Smoking and tympanoplasty: implications for prognosis and the Middle Ear Risk Index (MERI).

OBJECTIVES/HYPOTHESIS: The objectives of this study are to review the effects of smoking on preoperative middle ear disease severity, long-term surgical outcome, type and extent of surgery required, the need for ossicular chain reconstruction, and the long-term hearing results. STUDY DESIGN: A retrospective chart review. MATERIALS AND METHODS: The charts of 74 smokers and non-smokers who underwent over-under tympanoplasty were reviewed. An analysis of the disease severity (using the Middle Ear Risk Index [MERI]) at presentation and type of surgery was performed. A review of graft take and de

The transmastoid partial labyrinthectomy approach to medial skull base lesions.

INTRODUCTION: It has long been thought that surgical disruption of the membranous labyrinth invariably results in sensorineural hearing loss and balance dysfunction. Recent evidence suggests that the inner ear can withstand such manipulation without loss of function. The technique of transmastoid partial labyrinthectomy has recently been described as a means of providing access to lesions of the medial skull base by removing part of the labyrinth and at the same time attempting to preserve hearing and vestibular function of the lateral semicircular canal (LSCC) and otolithic organs. PROCEDURE…

Bilateral simultaneous hearing preservation mastoidectomy in otogenic meningitis.

Bilateral simultaneous hearing conservation mastoidectomy has not been previously documented. We present our experience with this procedure. Three patients with pyogenic meningitis secondary to bilateral unsafe ears had hearing preservation surgeries of both ears in a single sitting. Although ear surgery carries risk of inner ear damage, in our series there was no deterioration of inner ear function in the 6 ears operated on. The air conduction threshold improved in 3 (50%) of the ears. When meningitis results from bilateral otogenic foci as in our series, simultaneous mastoidectomy in both ea…

[Endocranial complications of acute and chronic otitis media in children and adolescents].

BACKGROUND: After introduction of antibiotics endocranial complications in otitis media are less common, but there are still diagnostic and therapeutic problems. PATIENTS: 124 patients (5 months-16 years) underwent mastoidectomy. The number, type and therapy of otitic complications is presented. RESULTS: Nine percent of the patients showed up with endocranial complications. Besides lateral sinus thrombosis (five patients), meningitis (five patients) and epidural abscess (five patients), one cerebral and one cerebellar abscess was found. Four patients developed more than one complication. TH…

Prefabricated ossicular homografts in middle ear reconstructions.

This paper is a review of 132 cases of ossicular reconstruction utilizing prefabricated homografts by clinical staff members of the UCLA Div … a resident training programme. The cases in this review can be divided into 3 basic categories: (1) mastoidectomy ossicular reconstruction; (2) exploratory tympanotomy with reconstruction; (3) stapedectomy. This study indicates that ossicular homografts are very well tolerated in their new transplanted positions. Our very low extrusion rate, extremely rar …

Fusobacterium necrophorum Meningitis 27 Pediatric Cases From a French National Observatory.

BACKGROUND: Fusobacterium necrophorum is primarily involved in ear, nose and throat (ENT) infections, and exceptionally causes meningitis. We aimed at describing specific clinical features associated with cases of F. necrophorum meningitis. METHODS: Cases of F. necrophorum meningitis were prospectively collected by a network of 259 pediatric wards covering 60% of French pediatric wards between 2001 and 2024. Children were categorized in 2 groups: "isolated" meningitis and "complicated" meningitis, defined by ≥1 local or systemic complication (mastoiditis, subperiosteal or epidural abscess, Le…

Other notable conditions included gingivitis/periodontitis, sinusitis/rhinosinusitis, mastoiditis, and dental abscesses, albeit at lower frequencies. Comparison of the Extended Perichondrium-Cartilage Butterfly Inlay Technique and Over-Underlay Technique for Repairing Subtotal Perforation: A 2-Year Follow-up Study.

High-resolution computed tomography (HRCT) demonstrated well-pneumatized mastoids and middle ears in both groups. Altered taste was reported in no PCBI group patients and 23.7% (9 patients) of the PCOU group patients (P < .05). Graft cholesteatoma was found in 4.9% (two patients) in the PCBI group and 7.9% (three patients) in th …

Haemophilus influenzae purulent pericarditis in an immunocompetent individual.

Purulent pericarditis is a rare bacterial illness in the post-antibiotic era that is defined as pericardial infection with gross or microsco … ve case. Sequalae from H. influenzae infection is usually limited to upper respiratory symptoms and mastoiditis, but rarely, pericarditis could occur. It is important to include pericarditis in the differential for chest pain in a patient with a recent history of upper respiratory symptoms. Pericarditis is a rare but potentially serious complication of re …

Chondromyxoid fibroma of the temporal bone: A rare case report.

RATIONALE: Chondromyxoid fibroma (CMF) is a rare form of benign bone tumor and easily misdiagnosed as fibrosarcoma. Hence, to explore the cl … f the imaging examination of the patient, a lesion occupying the temporal bone in the right ear and mastoiditis in the right middle ear was initially diagnosed. INTERVENTIONS: Right ear temporal bone tumor resection and abdominal fat extraction were conducted. OUTCOMES: Postoperative pathological results demonstrated myxoid fibroma of the temporal bone …

Comparison of first year of life acute otitis media admissions before and after the 13-valent pneumococcal conjugate vaccine.

BACKGROUND: Acute otitis media (AOM) is a common childhood infection, which is usually managed in the outpatient setting. Yet, the more severe cases are referred for inpatient treatment. We hypothesized that pneumococcal conjugate vaccines (PCVs), administered during the first year of life, would decrease AOM admissions rate in this age group. We studied the characteristics of infants admitted with AOM and acute mastoiditis (AM) in the PCV13 era, routinely given from November 2010 to all infants. METHODS: Charts of infants ≤1 year that were hospitalized during 1/1/2010-31/12/2015 with AOM, wi…

Results of VSB implantation at the short process of the incus in children with ear atresia.

OBJECTIVE(S): To describe a new and more simple surgical procedure for implanting the Vibrant Soundbridge (VSB) on the short process of the … the stapes. VSB coupling to the long process of the incus or the stapes may be challenging in small mastoids. Coupling to the short process of the incus is comparatively a simple procedure, since there is no need for posterior tympanotomy and has no proximity to the facial nerve. The results of this study are similar to other ear atresia cases in the lite …

Pressure buffering by the tympanic membrane. In vivo measurements of middle ear pressure fluctuations during elevator motion.

OBJECTIVES: The tympanic membrane (TM) represents a pressure buffer, which contributes to the overall pressure regulation of the middle ear … ME function as well as for ME modeling. The TM buffer capacity was found more efficient in smaller mastoids. Possible clinical implications are discussed.
