Mid-term Patient-reported Outcomes After Lateral Versus Crossed Pinning of Pediatric Supracondylar Humerus Fractures.

BACKGROUND: Closed reduction and percutaneous pinning in a crossed or lateral configuration is the standard treatment for supracondylar humerus (SCH) fractures. We compared mid-term patient-reported outcomes (PROs), radiographic outcomes, and complication rates between patients treated with crossed versus lateral pinning. METHODS: We reviewed 508 pediatric patients treated surgically for Gartland type-III SCH fractures from 2008 to 2017. We included patients aged 5 to 17 years at the time of telephone interviews, who had available radiographs. We excluded those unable to be reached by telepho

Effect of fracture levels on the strength of bone-implant constructs in subtrochanteric fracture models fixed using short cephalomedullary nails: A finite element analysis.

OBJECTIVES: This study was conducted to investigate the stress around nails and cortical bones in subtrochanteric (ST) fractures fixed using short cephalomedullary nails (CMNs) in finite element models (FEMs) and to determine the appropriate short CMN type for different fracture levels. METHODS: The following three types of short CMNs were used: type A, which is 170 mm in length and has 1 distal locking screw; type B, 200 mm in length and 1 distal screw; and type C, 200 mm in length and 2 distal screws. A total of 24 FEMs were tested on a transverse ST fracture at 8 levels [0, 10, 20, 25, 30,

Recommendation of use of checklists in tibial intramedullary nail removal: Retrospective study of mechanical complications related to nail removal.

BACKGROUND: The removal of implants such as intramedullary nails is one of the most common operations in orthopedic surgery. The indications for orthopedic implants removal will always remain a subject of conversation and hardly supported by literature. The aim of this study to report injuries of treatment in tibial nail removal and to determine if there are fracture characteristics, patient demographics, or surgical details that may predict a complication. METHODS: This is a retrospective seven-year (2010-2016) study including a total of 389 tibial intramedullary nail removals at the Helsink

Is calcar referenced tip-apex distance a better predicting factor for cutting out in biaxial cephalomedullary nails than tip-apex distance?

PURPOSE: To test the significance of calcar referenced tip-apex distance (CalTAD) and the length of anti-rotation screw (AR screw) as predictors for failure after biaxial cephalomedullary (CM) nailing of intertrochanteric fractures. METHODS: We retrospectively reviewed 190 consecutive fractures that had undergone biaxial CM nailing. Of these, 67 met the inclusion criteria of a non-pathological fracture with a minimum of 90 days radiological follow-up (mean 458 days; 91 days to 4.9 years). RESULTS: The overall failure rate was 15% (10 of 67). Failure was associated with a higher CalTAD in mos

Fat embolism syndrome after nailing an isolated open tibial fracture in a stable patient: a case report.

BACKGROUND: Fat embolism syndrome is a potentially fatal complication of long bone fractures. It is usually seen in the context of polytrauma or a femoral fracture. There are few reports of fat embolism syndrome occurring after isolated long bone fractures other than those of the femur. CASE PRESENTATION: We describe a case of fat embolism syndrome in a 33-year-old Caucasian man. He was being seen for an isolated Gustilo's grade II open tibial fracture. He was deemed clinically stable, so we proceeded to treat the fracture with intramedullary reamed nailing. He developed fat embolism syndrome

[Postoperative malrotation after closed reduction and intramedullary nailing of the femur: a retrospective 5-year analysis].

BACKGROUND: Minimally invasive treatment of diaphyseal femur fractures (DFF) with closed reduction and intramedullary nailing is a well established procedure. However, a femoral malrotation after intramedullary nailing is considered to be a substantial problem. Studies have described femoral malrotation (FMR) in 17-35 % after this procedure. Computed tomography (CT) of both femora is accepted as an objective, reproducible measurement method to determine a postoperative femoral malrotation. An anatomic reposition of the centreline of the femur remains of high importance since a malrotation > 15

Subtrochanteric fractures: treatment with cerclage wire and long intramedullary nail.

Fracture malalignment and nonunion are not infrequent after treating subtrochanteric fractures with intramedullary nails. The use of a cerclage wire with a minimally invasive approach to aid and maintain reduction in certain subtrochanteric fracture patterns can be an effective surgical strategy to improve outcome. It allows the surgeon to obtain and maintain an anatomic reduction with more bone contact, which will aid in fracture consolidation. This has the added advantage of optimizing the greater trochanteric starting point. It minimizes malreductions of the proximal femoral fragment, and,

A simple reduction technique in intramedullary nailing (MICRONAIL) of distal radius fracture - intrafocal elastic-spring pinning method.

On treating distal radius fracture with intramedullary nailing (MICRONAIL, Wright Medical), completed anatomical reduction is indispensable before inserting the nail. However, complete reduction in percutaneous pinning or in Kapandji intrafocal pinning is troublesome. Thus, majority of the operation time is usually spent on reducing the fracture appropriately. We have commenced the "intrafocal elastic-spring pinning" as a reduction technique and have simplified the reduction prior to nailing. First, curve the 1.5 mm Kirschner wire properly. Next, pins are introduced percutaneously and intrafoc

Rationale for reduced pressure reaming when stabilizing actual or impending pathological femoral fractures: a review of the literature.

Surgical treatment for impending or actual pathologic femoral fractures due to metastatic carcinoma has been shown to improve pain and quality of life. Surgery usually involves manipulation of the intramedullary (IM) portion of the femur to provide adequate stability and to restore function. IM nailing is generally considered the preferred treatment over plate osteosynthesis for most areas of the femur due to load sharing capabilities. Several reports have shown a high incidence of oxygen desaturation, hypotension, fat or air embolism, and mortality in the surgical treatment of femoral metasta

Geographic variation in device use for intertrochanteric hip fractures.

BACKGROUND: Hip fractures in the elderly are a common and costly problem, with intertrochanteric fractures accounting for almost half of these fractures. Most intertrochanteric fractures are treated with either a plate-and-screw device or an intramedullary nail device. We assessed the degree of geographic variation in use of intramedullary nailing for intertrochanteric femoral fractures among Medicare beneficiaries between 2000 and 2002. METHODS: Medicare 100% files (hospital and physician claims, and enrollment) for 2000 through 2002 were used to identify beneficiaries, sixty-five years of a

Risk of superior gluteal nerve and gluteus medius muscle injury during femoral nail insertion.

BACKGROUND: Abduction weakness and limping is a well-recognized complication of closed antegrade insertion of femoral nails. Iatrogenic injuries to the superior gluteal nerve and the gluteus medius muscle are the most likely contributing factors. The purpose of this study of cadavers was to assess the risk of nerve and muscle injury with various lower-limb positions used during nail insertion. METHODS: We studied thirteen hips of ten formalin-fixed adult cadavers. With the cadaver in the full lateral position, a 9-mm reamer was introduced in a retrograde fashion from the intercondylar notch a
