Risk factors for failing to achieve improvement after anatomic total shoulder arthroplasty for glenohumeral osteoarthritis.

BACKGROUND: Although anatomic total shoulder arthroplasty (TSA) successfully improves pain and function, not all patients improve clinically. This study was conducted to determine patient-related factors for failure to achieve improvement after primary TSA for osteoarthritis at 2 years postoperatively. METHODS: This prospective study reviewed an institutional shoulder registry for consecutive patients who underwent primary TSA for osteoarthritis from 2007 to 2013 with baseline and 2-year postoperative American Shoulder and Elbow Surgeons (ASES) Standardized Shoulder Assessment Form scores. A

Are the Pathologic Features of Enthesopathy, Tendinopathy, and Labral and Articular Disc Disease Related to Mucoid Degeneration? A Systematic Review.

BACKGROUND: Tendinopathy, enthesopathy, labral degeneration, and pathologic conditions of the articular disc (knee meniscus and ulnocarpal) are sometimes described in terms of inflammation or damage, while the histopathologic findings are often consistent with mucoid degeneration. A systematic review of the histopathology of these structures at diverse locations might reconceptualize these diseases as expected aspects of human aging. The potential benefits of this evolution might include healthier patient and clinician mindsets as well as a reduced likelihood of overdiagnosis and overtreatment

The association between Interleukin-6 rs1800795/rs1800797 polymorphisms and risk of rotator cuff tear in a Chinese population.

Expression of proinflammatory cytokines, such as interleukin (IL)-6 (IL-6) and metalloproteases, are elevated in patients with rotator cuff tear (RCT). In order to investigate the role of IL-6 gene polymorphisms on RCT risk, we genotyped two SNPs on IL-6 gene (rs1800795 and rs1800797) in 138 RCT patients and 137 healthy controls using polymerase chain reaction (PCR) and Sanger sequencing. The IL-6 expression in shoulder joint synovial fluid was determined by using enzyme-linked immunosorbent assay (ELISA) method. The constant score and visual analog scale (VAS) were used to evaluate the clinic

Plate Positioning Affects the Pressure on the Axillary Nerve Following a Deltopectoral Approach.

PURPOSE OF THE STUDY To investigate the pressure levels on the axillary nerve at different plate positions after plate fixation of a proximal humerus fracture. MATERIAL AND METHODS Eight fresh-frozen cadaveric specimens were used. The plates were applied on the lateral side of the humerus. A FlexiForce pressure sensor was placed between the nerve and the plate, and the pressure on the nerve was measured (Group 1). The plates were then placed in two different positions such that distal portion of the plates would have 30° anterior and 30° posterior angles to the anatomical axis of the humerus (

Preoperative GLP-1 Receptor Agonist Use Is Not Associated With Complications in Non-Obese Patients Undergoing Total Shoulder Arthroplasty: A Propensity Score Matched Large Database Analysis.

BACKGROUND: As the demand for total shoulder arthroplasty (TSA) increases, it is imperative to optimize preoperative risk factors. While glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are used for glycemic control or weight management in patients with obesity or diabetes, the impact of preoperative use on postoperative TSA outcomes in non-obese patients is unknown. PURPOSE: We sought to answer the question: Does preoperative use of GLP-1 RAs lead to increased risk of medical or surgical complications after TSA? METHODS: We conducted a retrospective cohort study using data from the Tri

Phenome-wide comorbidity network analysis reveals clinical risk patterns in enthesopathy and enthesitis.

BACKGROUND: Enthesopathy and enthesitis, including rotator cuff disease and other tendon disorders, represent a heterogeneous group of musculoskeletal conditions with complex etiologies. Understanding how systemic health profiles influence their onset remains a critical challenge in musculoskeletal medicine. METHODS: We conducted a large-scale, phenome-wide comorbidity analysis using longitudinal electronic health records (EHR) from 432,757 UK Biobank participants. Incident cases of peripheral enthesopathies were compared to controls across 434 baseline disease phenotypes. A directed ego netw

CCL4L2 participates in tendinopathy progression by promoting macrophage inflammatory responses: a single-cell analysis.

BACKGROUND: Tendinopathy is very common in clinical practice, which is highly prevalent in athletes, sports enthusiasts and other people involved in high-load weight-bearing activities. Common types of tendinopathy include rotator cuff injury, Achilles tendinitis, tennis elbow and so on. Macrophages (Macs) are key immune cells in the pathogenesis of tendinopathy. In this study, CCL4L2+ M1-related signaling pathways were screened by combining single-cell RNA sequencing (scRNA-seq) to explore their significance in tendinopathy treatment. METHODS: Immune cell populations were screened by Uniform

Si-ITGA6-Loaded Liposomes Inhibit Capsule Fibrosis via the FAK/PI3K/Akt Signaling Pathway in Adhesive Capsulitis of Shoulder.

BACKGROUND: Adhesive capsulitis of shoulder (ACS) is a common shoulder disease with pain, joint stiffness, and capsule fibrosis. The pathogenesis of ACS remains unclear, and long-term efficacy of current treatment for ACS is not satisfactory. Given the absence of targeted therapy, proteomic analysis is conducted on the capsules of ACS patients, in an effort to identify the potential target for ACS treatment. The liposome-based siRNA delivery system was utilized for in vivo experiments due to its non-immunogenicity, stability, and low off-target effects compared to other siRNA delivery methods.

Revisiting open capsuloplasty for the treatment of anterior shoulder instability: 35-year follow-up of the Du Toit procedure.

The Du Toit open capsuloplasty for the treatment of anterior shoulder instability is based on the concept of restoring joint stability by recreating the integrity of the anterior glenoid labrum and inferior gleno-humeral ligament using staples. The long-term validity of this procedure for the treatment of anterior shoulder instability was retrospectively assessed by a clinical or telephone interview and radiographic evaluation in 58 patients with an average 35-year follow-up between 1948 and 1974. The range of motion was evaluated by comparing the treated side with the contralateral one; subje

Influenza Vaccination Is Not Associated with Increased Number of Visits for Shoulder Pain.

BACKGROUND: Shoulder injury from vaccination was approved for automatic compensation from the Vaccine Injury Compensation Program (VICP)-a federal government program started in 1988 to shield the manufacturers of childhood vaccines from liability. The approval was made on the basis of case reports rather than experimental evidence. This, combined with the addition of influenza vaccination to the VICP in 2005 (which broadened coverage to include adults) and other social factors, was associated with a rapid rise in the number of claims of shoulder injury from vaccination over the last decade, wh

Multifactorial approach training for anterior shoulder instability after a Latarjet procedure: protocol for a randomised controlled trial.

INTRODUCTION: The Latarjet procedure is the mainstay treatment in high-demand patients with substantial glenoid bone loss or after failed capsulolabral repairs. Patients typically return to sport (RTS) within 6 months postoperatively, requiring intensive rehabilitation. Current rehabilitation protocols focus on mobility, strength and stability. Yet, psychological factors, such as fear of reinjury, are the main reason not to RTS. Therefore, this study aims to determine whether integrating psychological interventions into postoperative rehabilitation improves patient-reported shoulder function c
