Analysis of the anatomic changes of the aging facial skeleton using computer-assisted tomography.

PURPOSE: The bony skeleton serves as the scaffolding for the soft tissues of the face; however, age-related changes of bony morphology are n … el throughout adulthood. Relative to the globe, the facial skeleton appears to rotate such that the frontal bone moves anteriorly and inferiorly while the maxilla moves posteriorly and superiorly. This rotation causes bony angles to become more acute and likely has an effect on the position of overlying soft tissues. These changes appear to be more dramatic in …

Skeletal Stabilization After Sagittal Split Osteotomy Using a Biodegradable Osteosynthesis System: A Pilot Study.

Few studies have demonstrated the reliability of biodegradable osteosynthesis systems in orthognathic surgery; however, studies on the use of biodegradable osteosynthesis systems after mandibular osteotomy, including sagittal split ramus osteotomy (SSRO), are limited. This pilot study aimed to compare the safety and skeletal stability after segmental fixation using curved titanium and box-type biodegradable systems in SSRO for mandibular prognathism. Patients who underwent SSRO for correction of mandibular protrusion with malocclusion between September 2024 and March 2025 were included. After

Zygomatic pretemporal skull base approach for resection of a radiation-induced sphenoid wing meningioma: surgical technique.

In this video, we present the surgical technique and operative nuances of the zygomatic pretemporal skull base approach for resection of a large left sphenoid wing and middle cranial fossa radiation-induced meningioma (RIM) with invasion and encasement of the middle cerebral artery (MCA). RIMs represent a distinct and surgically challenging entity due to aggressive biological behavior, altered tissue planes, and frequent vascular involvement. In the present case, prior childhood cranial irradiation resulted in dense tumor adherence to the MCA within radiation-altered tissue, necessitating meti

In-hospital unexpected cerebral herniation-New neurosurgical quality control standards: A case report.

RATIONALE: In-hospital unexpected cerebral herniation (IHUCH) refers to abrupt, unanticipated cerebral herniation in patients who were previously considered stable. Its sudden onset may lead to delayed recognition and poor outcomes. PATIENT CONCERNS: A 50-year-old man was transferred to our institution after a motor vehicle collision. The patient's Glasgow Coma Scale score remained between 13 and 14 upon admission. At the sixth day, without warning, he acutely lost consciousness, dropping to Glasgow Coma Scale 6, with left pupillary dilatation and loss of light reflex. DIAGNOSES: Initial com

Disease-specific gait deviations in pediatric patients with X-linked hypophosphatemia.

BACKGROUND: X-linked hypophosphatemia (XLH) represents the most common genetic form of rickets featuring profound hypophosphatemia with associated skeletal and non-skeletal manifestations. Early onset gait disturbances contribute strongly to the burden of disease. However, no study has comprehensively characterized naturally occurring gait deviations in pediatric patients with XLH. RESEARCH QUESTIONS: Can disease-specific gait deviations and potentially influencing factors be identified by gait analysis in non-surgically treated children with XLH? METHODS: Gait laboratory assessments of 12 p

Visual outcome after fronto-temporo-orbito-zygomatic approach combined with early extradural and intradural optic nerve decompression in tuberculum and diaphragma sellae meningiomas.

OBJECT: The surgical challenge of the treatment of tuberculum (TSMs) and diaphragma sellae meningiomas (DSMs) is to preserve or improve the visual function. Extradural and intradural optic nerve decompression should reduce surgical trauma of the nerve achieving a good visual result. METHODS: We reported 37 consecutive TSMs and DSMs operated through fronto-temporo-orbito-zygomatic approach with extradural unroofing of the optical canal and early intradural incision of the dural sheath. Visual data were recorded measuring the visual impairment score (VIS), the visual acuity (VA), the visual fie

[Fracture of macroporous hydroxyapatite prosthesis].

Different prosthesis implants are offered to perform a cranioplasty after a decompressive craniectomy when autologous bone graft cannot be used. The authors report the case of a 25-year-old man who benefited a unilateral decompressive craniectomy after a severe head trauma. Seven months later, a cranioplasty using custom macroporous hydroxyapatite prosthesis was performed. The postoperative course was marked by a generalized seizure leading to a traumatic head injury. The CT-scan showed a comminutive fracture of the prosthesis and an extradural hematoma. The patient underwent a removal of the

Comparison of 1-point fixation with 2-point fixation in treating tripod fractures of the zygoma.

PURPOSE: Zygomatic tripod fracture is relatively common and generally requires open reduction-internal fixation through several incisions. However, lateral eyebrow incisions have sometimes left unsightly scars, and thus we have used 1-point fixation through a buccogingival incision so as not to leave scars in selected cases. The aim of this study was to compare 1-point fixation in the zygomaticomaxillary (ZM) area with 2-point fixation in the ZM and frontozygomatic (FZ) areas in tripod fractures. MATERIALS AND METHODS: This study is retrospective and was approved by the institutional review b

Examine your orofacial cleft patients for Gorlin-Goltz syndrome.

The Gorlin-Goltz syndrome is characterized by four primary symptoms: multiple nevoid basal cell epitheliomas that usually undergo malignant transformation; jaw keratocysts that show constant growth; skeletal anomalies; and intracranial calcifications. A myriad of additional findings may also be noted. Among the most frequent are: palmar and plantar pits, a characteristic flattened facies and broad nasal root, frontal and parietal bossing, mandibular prognathia, hypertelorism, strabismus, dystrophia of the canthi, and clefts of the lip, alveolus, and/or palate. In this study, we review the lite

The characteristics of midfacial fractures and the association with ocular injury: a prospective study.

Ocular injuries commonly occur in patients with facial fractures. This prospective study was set up to determine the incidence of ocular injuries, as assessed by an ophthalmologist, in patients who had sustained midfacial fractures. Over a 2-year period, a study of 363 patients who had sustained midfacial trauma sufficient to lead to a facial bone fracture (438 fractures) was undertaken and patients received a comprehensive examination by an ophthalmologist and an orthoptist within 1 week of injury. The characteristics of the eye injuries sustained were related to the aetiology of the fracture
