tarsal plate of the upper eyelid, fitting the curvature of the eye. According to follow-up examinations (after at least 5 years), none of the gold weights had extruded; all patients experienced

[Wegener's granulomatosis (WG) presented with hearing loss and without positive serologic ANCA].

BACKGROUND: Wegener's granulomatosis (WG) is a granulomatous inflammation involving the upper and lower respiratory tract and necrotizing vasculitis affecting small to medium-sized vessels. In contrast to a generalised WG with glomerulonephritis initial or isolated forms of the upper respiratory tract may be a diagnostic challenge. PATIENT: We report the case of a 33 year old man with clinical signs of a limited WG exhibiting an imminent irreversible hearing loss, negative PR3-ANCA (anti neutrophil cytoplasmic antibodies with proteinase 3 as target) in serum and an ambiguous histology. Postoperative vestibular-evoked myogenic potentials in cases with vestibular schwannomas.

Although still the subject of discussion, vestibular-evoked myogenic potentials (VEMPs) have been considered to reflect the function of the saccular and, more recently, the cochlear tracts. To accurately determine the precise afferent pathway carrying VEMPs, we studied the outcomes of VEMPs and other examinations in patients with unilateral vestibular schwannomas. Eleven patients with unilateral vestibular schwannomas resected using a middle cranial fossa approach were included in the study. Patients underwent pure-tone threshold audiometry, caloric tests and analysis of auditory brainstem res
