Patient-controlled analgesia in the management of postoperative pain.

Patient-controlled analgesia (PCA) is a delivery system with which patients self-administer predetermined doses of analgesic medication to relieve their pain. Since its introduction in the early 1980s, the daily management of postoperative pain has been extensively optimised. The use of PCA in hospitals has been increasing because of its proven advantages over conventional intramuscular injections. These include improved pain relief, greater patient satisfaction, less sedation and fewer postoperative complications. All PCA modes contain the following variables: initial loading dose, demand dos

Strategies for rehabilitation management with implants in patients with down syndrome: a scoping review.

BACKGROUND: Down Syndrome (DS), caused by an extra chromosome 21, has a prevalence of 24.7 per 10,000 live births in Chile, the highest in Latin America. Individuals with DS commonly present orofacial and dental anomalies, complicating oral health management. Many depend on removable prostheses, which represent challenges in hygiene, handling, and adaptation. Dental implants are a promising alternative, offering improved stability and functionality. However, successful rehabilitation requires addressing specific anatomical, physiological, and behavioral considerations. This scoping review comp

INTRODUCTION: Distal radius fractures are the most common fractures seen in the emergency department in children in the USA. However, no established or standardised guidelines exist for the optimal management of completely displaced fractures in younger children. The proposed multicentre randomised trial will compare functional outcomes between children treated with fracture reduction under sedation versus children treated with simple immobilisation. METHODS AND ANALYSIS: Participants aged 4-10 years presenting to the emergency department with 100% dorsally translated metaphyseal fractures of

Outcomes of tympanostomy tube insertion in the emergency department versus the operating room for acute mastoiditis in children.

BACKGROUND: Acute mastoiditis is a complication of acute otitis media in children that often requires surgical intervention. The increasing availability of procedural sedation in pediatric emergency departments (PED) means that some surgical procedures can be performed in the PED rather than in the operating room (OR), but the clinical outcomes of this practice remain unclear. OBJECTIVE: To compare the outcomes of Tympanostomy Tube (TT) insertion for acute mastoiditis performed in the PED vs. in the OR in terms of complications, readmissions, and effectiveness. METHODS: A retrospective obser…

Minimally Invasive Controlled Growing Rods for the Surgical Treatment of Early-Onset Scoliosis-A Surgical Technique Video.

BACKGROUND: Spinal deformities in children and adolescents can be easily divided into those occurring and diagnosed before the age of 10-early-onset scoliosis-and those occurring and diagnosed after the age of 10-late-onset scoliosis. When the curvature continues to progress and exceeds a Cobb angle of more than 60-65 degrees, surgical treatment should be considered. The most common treatment procedure for EOS is the surgical correction of the deformity using standard growing rods (SGRs), and in the case of congenital defects with additional hemivertebrae, it is the resection of the hemiverteb

Virtual reality-assisted distraction during transcatheter aortic valve implantation under local anaesthesia: A randomised study.

BACKGROUND: A minimal approach, using local anaesthesia alone, has been advocated to promote faster transcatheter aortic valve replacement (TAVR) procedures in intermediate-risk patients. Pre- and periprocedural anxiety and pain remain a concern. Virtual reality (VR) is a form of non-pharmacological distraction that can potentially modulate pain and anxiety. This randomised study explored whether VR reduces pain and anxiety during TAVR without sedation and compared the effects of VR with those of standard care. METHODS AND RESULTS: Between June 2022 and March 2023, 207 patients underwent tran
